SAMENVATTING
Levertransplantatie is geïndiceerd bij alle leverziektes die een negatieve invloed hebben op de levensverwachting en / of een ernstige aantasting geven van de kwaliteit van leven, die definitief of voor lange duur verholpen kunnen worden door levertransplantatie, en die niet op andere wijze effectief zijn te behandelen. Technisch gezien zijn er meerdere mogelijkheden voor levertransplantatie. Met name op de kinderleeftijd bestaan alternatieven voor de conventionele postmortale full-size donor-lever: levende donor-levertransplantatie van het linkslaterale segment of een leverkwab, split levertransplantatie (segmenten 2, 3 en eventueel 4). De indicatiestelling, evaluatie van donoren en ontvangers zijn in de desbetreffende protocollen vastgelegd. Voor de patiëntenselectie, de indicatiestelling en timing van levertransplantatie is in de centra een multidisciplinair behandelteam verantwoordelijk.

TOEPASSING
3.1
INDICATIE, SELECTIE EN TIMING VOOR LEVERTRANSPLANTATIE
3.1.1
Indicatie (diagnosecategorieën)
In Tabel I treft u een overzicht aan van de diagnose categorieën waarvoor in Nederland levertransplantaties worden verricht.

3.1.2
Verwijzing en selectie van patiënten, contra-indicaties
De verwijzing van patiënten met een diagnose passend in een van de hoofdgroepen van Tabel I naar een levertransplantatiecentrum vindt plaats door de eigen huisarts of specialist. Voor patiënten < 16 jaar vinden levertransplantaties momenteel in Groningen plaats, vanaf 16 jaar vinden levertransplantaties in Leiden, Groningen en Rotterdam plaats. Verwijzing vindt plaats naar een hepatoloog (maag / darm / leverziekten) of een kinderhepatoloog van één van de transplantatiecentra. Deze beoordeelt, klinisch of poliklinisch, op grond van de meegestuurde informatie en eigen onderzoek of de leverziekte past in de in Tabel I genoemde algemeen aanvaarde indicaties voor levertransplantatie. De hepatoloog verzamelt ook de gegevens die noodzakelijk zijn om, op gestandaardiseerde wijze, de ernst van de leverziekte vast te stellen, om daaruit de levensverwachting van de patiënt te kunnen bepalen. De ernst van de leverziekte en de progressie ervan wordt met behulp van internationaal geaccepteerde criteria vertaald in een prognose betreffende de levensverwachting en de kans op eenjaars-overleving. De modellen die hiervoor worden gebruikt zijn vermeld in Tabel II. Tabel II: Modellen in gebruik voor de schatting van de prognose

Indien de 1-jaars levensverwachting en / of -kwaliteit zonder OLT slechter is dan met OLT is de patiënt kandidaat voor levertransplantatie (OLT). Belangrijk is echter ook de ontwikkeling van de Child-Pugh-, MELD- en PELD-score in de tijd: een oplopende Child-Pugh score die de 9 nog niet bereikt heeft vormt ook reden voor overleg met een transplantatiecentrum. Dit geldt zeker bij Child-Pugh klasse C en bij B met progressie, dus child score > 8. De MELD-, PELD- en ziektespecifieke scores kunnen ook behulpzaam zijn. De (kinder-)hepatoloog beoordeelt, zonodig in samenspraak met andere specialisten zoais anesthesisten, longartsen, chirurgen, nefrologen, psychiaters e.d., de algemene conditie van de patiënt en eventuele evidente contra-indicaties tegen de voorgenomen behandeling. De absolute en relatieve contra-indicaties staan vermeld in Tabel IV. Bij absolute contra-indicaties hoeft niet met een transplantatiecentrum overlegd te worden. In alle andere gevallen wel. 3.1.3
Timing van de levertransplantatie
Als de indicatie voor levertransplantatie is bevestigd, de ernst van de leverziekte is vastgesteld en de kwaliteit van leven is beoordeeld, zal moeten worden vastgesteld op welke termijn de ingreep zal moeten worden verricht. Deze timing is enerzijds gebaseerd op de prognose van de leverziekte van de betreffende patiënt en anderzijds op de bekende overlevingspercentages van levertransplantaties verricht voor de leverziekte van de patiënt. Afhankelijk van het ziektebeeld en de bepaalde prognose wordt patiënt volgens bij de NTS vastgelegde criteria in één van de vier medische urgentiecategorieën op de wachtlijst geplaatst voor levertransplantatie (zie Tabel V en bijlage voor de urgentiecategorieën). Wanneer de kansen van de patiënt beter worden met een transplantaat dan met zijn eigen lever is het moment van de transplantatie aangebroken. De hepatologen die betrokken zijn bij de selectie van deze patiënten zullen dit moment moeten inschatten. In de ideale situatie worden de patiënten zo'n twaalf (6-18) maanden voor het bereiken van deze situatie op de wachtlijst voor transplantatie geplaatst. Naast de progressie van de leverziekte zelf bestaan er nog andere factoren die de timing van de transplantatie kunnen beïnvloeden. In de praktijk betekent dit veelal een naar voren schuiven van de plaatsing op de wachtlijst. Deze factoren zijn:

Het is van belang dat de verwijzers de patiënten niet te laat aanmelden bij het transplantatiecentrum. Wanneer patiënten te laat worden verwezen moeten zij in een hogere medische urgentiecategorie op de wachtlijst worden geplaatst, waarbij zij de tijdig aangemelde patiënten passeren. De resultaten van levertransplantaties bij patiënten die te laat zijn aangemeld zijn slechter dan die van patiënten die op tijd (electief) worden getransplanteerd. Daarbij verslechtert de conditie van de patiënten die telkens worden gepasseerd tijdens de onbedoeld verlengde wachtperiode, waardoor hun kansen op succes van de transplantatie afnemen. Het is derhalve van groot belang dat voor de inschatting van het natuurlijk beloop van de leverziekte (de prognose) regelmatig overleg plaatsvindt tussen de verwijzer, het transplantatiecentrum en de patiënt. De gevolgde procedures bij de indicatiestelling, selectie en timing voor levertransplantatie in de transplantatiecentra zijn schematisch weergegeven in figuur 1. -

In sommige gevallen (Budd Chiari syndroom, M. Wilson, autoimmuun hepatitis pathologische leverziekte) spant het erom of met conservatieve behandeling transplantatie kan worden vermeden. In dergelijke gevallen heeft behandeling in een transplantatiecentrum de voorkeur. Voor prognostische modellen zie Tabel II.

3.2
VIRALE HEPATITIS
3.2.1
Patiënten met leverfalen als gevolg van een chronische Hepatitis B- of D-infectie

Alle patiënten ongeacht hun virale status komen in aanmerking voor transplantatie. Bij een hoge virale replicatie wordt (meestal kort) voor de transplantatie gestart met een nucleosi-de analoog. Postoperatief komen alle patiënten in aanmerking voor profylaxe van hepatitis B virus 'recurrence'. De standaard therapie is hepatitis B immunoglobuline in combinatie met een nucleoside analoog, meestal lamivudine. Bij resistentie tegen lamivudine kan een ander nucleoside analoog (bijv. Adefovir-Dipivoxil of Entecavir) in aanmerking komen.
3.2.2
Patiënten met leverfalen als gevolg van een chronische hepatitis C-infectie
De 1-jaarsoverleving van de hepatitis C-patiënt posttransplantatie is vergelijkbaar met die voor cholestatische leverziekten (PBC, PSC), maar de 5-jaars overleving is iets lager. Transplantaatreïnfectie met HCV is vrijwel universeel bij HCV-RNA postieve ontvangers. Zo mogelijk wordt gepoogd HCV voor de transplantatie te 'klaren'. Dit gebeurt momenteel met behandeling met PEG interferon en ribavirin. Histologisch leidt HCV-reïnfectie van het transplantaat bij 50% van de patiënten tot een (meestal milde) chronische hepatitis. 5-10 jaren na transplantatie heeft dit bij ± 10% van de patiënten toch geleid tot meer of minder ernstige vormen van fibrose in het transplantaat en zelden leverfalen; bij het merendeel van de patiënten dus niet. Op grond van de huidige gegevens is de aanwezigheid van een hepatitis C infectie preoperatief geen contra-indicatie voor transplantatie.

PRIMAIRE BILAIRE CIRROSE, PRIMAIRE SCLEROSERENDE CHOLANGITIS EN BILAIRE ATRESIE (511;38;75-104)
Als indicatie voor levertransplantatie wordt algemeen een Child-Pugh score > 8 gebruikt. Met name bij de chronische cholestatische leverziekten kunnen er echter incidenteel patiënten zijn waarbij de Child-Pugh score < 8 is maar de complicaties (invaliderende moeheid, medicamenteus onbehandelbare jeuk, hepatische osteodystrofie, anderszins onbehandelbare cholangitiden (PSC), gewichtsverlies) de indicatiestelling tot transplantatie positief kunnen beïnvloeden. Bij Primair Biliaire Cirrose (PBC) wordt aanbevolen om in een niet te laat stadium van de ziekte transplantatie te overwegen(100). Dit advies is gebaseerd op een modelmatige benadering van de overleving voor PBC: het zgn. Christensen-model(89) en het in de Mayo-Clinic ontwikkelde model (101-103). In de Internationale literatuur doet vooral het Mayo-Clinic-model opgang(81). De overeenkomsten tussen selectie op basis van de in de eerste alinea omschreven criteria en de selectie op grond van het Mayo-Clinic-model zijn groot. In de prognostische modellen is de hoogte van het serum bilirubine veruit de belangrijkste factor. Voor Primair Scleroserende cholangitis (PSC) is een bruikbaar overlevingsmodel beschikbaar, dat eveneens uit de Mayo-clinic afkomstig is. Zelf berekenen van de prognostische scores voor PSC en PBC is mogelijk op de website: http://www.mayo.edu/ Conversiefactoren: Bilirubine 1 mg/dL = 17.10 mol/L. Creatinine 1 mg/dl + 88.40 umol/l Met name ook bij deze ziekte wordt aanbevolen in een niet te laat stadium van de ziekte levertransplantatie te overwegen. Dit betekent dat galwegcomplicaties een reden kunnen zijn om levertransplantatie te verrichten, nog voor Child-Pugh klasse C bereikt is. Van belang hierbij is, dat PSC als een premaligne ziekte te beschouwen is; op termijn zal ongeveer 15-30%104 van de patiënten een cholangiocarcinoom ontwikkelen. Indien zich een cholangio-carcinoom ontwikkeld heeft is transplantatie momenteel zinloos gezien de hoge kans op recidief. Er bestaat een associatie van PSC met Colitis Ulcerosa en andere vormen van inflammatoire darmziekten. Eventueel moet bij dit geassocieerde ziektebeeld na OLT rekening gehouden worden met de keuze van immunosuppressiva. Biliaire atresie (BA) is de primaire indicatie voor levertransplantatie op de kinderleeftijd (35-50% van alle patiënten met chronisch leverfalen). Door initiële chirurgische therapie (Kasai-operatie) en conventionele behandeling inclusief intensieve voedingstherapie is levertransplantatie soms pas op latere leeftijd noodzakelijk. Echter, op grond van een snelle progressie wordt de indicatie voor levertransplantatie in een grote groep (30%) in het eerste levensjaar gesteld. Gezien de schaarste aan orgaandonoren voor jonge donoren komen deze patiënten met name in aanmerking voor levende donor-levertransplantatie of split-levertransplantatie (zie desbetreffende protocollen). Ook hier is het belangrijk dat een indicatiestelling voor levertransplantatie op tijd geschiedt, omdat bij de meeste patiënten het leverfalen een snel beloop kan nemen. Indicatoren voor een slecht beloop zonder levertransplantatie zijn vooral groeiachterstand, hyperbilirubinemie en portale hypertensie.

3.4
POST-ALCOHOLISCHE CIRROSE (23-29;34;105-114)
De medische criteria zijn niet anders dan voor patiënten met andere chronische leverziekten. De indicatie voor levertransplantatie wordt gesteld, indien er sprake is van een gedecompenseerde levercirrose (Child-Pugh score > 8 punten) ondanks alcoholabstinentie van tenminste 6 maanden vóór plaatsing op de wachtlijst door OLT. Extra aandacht voor cardiomyopathie en hartfalen en mogelijke maligniteiten in orofarynx, slokdarm of longen is vereist. De psychiatrische criteria zijn gericht op het selecteren van patiënten met een zeer laag risico op hernieuwd alcoholmisbruik Bij de selectie voor levertransplantatie bij patiënten met een postalcoholische levercirrose zijn de volgende additionele psychosociale criteria van belang: -

AUTOIMMUUN HEPATITIS (12-16;115-126)
Patiënten met een gedecompenseerde cirrose op basis van auto-immuun hepatitis (AIH) kunnen nog geheel responderen met immunosuppressieve therapie. Levertransplantatie is geïndiceerd bij progressie van de leverziekte ondanks immunosuppressieve therapie. Bij patiënten die onder prednison / imuran progressie tonen naar een Child-Pugh score > 8 is levertransplantatie een behandelingsmogelijkheid met minstens even goede resultaten als bijvoorbeeld bij PBC.

3.6
LEVERTUMOREN
3.6.1
Hepatocellulair carcinoom (HCC) (42-43;127-143)
Een HCC is waarschijnlijk bij een haard in een cirrotische lever bij afbeeldend onderzoek, vooral als het een haard is die in de arteriële fase van contrast (bij CT) aankleurt. Een verhoogd en stijgend alfafoetoproteine ondersteunt de waarschijnlijkheidsdiagnose. Echter, in de helft van de in Nederland gediagnosticeerde HCC's is het alfa foetoproteïne normaal. Veelal is er een virale of alcoholische aetiologie van de cirrose, maar andere aetiologieën (bijvoorbeeld haemochromatose) zijn mogelijk. Soms is er geen cirrose bij een HCC. In algemene zin wordt om reden van de kans op entmetastasen punctie uit de haard afgeraden zonder vooroverleg met het transplantatiecentrum. Bij de in opzet curatieve chirurgische behandeling van patiënten met een histologisch bewezen niet-gemetastaseerd hepatocellulair carcinoom (HCC) spelen meerdere factoren een rol met betrekking tot de therapie van voorkeur. Belangrijke factoren zijn de diameter en het aantal haarden, de aan- of afwezigheid van cirrose, de aetiologie van de cirrose en de lokalisatie van de haard(en): centraal in de lever gelegen, perifeer in de lever gelegen of beide. Levertransplantatie is slechts zinvol bij kleine irresectube HCC's zonder metastasering. In deze gevallen is de 5-jaars overleving na transplantatie redelijk nl. > 50%. 'Klein' wil zeggen: één tumor < 5 cm Ø of maximaal twee tumoren < 3 cm Ø zonder vena portaeinvasie en zonder positieve lymfeklieren. De afmetingen van een HCC worden bij voorkeur vastgesteld met behulp van echografisch onderzoek en een MRI en / of CT-scan. Bij kleine asymptomatische HCC's van 3 cm of minder is waar mogelijk (afhankelijk van lokalisatie en leverfunctie) resectie de therapie van keuze, indien een vrije resectiemarge van 1 cm of meer kan worden verkregen. Een 5-jaars overleving van 50% wordt bereikt bij geselecteerde patiënten met Child-Pugh A levercirrose, een perifere tumor kleiner dan 2 cm en vrije resectieranden. Patiënten met Child-Pugh B of C levercirrose zijn gezien de grote kans op complicaties zelden kandidaat voor grote leverresecties. Portale hypertensie verhoogt het risico van een resectie. Levertransplantatie is dan soms een optie. Samenvattend moet de behandeling van HCC gericht zijn op complete tumorresectie. Dit is te bereiken of door partiële leverresectie of door complete leverresectie bij levertransplantatie (Tabel VII). Welke van deze twee vormen van behandeling de voorkeur verdient in termen van morbiditeit en overleving moet nader worden vastgesteld. Tijdens de wachtperiode wordt elke drie maanden een nieuwe MRI en / of CT-scan van buik + longen en een botscan gemaakt om progressie van de tumor of metastasering vast te stellen en hiermee te bepalen of de patiënt nog aan de criteria voor OLT voldoet. Verder wordt in afwachting van OLT getracht de tumor locaal te behandelen (met RF-ablatie, alcoholinjectie of lasertherapie). Tabel VII: Therapie van voorkeur bij HCC

In bepaalde gevallen kan in plaats van een resectie een RF-ablatie of percutane ethanol-injectie de voorkeur genieten.
3.6.2
Andere levertumoren
Gezien het bijzondere biologische gedrag kan levertransplantatie een reële behandelingsmogelijkheid zijn bij gemetastaseerde endocriene tumoren (apudoma) van het maag-darmkanaal, hepatoblastomen en haemangio endotheliomen. Ook bij niet-resectabele goedaardige tumoren, zoals leverceladenomen en haemangio-men is levertransplantatie soms een optie. Het zeldzame karakter en het incidentele voorkomen binnen de transplantatiecentra van deze tumoren maakt het geven van algemene richtlijnen niet zinvol.
3.7
BUDD-CHIARI SYNDROOM (144-152)
De aandoening kent meestal een langzaam progressief karakter in het beloop van weken of maanden doch kan ook fulminant verlopen. Medicamenteuze behandeling met anticoagulantia, soms thrombolyse, en diuretica wordt gevolgd door verbetering van het klinisch beeld in 20-30% van de gevallen zonder dat causale therapie wordt bedreven. Het therapeutisch beleid wordt bepaald door de klinische presentatie van de patiënt en de restfunctie van de lever. De diagnose wordt gesteld met duplex-echografie en zonodig aangevuld met (MR-) angiografie en/of dynamische CT-scan, en drukmeting in de vena cava. De chirurgische of radiologische behandeling van het BCS heeft tot doel de out-flow van de lever te verbeteren en de leverfunctie te behouden alsmede het gevaar van levensbedreigende bloedingen uit oesophagusvarices te verminderen. Dit kan bereikt worden door drie behandelingsmodaliteiten: -
de portosystemische shunt of TIPS lokale desobstructie en eventueel stenten van de levervene(n) en / of vena cava inferior levertransplantatie

Voor orthotope levertransplantatie bestaan in het geval van BCS in principe 3 indicaties, waarbij steeds de prognose van het primaire lijden niet uit het oog mag worden verloren (figuur II). Het verdient aanbeveling dat de behandeling van het BCSyndroom steeds in of in nauwe samenwerking met een levertransplantatiecentrum plaatsvindt. In vroege stadia van het acuut verlopende BCS is shunting met decompressie van de lever zeker nog te overwegen. Bij een chronische vorm van BCS is met name de residuele functie belangrijk voor de keuze tussen shunt en transplantatie. Een chronisch BCS kan leiden tot een eindstadium leverfalen met icterus, portale hypertensie en encephalopathie. Alvorens histologisch onderzoek te verrichten is het raadzaam in overleg met het transplantatiecentrum te treden. Histologie speelt namelijk géén belangrijke rol in het bepalen van het verdere beleid met betrekking tot shunt of eventuele levertransplantatie. Hierbij is de resterende functie (Child-Pugh score) richtinggevend. Indien na een geslaagde TIPS- of shuntprocedure progressief leverfunctieverval optreedt is orthotope levertransplantatie de behandeling van keuze. Bij een zeer klein percentage van de patiënten kan het BCS snel verlopen, waarbij binnen enkele dagen tot weken het beeld van een acute leverinsufficiëntie ontstaat. Het uitgebreide levercelverval maakt de kans op herstel na een shuntprocedure dan vaak onaannemelijk. Ook in deze zeldzame gevallen is levertransplantatie geïndiceerd.

STOFWISSELINGSZIEKTEN VAN DE LEVER (153-161)
De aangeboren stofwisselingsziekten van de lever vormen een groep leveraandoeningen, waarbij één of meerdere enzymen of celmembraanreceptoren niet door de lever worden aangemakat (Tabel VIII) Tabel VIII: Potentiële indicatie voor levertransplantatie
Bij een aantal van deze aangeboren stofwisselingsziekten is levertransplantatie pas geïndiceerd gedurende de adolescentie. Van deze op zichzelf zeldzame aandoeningen komen patiënten met bijvoorbeeld Alpha-1-antitrypsine deficiëntie, ziekte van Wilson en tyrosinaemie frequent in aanmerking voor levertransplantatie. Bij tyrosinaemie en Alpha-1-antitrypsine deficiëntie speelt het risico op de ontwikkeling van maligniteit in de eigen lever een rol in de indicatiestelling tot transplantatie. Zeldzame aandoeningen als de ziekte van Crigler-Najjar, hyperlipidaemie, protoporfyrie en glycogeen stapelingsziekten kunnen ook een indicatie voor levertransplantatie zijn. Bij mucoviscoidose (CF) kan er een indicatie zijn voor lever+ longtransplantatie, maar ook voor geïsoleerde levertransplantatie. Bij primaire hyperoxalurie is, naast niertransplantatie, ook levertransplantatie noodzakelijk. Een mogelijk alternatief is een preemptieve levertransplantatie. Een deel van deze patiënten ontwikkelt als jong volwassene hetzij complicaties van de primaire aandoening hetzij het eindstadium van een leverfalen. Op grond van de algemene selectiecriteria wordt in het laatste geval de indicatie voor levertransplantatie gesteld. Bij haemochromatose is de prognose na OLT vaak matig ten gevolge van systemisch ziek zijn (bijvoorbeeld cardiaal).

ACUUT LEVERFALEN (1;2;162-175)
De sterfte bij acuut leverfalen is hoog: 65-85% van de patiënten overlijdt ondanks intensieve therapie. Levertransplantatie is een effectieve behandeling voor patiënten met acuut leverfalen. Het is van groot belang dat patiënten tijdig verwezen worden naar een levertransplantatiecentrum. Vaak is intubatie vóór transport verstandig. De beslissing wie en wanneer te transplanteren is moeilijk. Twee prognostische modellen worden gebruikt bij deze besluitvorming: de Clychy criteria en de King's College criteria.
3.9.1
Clichy criteria
De Clichy criteria (Tabel IX), gebaseerd op de mate van encephalopathie en het factor V gehalte, zijn ontwikkeld voor patiënten met acuut leverfalen op basis van een virale hepatitis, maar later door anderen gebruikt bij patiënten met acuut leverfalen met niet-virale oorzaken. Indien er sprake is van een encephalopathie graad III of IV en het factor V gehalte is < 20% (leeftijd < 30 jaar) of < 30% (leeftijd > 30 jaar), wordt gekozen voor transplantatie. Gezien de centrale rol, die de stollingsparameter factor V in dit beslismodel speelt, is het belangrijk geen stollingsfactoren te supleren, tenzij er een manifeste bloeding is. Dit geldt ook voor de PT/INR in het King's College model.

King's College criteria
Het tweede prognostische model (Tabel X) is ontwikkeld door de Liver Unit van het King's College Hospital in London Tabel X: King's College criteria voor levertransplantatie bij acuut leverfalen
Patiënten met acuut leverfalen worden op grond van deze twee prognostische modellen beoordeeld voor een levertransplantatie. De beoordeling vindt plaats bij opname en met tussenperioden van 12 uur totdat levensgevaar geweken is en op het moment dat een donorlever beschikbaar is. Indien de patiënt op basis van de criteria een levertransplantatie nodig heeft, wordt deze aangemeld bij Eurotransplant en met instemming van de verantwoordelijke arts van Eurotransplant in urgentiecategorie I op de wachtlijst geplaatst (zie Tabel V).
