Effects of carbohydrate supplementation on performance and carbohydrate oxidation after intensified cycling training Shona L. Halson,1,2 Graeme I. Jeukendrup1 To study the effects of carbohydrate (CHO) supplementation on performance changes and symptoms of overreaching, six male endurance cyclists completed 1 wk of normal (N), 8 days of intensified (ITP), and 2 wk of recovery training (R) on two occasions in a randomized crossover design. Subjects completed one trial with a 6% CHO solution provided before and during training and a 20% solution in the 1 h postexercise (H-CHO trial). On the other occasion, subjects consumed a 2% CHO solution at the same time points (L-CHO). A significant decline in time to fatigue at 63% maximal power output (H-CHO: 17 ± 3%; L-CHO: 26 ± 7%) and a significant increase in mood disturbance occurred in both trials after ITP. The decline in performance was significantly greater in the L-CHO trial. After ITP, a significant decrease in estimated muscle glycogen oxidation (H-CHO: N 49.3 ± 2.9 kcal/30 min, ITP 32.6 ± 3.4 kcal/30 min; L-CHO: N 49.1 ± 30 kcal/30 min, ITP 39.0 ± 5.6 kcal/30 min) and increase in fat oxidation (H-CHO: N 16.3 ± 2.4 kcal/30 min, ITP 27.8 ± 2.3 kcal/30 min; L-CHO: N 16.9 ± 2.6 kcal/30 min, ITP: 25.4 ± 3.5 kcal/30 min) occurred alongside significant increases in glycerol and free fatty acids and decreases in free triglycerides in both trials. An interaction effect was observed for submaximal plasma concentrations of cortisol and epinephrine, with significantly greater reductions in these stress hormones in L-CHO compared with H-CHO after ITP. These findings suggest that CHO supplementation can reduce the symptoms of overreaching but cannot prevent its development. Methods: We collected data in professional cyclists from the first TT of the 1998 Tour de France (TT1, 58 km distance; n = 6 cyclists) and the first (TT2, 56.5 km; n = 5) and second TT of the 1999 Tour de France (TT3, 57 km; n = 5). Results: A negative relationship was found between power output (W) at VT (VTWatt) and TT final time (s) in TT1 (r = –0.864; p = 0.026; standard error of estimate (SEE) of 73 s; and 95% confidence limits (95% CL) –0.98; –0.18), TT2 (r = –0.77; p = 0.27; SEE of 139 s; and 95% CL – 0.98; 0.35), and TT3 (r = –0.923; p = 0.025; SEE of 94 s; and 95% CL –1.00; –0.22). Conclusions: Actual performance in long TT during the Tour de France (>50 km distance, performed after at least 1–2 weeks of continuous competition), in which some cumulative fatigue inevitably occurs, is related, at least in part, to the power output that elicits the VT. No other routine physiological variable (for example, VO2max or Wpeak) is related to performance in this type of event.

Objectives: The respiratory compensation point (RCP) marks the onset of hyperventilation ("respiratory compensation") during incremental exercise. Its physiological meaning has not yet been definitely determined, but the most common explanation is a failure of the body’s buffering mechanisms which leads to metabolic (lactic) acidosis. It was intended to test this experimentally. Methods: During a first ramp-like exercise test on a cycle ergometer, RCP (range: 2.51–3.73 l*min–1 oxygen uptake) was determined from gas exchange measurements in five healthy subjects (age 26–42; body mass index (BMI) 20.7–23.9 kg*m–2; VO2peak 51.3–62.1 ml*min–1*kg–1). On the basis of simultaneous determinations of blood pH and base excess, the necessary amount of bicarbonate to completely buffer the metabolic acidosis was calculated. This quantity was administered intravenously in small doses during a second, otherwise identical, exercise test. Results: In each subject sufficient compensation for the acidosis, that is, a pH value constantly above 7.37, was attained during the second test. A delay but no disappearance of the hyperventilation was present in all participants when compared with the first test. RCP occurred on average at a significantly (p = 0.043) higher oxygen uptake (+0.15 l*min–1) compared with the first test. Conclusions: For the first time it was directly demonstrated that exercise induced lactic acidosis is causally involved in the hyperventilation which starts at RCP. However, it does not represent the only additional stimulus of ventilation during intense exercise. Muscle afferents and other sensory inputs from exercising muscles are alternative triggering mechanisms.

Renström, MD, PhD Background: The use of sports massage is very common in the athletic community. However, only a few studies have shown any therapeutic effect of massage. Hypothesis: Sports massage can improve the recovery after eccentric exercise. Study Design: Prospective randomized clinical trial. Methods: Sixteen subjects performed 300 maximal eccentric contractions of the quadriceps muscle bilaterally. Massage was given to 1 leg, whereas the other leg served as a control. Subjects were treated once daily for 3 days. Maximal strength was tested on a Kin-Com dynamometer, and functional tests were based on 1-leg long jumps. Pain was evaluated using a visual analog scale. Results: There was a marked loss of strength and function of the quadriceps directly after exercise and on the third day after exercise. The massage treatment did not affect the level or duration of pain or the loss of strength or function following exercise. Conclusion: Sports massage could not improve the recovery after eccentric exercise. The Effect of Glucose Infusion on Glucose Kinetics during a 1-h Time Trial. Purpose and Methods: To investigate the effect of glucose infusion on glucose kinetics and performance, six endurance cyclists ([latin capital V with dot above]O2max = 61.7 +/- 2.0 (mean +/- SE) mL[middle dot]kg-1[middle dot]min-1) completed two performance trials in which they had to accomplish a set amount of work as quickly as possible (991 +/- 41 kJ). Subjects were infused with either glucose (20% in saline; carbohydrate (CHO)) at a rate of 1 g[middle dot]min-1 or saline (0.9% saline; placebo (PLA)). It was hypothesized that time trial performance would be unaffected by the infusion of glucose, as endogenous stores of CHO would not be limiting in the PLA trial. Results: Plasma glucose concentration increased from 4.8 +/- 0.1 mmol[middle dot]L-1 to 5.9 +/0.3 mmol[middle dot]L-1 during the PLA trial and from 4.9 +/- 0.1 mmol[middle dot]L-1 at rest to 12.4 +/- 1.1 mmol[middle dot]L-1 during the CHO trial. These values were significantly higher at all time points during the CHO trial compared with PLA (P < 0.001). In the final stages of the time

trial, Rd in the PLA trial was 49 +/- 5 [mu]mol[middle dot]kg-1[middle dot]min-1 compared with 88 +/- 7 [mu]mol[middle dot]kg-1[middle dot]min-1 in the CHO trial (P < 0.05). Despite these differences, there was no difference in performance time between PLA and CHO (60.04 +/- 1.47 min, PLA, vs 59.90 +/- 1.49 min, CHO, respectively). Infused carbohydrate oxidation in the last 25% of the CHO trial was at least 675 +/- 120 [mu]mol[middle dot]kg-1 and contributed 17 +/4% to total carbohydrate oxidation. Conclusion: The results demonstrate that glucose infusion had no effect on 1-h cycle time-trial performance, despite an increased availability of plasma glucose for oxidation and evidence of increased glucose uptake into the tissues. Sports Exerc., Vol. 36, No. 7, pp. 1239-1243, 2004. Purpose: Ingestion of sodium bicarbonate is known to enhance sprint performance, probably via increased buffering of intracellular acidity. The goal was to compare the effect of ingestion of sodium bicarbonate with that of other potential buffering agents (sodium citrate and sodium lactate) and of a placebo (sodium chloride) on sprinting. Methods: In a double-blind randomized crossover trial, 15 competitive male endurance runners performed a run to exhaustion 90 min after ingestion of each of the agents in the same osmolar dose relative to body mass (3.6 mosmol[middle dot]kg-1) on separate days. The agents were packed in gelatin capsules and ingested with 750 mL of water over 90 min. During each treatment we assayed serial finger-prick blood samples for lactate and bicarbonate. A familiarization trial was used to set a treadmill speed for each runner's set of runs. We converted changes in run time between treatments into changes in a time trial of similar duration using the critical-power model, and we estimated likelihood of practical benefit using 0.5% as the smallest worthwhile change in time-trial performance. Results: The mean run times to exhaustion for each treatment were: bicarbonate 82.3 s, lactate 80.2 s, citrate 78.2 s, and chloride 77.4 s. Relative to bicarbonate, the effects on equivalent timetrial time were lactate 1.0%, citrate 2.2%, and chloride 2.7% (90% likely limits +/-2.1%). Ingested lactate and citrate both appeared to be converted to bicarbonate before the run. There were no substantial differences in gut discomfort between the buffer treatments. Conclusion: Bicarbonate is possibly more beneficial to sprint performance than lactate and probably more beneficial than citrate or chloride. We recommend ingestion of sodium bicarbonate to enhance sprint performance. Serum electrolyte concentrations and hydration status are not associated with exercise associated muscle cramping (EAMC) in distance runners M P Schwellnus1, J Nicol1, R Laubscher2 and T D Noakes3 Objectives: To determine whether acute exercise associated muscle cramping (EAMC) in distance runners is related to changes in serum electrolyte concentrations and hydration status. Methods: A cohort of 72 runners participating in an ultra-distance road race was followed up for the development of EAMC. All subjects were weighed before and immediately after the race. Blood samples were taken before the race, immediately after the race, and 60 minutes after the race. Blood samples were analysed for glucose, protein, sodium, potassium, calcium, and magnesium concentrations, as well as serum osmolality, haemoglobin, and packed cell volume. Runners who suffered from acute EAMC during the race formed the cramp group (cramp, n = 21), while runners with no history of EAMC during the race formed the control group (control, n = 22). Results: There were no significant differences between the two groups for pre-race or post-race body weight, per cent change in body weight, blood volume, plasma volume, or red cell volume. The immediate post-race serum sodium concentration was significantly lower (p = 0.004) in the cramp group (mean (SD), 139.8 (3.1) mmol/l) than in the control group (142.3 (2.1) mmol/l). The immediate post-race serum magnesium concentration was significantly higher (p = 0.03) in the cramp group (0.73 (0.06) mmol/l) than in the control group (0.67 (0.08) mmol/l).

Conclusions: There are no clinically significant alterations in serum electrolyte concentrations and there is no alteration in hydration status in runners with EAMC participating in an ultra-distance race. Purpose: Although manufacturers of bicycle power monitoring devices SRM and Power Tap (PT) claim accuracy to within 2.5%, there are limited scientific data available in support. The purpose of this investigation was to assess the accuracy of SRM and PT under different conditions. Methods: First, 19 SRM were calibrated, raced for 11 months, and retested using a dynamic CALRIG (50-1000 W at 100 rpm). Second, using the same procedure, five PT were repeat tested on alternate days. Third, the most accurate SRM and PT were tested for the influence of cadence (60, 80, 100, 120 rpm), temperature (8 and 21[degrees]C) and time (1 h at ~300 W) on accuracy. Finally, the same SRM and PT were downloaded and compared after random cadence and gear surges using the CALRIG and on a training ride. Results: The mean error scores for SRM and PT factory calibration over a range of 50-1000 W were 2.3 +/- 4.9% and -2.5 +/- 0.5%, respectively. A second set of trials provided stable results for 15 calibrated SRM after 11 months (-0.8 +/- 1.7%), and follow-up testing of all PT units confirmed these findings (-2.7 +/- 0.1%). Accuracy for SRM and PT was not largely influenced by time and cadence; however, power output readings were noticeably influenced by temperature (5.2% for SRM and 8.4% for PT). During field trials, SRM average and max power were 4.8% and 7.3% lower, respectively, compared with PT. Conclusions: When operated according to manufacturers instructions, both SRM and PT offer the coach, athlete, and sport scientist the ability to accurately monitor power output in the lab and the field. Calibration procedures matching performance tests (duration, power, cadence, and temperature) are, however, advised as the error associated with each unit may vary. Introduction: The purpose of this study was to determine whether endurance cycling performance and postexercise muscle damage were altered when consuming a carbohydrate and protein beverage (CHO+P; 7.3% and 1.8% concentrations) versus a carbohydrate-only (CHO; 7.3%) beverage. Methods: Fifteen male cyclists (mean [latin capital V with dot above]O2peak = 52.6 +/- 10.3 mL[middle dot]kg-1[middle dot]min-1) rode a cycle ergometer at 75% [latin capital V with dot above]O2peak to volitional exhaustion, followed 12-15 h later by a second ride to exhaustion at 85% [latin capital V with dot above]O2peak. Subjects consumed 1.8 mL[middle dot]kg-1 BW of randomly assigned CHO or CHO+P beverage every 15 min of exercise, and 10 mL[middle dot]kg-1 BW immediately after exercise. Beverages were matched for carbohydrate content, resulting in 20% lower total caloric content per administration of CHO beverage. Subjects were blinded to treatment beverage and repeated the same protocol seven to 14 d later with the other beverage. Results: In the first ride (75% [latin capital V with dot above]O2peak), subjects rode 29% longer (P < 0.05) when consuming the CHO+P beverage (106.3 +/- 45.2 min) than the CHO beverage (82.3 +/- 32.6 min). In the second ride (85% [latin capital V with dot above]O2peak), subjects performed 40% longer when consuming the CHO+P beverage (43.6 +/- 12.5 min) than when consuming the CHO beverage (31.2 +/- 8.7 min). Peak postexercise plasma CPK levels, indicative

of muscle damage, were 83% lower after the CHO+P trial (216.3 +/- 122.0 U[middle dot]L-1) than the CHO trial (1318.1 +/- 1935.6 U[middle dot]L-1). There were no significant differences in exercising levels of [latin capital V with dot above]O2, ventilation, heart rate, RPE, blood glucose, or blood lactate between treatments in either trial. Conclusion: A carbohydrate beverage with additional protein calories produced significant improvements in time to fatigue and reductions in muscle damage in endurance athletes. Further research is necessary to determine whether these effects were the result of higher total caloric content of the CHO+P beverage or due to specific protein-mediated mechanisms. Purpose: The purpose of the present study was to examine markers of cardiac function and cardiac damage during a simulated half-ironman triathlon in highly trained athletes. Methods: Nine highly trained male triathletes volunteered for the study (mean +/- SD; age: 33 +/3 yr; height: 1.8 +/- 0.1 m; body mass: 77.7 +/- 3.2 kg). The subjects completed a half-ironman triathlon; 1.9-km swim in an indoor 20-m pool, followed by a laboratory-based 90-km cycle and 21.1-km run. Venous blood samples were drawn and echocardiographic assessment completed before the start of exercise, immediately after each stage, and 24 h postexercise. Serum was analyzed for total creatine kinase activity (CK), creatine kinase isoenzyme MBmass (CK-MBmass), and cardiac troponin T (cTnT). Left ventricular systolic (stroke volume, and systolic blood pressure/end systolic volume ratio (SBP/ESV)) and diastolic (ratio of early [E] to late [A], ventricular filling E:A) measurements were derived from echocardiographic assessment. Results: The mean completion time of the half-ironman triathlon was 301 +/- 28 min. Left ventricular contractility (SBP/ESV) was significantly reduced after the half-ironman triathlon (P < 0.05). A significant reduction in E:A was observed after the run phase of the half-ironman triathlon (P < 0.05). Significant increases in CK and CK-MBmass were observed during and after the halfironman triathlon (P < 0.05), and cTnT was elevated in four subjects over the course of the halfironman triathlon. Conclusions: The physiologic stress imposed by the half-ironman triathlon resulted in a reduced left ventricular contractility and altered diastolic filling, coupled with minimal cardiac damage in a number of highly trained male triathletes. The mechanisms behind such altered cardiac function and cardiac damage after prolonged exercise are yet to be elucidated. Context Healthy lifestyle factors are associated with maintenance of erectile function in men. Objective To determine the effect of weight loss and increased physical activity on erectile and endothelial functions in obese men. Design, Setting, and Patients Randomized, single-blind trial of 110 obese men (body mass index 30) aged 35 to 55 years, without diabetes, hypertension, or hyperlipidemia, who had erectile dysfunction that was determined by having a score of 21 or less on the International Index of

Erectile Function (IIEF). The study was conducted from October 2000 to October 2003 at a university hospital in Italy. Interventions The 55 men randomly assigned to the intervention group received detailed advice about how to achieve a loss of 10% or more in their total body weight by reducing caloric intake and increasing their level of physical activity. Men in the control group (n = 55) were given general information about healthy food choices and exercise. Main Outcomes Measures Erectile function score, levels of cholesterol and tryglycerides, circulating levels of interleukin 6, interleukin 8, and C-reactive protein, and endothelial function as assessed by vascular responses to L-arginine. Results After 2 years, body mass index decreased more in the intervention group (from a mean [SD] of 36.9 [2.5] to 31.2 [2.1]) than in the control group (from 36.4 [2.3] to 35.7 [2.5]) (P<.001), as did serum concentrations of interleukin 6 (P = .03), and C-reactive protein (P = .02). The mean (SD) level of physical activity increased more in the intervention group (from 48 [10] to 195 [36] min/wk; P<.001) than in the control group (from 51 [9] to 84 [28] min/wk; P<.001). The mean (SD) IIEF score improved in the intervention group (from 13.9 [4.0] to 17 [5]; P<.001), but remained stable in the control group (from 13.5 [4.0] to 13.6 [4.1]; P = .89). Seventeen men in the intervention group and 3 in the control group (P = .001) reported an IIEF score of 22 or higher. In multivariate analyses, changes in body mass index (P = .02), physical activity (P = .02), and Creactive protein (P = .03) were independently associated with changes in IIEF score. Conclusion Lifestyle changes are associated with improvement in sexual function in about one third of obese men with erectile dysfunction at baseline. MALLIAROPOULOS, N., S. PAPALEXANDRIS, A. For years, stretching has been an integral part of fitness, practice, and rehabilitation programs to decrease muscle stiffness and relieve pain associated with it. The increased joint range of motion (ROM), indicating the degree of muscle flexibility, has a benefit of stretching proven for healthy tissues. Purpose: The objective of our study was the assessment of the effects of stretching in the rehabilitation of hamstring injuries. Methods: We followed up 80 Greek athletes, of average age 20.5 yr, with "second-degree" strain of the hamstring muscles. The athletes were divided into two groups, A and B. For both groups, we estimated the time required for the rehabilitation of the decreased knee ROM and the total time before the athletes could return to a full training program. Results: Group B, which carried out a more intensive stretching program, was found to have a statistically significant shorter time of regaining normal ROM (5.57 +/- 0.71 d) and rehabilitation period (13.27 +/- 0.71 d) in comparison with group A (7.32 +/- 0.525 d and 15.05 +/- 0.81 d, respectively). Conclusion: Our results suggest that stretching is of great importance in treating muscle strain injuries in that it improves the effectiveness of the rest rehabilitation program. caused by facial cooling. The purpose of this study was to compare the effects on BR of cold air inhalation and of facial exposure to cold air, as well as the combined effect of both. Methods: Fourteen children with asthma (eight girls) performed four exercise challenge tests in a climatic chamber, under one of the following conditions: 1) inhaling warm air while the face was exposed to warm air (WW, 21[degrees]C, 25% relative humidity (RH)); 2) inhaling warm air while the face was exposed to cold air (WC, 0[degrees]C, 80% RH); 3) inhaling cold air while the face was exposed to cold air (CC); and 4) inhaling cold air while the face was exposed to warm air (CW). The study was analyzed, using a one- and two-way ANOVA. Results: Postexercise forced expiratory volume in the first second (FEV1) and maximal midexpiratory flow (MMEF) values as percent predicted (% pred) showed significant reductions over time (P < 0.001), significant differences among the four experimental conditions (P < 0.001) and a significant condition x time interaction (FEV1:P < 0.001, MMEF:P < 0.01). FEV1 was significantly lower for CC and WC, as compared with WW and CW at 5 and 10 min postexercise. The lowest postexercise values for FEV1 occurred in the CC and WC sessions (76% predicted in both). A similar pattern was obtained for MMEF. Conclusion: Facial cooling combined with either cold or warm air inhalation causes the greatest EIB, as compared with the isolated challenge with cold air inhalation. We suggest that vagal mechanisms play a major role in exercise and cold-induced bronchoconstriction. BISHOP, D., J. EDGE, C. DAVIS, and C. GOODMAN Induced Metabolic Alkalosis Affects Muscle Metabolism and Repeated-Sprint Ability. Medicine & Science in Sports & Exercise. 36(5):807-813, May 2004. Purpose: The purpose of this study was to assess the effects of induced metabolic alkalosis, via sodium bicarbonate (NaHCO3) ingestion, on muscle metabolism and power output during repeated short-duration cycle sprints. Methods: Ten active females (mean +/- SD: age = 19 +/- 2 yr, [latin capital V with dot above]O2max = 41.0 +/- 8.8 mL[middle dot]kg -1[middle dot]min -1) ingested either 0.3 g[middle dot]kg -1 NaHCO3 or 0.207 g[middle dot]kg -1 of NaCl (CON), in a double-blind, random, counterbalanced order, 90 min before performing a repeated-sprint ability (RSA) test (5 x 6-s allout cycle sprints every 30 s). Results: Compared with CON, there was a significant increase in resting blood bicarbonate concentration [HCO3-] (23.6 +/- 1.1 vs 30.0 +/- 3.0 mmol[middle dot]L -1) and pH (7.42 +/- 0.02 vs 7.50 +/- 0.04), but no significant difference in resting lactate concentration [La-] (0.8 +/- 0.2 vs 0.8 +/- 0.3 mmol[middle dot]L -1) during the NaHCO3 trial. Muscle biopsies revealed no significant difference in resting muscle [La-], pH, or buffer capacity ([beta]in vitro) between trials (P > 0.05). Compared with CON, the NaHCO3 trial resulted in a significant increase in total work (15.7 +/- 3.0 vs 16.5 +/- 3.1 kJ) and a significant improvement in work and power output in sprints 3, 4, and 5. Despite no significant difference in posttest muscle pH between conditions, the NaHCO3 trial resulted in significantly greater posttest muscle [La-]. Conclusions: As NaHCO3 ingestion does not increase resting muscle pH or [beta]in vitro, it is likely that the improved performance is a result of the greater extracellular bufferconcentration increasing H+ efflux from the muscles into the blood. The significant increase in posttest muscle [La-] in NaHCO3 suggests that an increased anaerobic energy contribution is one mechanism by which NaHCO3 ingestion improved RSA. BUFORD, B. N., and A. J. KOCH. Glycine-Arginine-α-Ketoisocaproic Acid Improves Performance of Repeated Cycling Sprints. Med. Sci. Sports Exerc., Vol. 36, No. 4, pp. 583-587, 2004. Purpose: The purpose of this study was to determine the effect of glycine-arginine-α-ketoisocaproic acid (GAKIC) supplementation on repeated bouts of anaerobic cycling performance. Methods: Ten men completed a randomized, double-blinded, placebo-controlled exercise protocol of two sessions separated by 7 d. Plasma lactate was analyzed in blood collected 45 min before exercise (REST) and 5 min postexercise (POST). Subjects consumed either 11.2-g GAKIC or placebo (PLC) during a 45-min period between the REST and exercise. Mean power, peak power, and fatigue values were assessed from five supramaximal, 10-s cycle ergometer sprints, separated by 1-min rest intervals. Data were analyzed using repeated measures ANCOVA. Results: A significant treatment × time interaction (P = 0.039) was observed for the change in mean power output over the five sprints between the GAKIC and PLC treatments. Post hoc

analyses revealed a greater retention of mean power (P = 0.038) between sprints 1 and 2 after GAKIC (-1 ± 9 W) versus PLC treatment (-47 ± 18 W). No other performance variables differed between PLC and GAKIC. POST lactate was increased (P < 0.001) above REST, but there was no difference between treatments (P = 0.936). Conclusion: These data support an ergogenic effect of GAKIC for attenuating the decline in mean power during repeated bouts of supramaximal exercise. According to surveys and media reports, the legal and illegal use of these drugs is gaining popularity. Testosterone restores sex drive and boosts muscle mass, making it central to 2 of society’s rising preoccupations: perfecting the male body and sustaining the male libido. The anabolic effects of AAS have been questioned for decades, but recent scientific investigation of supraphysiologic doses supports the efficacy of these regimens. Testosterone has potent anabolic effects on the musculoskeletal system, including an increase in lean body mass, a dose-related hypertrophy of muscle fibers, and an increase in muscle strength. For athletes requiring speed and strength and men desiring a cosmetic muscle makeover, illegal steroids are a powerful lure, despite the risk of subjective side effects. Recent clinical studies have discovered novel therapeutic uses for physiologic doses of AAS, without any significant adverse effects in the short term. In the wake of important scientific advances during the past decade, the positive and negative effects of AAS warrant reevaluation. Guidelines for the clinical evaluation of AAS users will be presented for sports medicine practitioners.

Effects of leg massage on recovery from high intensity cycling exercise A Robertson1, J M Watt2 and S D R Galloway1 Background: The effect of massage on recovery from high intensity exercise is debatable. Many studies on massage suffer from methodological flaws such as poor standardisation of previous exercise, lack of dietary control, and inappropriate massage duration. Objective: To examine the effects of leg massage compared with passive recovery on lactate clearance, muscular power output, and fatigue characteristics after repeated high intensity cycling exercise, with the conditions before the intervention controlled and standardised. Methods: Nine male games players participated. They attended the laboratory on two occasions one week apart and at the same time of day. Dietary intake and activity were replicated for the two preceding days on each occasion. After baseline measurement of heart rate and blood lactate concentration, subjects performed a standardised warm up on the cycle ergometer. This was followed by six standardised 30 second high intensity exercise bouts, interspersed with 30 seconds of active recovery. After five minutes of active recovery and either 20 minutes of leg massage or supine passive rest, subjects performed a second standardised warm up and a 30 second Wingate test. Capillary blood samples were drawn at intervals, and heart rate, peak power, mean power, and fatigue index were recorded. Results: There were no significant differences in mean power during the initial high intensity exercise bouts (p = 0.92). No main effect of massage was observed on blood lactate concentration between trials (p = 0.82) or heart rate (p = 0.81). There was no difference in the maximum power (p = 0.75) or mean power (p = 0.66) in the subsequent Wingate test, but a significantly lower fatigue index was observed in the massage trial (p = 0.04; mean (SD) fatigue index 30.2 (4.1)% v 34.2 (3.3)%).

Conclusions: No measurable physiological effects of leg massage compared with passive recovery were observed on recovery from high intensity exercise, but the subsequent effect on fatigue index warrants further investigation. 1. Abstract Plantar pressure data were recorded in two different shoe types to determine the effect of cycling shoe stiffness on peak plantar forefoot pressure in cyclists. Two pairs of shoes of the same size and manufacturer, identical except for outsole material and stiffness, were tested. Shoe stiffness measurements were collected under controlled conditions and in two different configurations using a dynamic hydraulic tensile testing machine. Measurements of plantar pressure were done using Pedar capacitive-based sensor insoles while subjects pedaled in a seated position at a controlled power output. Power output was set at a constant value of 400 W across all subjects by a magnetic resistance trainer unit. The pressure distribution in carbon-fiber-composite shoes during cycling was compared to cycling shoes made with plastic soles. Carbon fiber shoes presented stiffness values 42% and 550% higher than plastic shoes in longitudinal bending and three-point bending, respectively. The shoes made with carbon fiber produced peak plantar pressures 18% higher than those of plastic design (121 kPa vs. 103 kPa, p = .005). Competitive or professional cyclists suffering from metatarsalgia or ischemia should be especially careful when using carbon fiber cycling shoes because the shoes increase peak plantar pressure, which may aggravate these foot conditions. Abstract The aim of this study was to determine the response of cyclists to manipulations of cadence and power output in terms of force application and plantar pressure distribution. Two groups of cyclists, 17 recreational and 12 competitive, rode at three nominal cadences (60, 80, 100 rev x min(-1)) and four power outputs (100, 200, 300, 400 W) while simultaneous force and in-shoe pressure data were collected. Two piezoelectric triaxial force transducers mounted in the right pedal measured components of the pedal force and orientation, and a discrete transducer system with 12 transducers recorded the in-shoe pressures. Force application was characterized by calculating peak resultant and peak effective pedal forces and positive and negative impulses. In-shoe pressures were analysed as peak pressures and as the percent relative load. The force data showed no significant group effect but there was a cadence and power main effect. The impulse data showed a significant three-way interaction. Increased cadence resulted in a decreased positive impulse, while increased power output resulted in an increased impulse. The competitive group produced less positive impulse but the difference became less at higher cadences. Few betweengroup differences were found in pressure, notable only in the pressure under the first metatarsal region. This showed a consistent pattern of in-shoe pressure distribution, where the primary loading structures were the first metatarsal and hallux. There was no indication that pressure at specific sites influenced the pedal force application. The absence of group differences indicated that pressure distribution was not the result of training, but reflected the intrinsic relationship between the foot, the shoe and the pedal.

Purpose: It was hypothesized that metabolic adaptations in single muscle cells after a taper period are fiber type (I and II) specific and protocol regimen dependent. Methods: After 7-wk intensive endurance training, 22 male cyclists (O2max = 4.42 ± 0.40 L·min-1) were randomly assigned to one of three 7-d taper groups: the control group (CON, N = 7) continued weekly training, the first experimental group (INT) maintained training intensity but reduced duration (N = 7), and the second experimental group (DUR) maintained training duration but reduced exercise intensity (N = 8). Each cyclist completed a simulated 40-km time trial (40TT) before and after tapering on a set of wind-loaded rollers using their own bicycle. Muscle biopsies were taken immediately before the 40TT both before and after tapering, and analyzed for mATPase, succinate dehydrogenase (SDH), cyctochrome oxidase (CYTOX), alphaglycerolphosphate dehydrogenase (α-GPD), and beta-hydroxyacyl CoA dehydrogenase (β-HOAD) in Type I and II fibers, separately, using quantitative histochemistry. Results: The results showed significant (P ≤ 0.05) increases in SDH (Type I) and mATPase, CYTOX, βHOAD, and SDH (Type II fibers) in the INT group, and significant increases in CYTOX (Type I) and β-HOAD (Type I and II fibers) in the DUR group. Regression analysis showed that the change (posttaper minus pretaper) in simulated 40-km endurance time was correlated with the change in CYTOX and SDH activity for all groups combined (r2 = 0.62-0.72). Conclusion: These results demonstrated that the metabolic properties of different fiber types are altered with tapering, that the type of taper protocol used influences their physiological adaptation, and that improvements in simulated 40-km endurance time were related to changes in metabolic properties of the muscle at the single fiber level. Purpose: To examine the effect of prolonged cycling on ratings of perceived exertion (RPE) in boys and men and whether carbohydrate (CHO) ingestion would lower RPE during exercise. Methods: Ten boys (9-10 yr) and 10 men (20-25 yr) cycled for 60 min at ~70% O2peak on two occasions. In a double-blind, counterbalanced design, a total volume of 24 mL·kg-1 body mass of either a 6% CHO-electrolyte (CT) or flavored water (WT) beverage was consumed intermittently before and during exercise in each trial. Oxygen consumption (O2), ventilation (E), respiratory rate (RR), RPE (Borg's 6-20 scale), and heart rate (HR) were recorded periodically throughout exercise. Plasma glucose (GLU) was determined before and after exercise. Results: Postexercise GLU was not different between age groups but higher (P < 0.001) during CT (5.6 ± 0.2 mmol·L-1) compared with WT (4.7 ± 0.1 mmol·L-1). CHO ingestion had no effect (P > 0.05) on O2, E, RR, or RPE in either group. RR during exercise was higher (P < 0.01) in boys (39.0 ± 2.2 breaths·min-1) than in men (30.9 ± 1.3 breaths·min-1). HR was slightly higher (P = 0.047) during CT (160 ± 3 beats·min-1) compared with WT (156 ± 4 beats·min-1) and increased less over time (P < 0.01) in boys compared with men. RPE at 5 min of exercise was similar (P > 0.05) between boys (11.8 ± 0.7) and men (12.0 ± 0.7) but increased faster (P < 0.01) over time in boys. The average exercise RPE was higher (P < 0.01) in boys (15.8 ± 0.5) than in men (14.0 ± 0.4). Conclusions: The higher and faster increase in RPE during exercise in boys, compared with men, may reflect a sensitivity to RR that outweighed any effect of CHO ingestion on RPE. Purpose: In the HERITAGE Family Study, heart rate (HR) associated with various percentages of maximal oxygen intake (O2max) was used to prescribe exercise intensity. When fitness improved, HR at the same power output (PO) decreased, and PO was increased to produce the prescribed HR. Although we assumed that subjects were again working at the same %O2max, there were no studies with a large heterogeneous population to determine whether this was correct. Methods: Therefore, 653 subjects with complete data were classified by age, sex, race, initial O2max, and O2max response after 20 wk of training. Results: All groups had a significant increase in O2max and a significant decrease in HR at the same absolute PO after training but no difference in HR at the same relative intensity. Conclusions: Training does not affect HR at a given %O2max in a heterogeneous population of men and women, blacks and whites aged 17-65 yr with different initial O2max values and different responses to training. Es konnte damit erstmals unmittelbar experimentell demonstriert werden, dass die belastungsinduzierte Laktatazidose zur am RCP einsetzenden Hyperventilation ursächlich beiträgt, jedoch wahrscheinlich nicht den einzigen zusätzlichen Ventilationsstimulus darstellt. Based on the determination of cardiac troponin (cTnT), brain natriuretic peptide (BNP), and echocardiographic measurements, recent investigations have reported myocardial damage and reversible cardiac dysfunction after prolonged endurance exercise in apparently healthy subjects. In the present study, we investigated the myocardial stress reaction in professional endurance athletes after strenuous competitive physical exercise. Methods: Eleven highly trained male professional road cyclists (age 27 ± 4 yr; O2peak 67 ± 5 mL·kg-1·min1; training workload 34,000 ± 2,500 km·yr-1) were examined. The following parameters were determined before and after one stage of a 5-d professional cycling race: BNP, cTnT (thirdgeneration assay that shows no cross reactivity with skeletal TnT), creatine kinase (CK), creatine kinase MB (CKMB), myoglobin (Myo), and urea. All participants were submitted to a careful cardiac examination including echocardiography and stress ECG. Results: None of the athletes showed pathological findings in the cardiac examination. CK (P < 0.01), CKMB (P < 0.05), and Myo (P < 0.01) were increased after the race. Normal postexercise cTnT levels indicate that the increase in CK, CKMB, and Myo was of noncardiac origin. In contrast, BNP rose significantly from 47.5 ± 37.5 to 75.3 ± 55.3 pg·mL-1 (P < 0.01). Pre- and postexercise values of BNP as well as the individual exercise-induced increase in BNP were significantly correlated with age (R2 = 0.68, R2 = 0.66, and R2 = 0.58, respectively; P < 0.05). Conclusion: Strenuous endurance exercise in professional road cyclists does not result in structural myocardial damage. The rise in BNP in older athletes may reflect a reversible, mainly diastolic left ventricular dysfunction. In this presentation, three dietary supplements will be evaluated: 1) an energy bar containing conjugated linoleic acid (CLA), 2) an agent purported to block the Myostatin protein, and 3) a phytochemical version of human growth hormone. Ads for all three supplements claim to induce weight and fat loss, increase lean mass, and enhance metabolic rate. Conjugated Linoleic Acid. Energy bars have long been a staple of nutritional foods for athletes, but recently manufacturers have added ingredients other than macro or micronutrients. The energy bar reviewed in this presentation contains conjugated linoleic acid (CLA). With 220 calories and 1.5 grams of CLA per bar, the manufacturer recommends one bar daily as a meal replacement. CLA was discovered in 1978 when researchers found a substance in fried ground beef that appeared to inhibit mutagenesis in laboratory animals. CLA is a naturally occurring fatty acid found in the fat of ruminant animals and dairy products and is an isomer of linoleic acid. CLA has been shown to lower adiposity in animals and sometimes in humans, but the human studies did not have rigorous controls for diet and exercise. The mechanism of action is unknown, but CLA is theorized to reduce lipoprotein lipase activity, reduce intracellular triglycerides, and enhance glycerol release from adipose tissue. The benefits of CLA on body composition are documented in animals, but scant support for such effects exists for humans. More promising is the research on CLA and cancer prevention. Myostatin Inhibitor. "Double-muscled" animals have been of great interest to the livestock industry for hundreds of years. The "Belgian Blue" and "Piedmontese" cattle exhibit muscular hypertrophy producing greater meat yields with lower fat. In 1997, researchers at Johns Hopkins discovered and named a protein, Myostatin, as a specific inhibitor of muscle cell growth. Moreover, they determined that a genetic mutation resulting in the absence of Myostatin was responsible for "double-muscled" cattle. The supplement industry was quick to develop products that purportedly neutralized Myostatin, allowing supplement users to "grow new muscles." Preliminary human studies suggest that Myostatin may play a role in AIDS-associated muscle wasting and in agerelated sarcopenia, but no dietary supplement can alter gene expression.

Phytochemical HGH. Ever since 1990, when a study revealed that older men who were given weekly injections of growth hormone for 6 months increased lean body mass and decreased fat mass, growth hormone has been viewed as the anti-aging drug. Subsequent studies have shown that growth hormone can alter body composition, but not improve muscle strength. Growth hormone has some negative side effects, including increases in blood pressure and blood glucose levels. Other potential problems include faster growth of cancers. Recently, supplement manufacturers have been touting a phytochemical or nutraceutical version of growth hormone, called Symbiotropin. The claims for the "all natural" supplement say it has all of the benefit of human growth hormone with none of the side effects. It claims to be the answer to "andropause." No published research exists about the effects of the dietary supplements claiming to be substitutes for human growth hormone, although websites selling the product provide papers that were presented at "scientific" meetings. SPEARS, I. R., N. K. CUMMINS, Z. BRENCHLEY, C. DONOHUE, C. TURNBULL, S. BURTON, and G. A. MACHO. The Effect of Saddle Design on Stresses in the Perineum during Cycling. Medicine & Science in Sports & Exercise 2003; 35(9):1620-1625 Purpose: Repetitive internal stress in the perineum has been associated with soft-tissue trauma in bicyclists. Using an engineering approach, the purpose of this study was to quantify the amount of compression exerted in the perineum for a range of saddle widths and orientations. Methods: Computer tomography was used to create a three-dimensional voxel-based finite element model of the right side of the male perineum-pelvis. For the creation of the saddle model, a commercially available saddle was digitized and the surface manipulated to represent a variety of saddle widths and orientations. The two models were merged, and a static downward load of 189 N was applied to the model at the region representing the sacroiliac joint. For validation purposes, external stresses along the perineum-saddle interface were compared with the results of pressure sensitive film. Good agreement was found for these external stresses. The saddles were then stretched and rotated, and the magnitude and location of maximum stresses within the perineum were both recorded. In all cases, the model of the pelvis-perineum was held in an upright position. Results: Stresses within the perineum were reduced when the saddle was sufficiently wide to support both ischial tuberosities. This supporting mechanism was best achieved when the saddle was at least two times wider than the bi-ischial width of the cyclist. Stresses in the anterior of the perineum were reduced when the saddle was tilted downward, whereas stresses in the posterior were reduced when the saddle was tilted upward. Conclusions: Recommendations that saddles should be sufficiently wide to support the ischial tuberosities appear to be well founded. Recommendations that saddles be tilted downward (i.e., nose down) are supported by the model, but with caution, given the limitations of the model. Methods: Subjects were sedentary volunteers (two or fewer days a week of exercise for less than 30 minutes a day for the previous three months), 18–29 years of age, with a naturally acquired URTI (three to four days of onset). All subjects were screened—for example, asthma, hay fever—by a doctor and were afebrile. Volunteers were alternately assigned to an exercise (EX) group (four men, seven women) or a non-exercise (NEX) group (three men, eight women). Subjects in the EX group completed 30 minutes of supervised exercise at 70% of target heart rate range for five days of a seven day period. For the initial screening, and every 12 hours, all subjects completed a 13 item symptom severity checklist and a physical activity log. Cold symptom scores were obtained until the subjects were asymptomatic. Significance was set at p<=" BORDER="0">0.05.

Results: There were no significant differences between EX and NEX group mean symptom scores for the morning and evening reporting periods. There were also no differences between the groups for the mean number of days from the baseline symptom score to when the subjects were asymptomatic. There were no differences between physical activity levels, other than what was assigned in the EX group. Conclusion: Moderate exercise in sedentary subjects with naturally acquired URTI probably does not alter the overall severity and duration of the illness. Previously sedentary people who have acquired a URTI and have just initiated an exercise programme may continue to exercise. Methods: Thirteen female endurance athletes completed three four hour runs on a 400 m track. Environmental conditions differed between the three trials: 5.3°C and snow (trial 1), 19.0°C and sunny weather (trial 2), 13.9°C and precipitation (trial 3). They consumed 1 litre of fluid an hour during the trials with randomised intake of fluids: one trial (H) with high sodium concentration (680 mg/l), one trial (L) with low sodium concentration (410 mg/l), and one trial with only water (W). Before and after the trials, subjects were weighed and blood samples were taken for analysis of [Na+]plasma, packed cell volume, and mean corpuscular volume. Results: The mean (SD) decrease in [Na+]plasma over the whole trial was significantly (p<0.001) less in trial H (2.5 (2.5) mmol/l) than in trial W (6.2 (2.1) mmol/l). Mild hyponatraemia ([Na+]plasma = 130–135 mmol/l) was observed in only six women (46%) in trial H compared with nine (69%) in trial L, and 12 (92%) in trial W. Two subjects (17%) in trial W developed severe hyponatraemia ([Na+]plasma<130 mmol/l). No significant differences were found in performance or haematological variables with the three different fluids. There was no significant correlation between[Na+]plasma after the run and performance. There was a significant correlation between changes in [Na+]plasma and changes in body weight. Conclusions: Exercise induced hyponatraemia in women is likely to develop from fluid overload during prolonged exercise. This can be minimised by the use of replacement fluids of high sodium concentration. Sodium replacement of at least 680 mg/h is recommended for women in a state of fluid overload during endurance exercise of four hours. However, higher [Na+]plasma after the run and smaller decreases in [Na+]plasma during the trials were no indication of better performance over four hours. Bij fietsen over lange afstanden komt “de fietshand”, gevoelsstoornis en/of krachtsverlies aan de handen, zeer vaak voor. Bij 25 mountainbikers, kwam dit bij 23 onder hen voor, ongeacht de getraindheid. De gevoelsstoornis komt voornamelijk aan de ulnaire (pink) zijde voor. Bij MTB’ers komt de fietshand vaker dan bij wegfietsers. Bij 14 vrouwen werd onderzoek gedaan naar het effect van de pil op prestaties. Naast een toename in het vet% door de pil, was er een afname in de VO2max van bijna 5% bij de pilgebruiksters. Asker Jeukendrup (werkzaam aan de Universiteit van Birmingham) stuurde mij enkele zeer lezenswaardige artikelen.

Daar putte ik de volgende praktische conclusies uit: Bij fysical performance, prestatie vermogen, is ervaring en kennis een factor die meespeelt. Bij een maximaal test, bv. tijdens een test bij mij op de fiets, is er ook een duidelijk leereffect, waardoor de resultaten “verbeteren”, zonder dat er sprake is van daadwerkelijk vooruitgang. Ger Bongers heeft mij daar reeds meerdere malen op gewezen! Overtraining: Tijdens overtraining zijn er afwijkingen in het bloed te vinden: Stijging CPK, stijging Glutamaat; daarnaast daling Haematocriet, Hemoglobine en de Glutamine/Glutamaat ratio. Tevoren drinken van Koolhydraatrijke Sportdrank: 45 minuten voor een tijdrit werden 3 verschillende drankjes gedronken: met glucose, met galactose of met trehalose; er waren duidelijke verschillen in glucose en insuline gehalte in het bloed, maar er was geen enkel verschil in resultaat (tijd in de tijdrit). In het verleden was al eens aangetoond dat er ook geen verschil is tussen glucose of fructose. Kortom……… tevoren Roosvicee is een goedkoop alternatief en de claims van de verschillende Merken is niet waar te maken….. Maar het kan nog saillanter: In een ander onderzoek werd vergeleken: water, een drank met kleine hoeveelheid koolhydraten en een drank met gemiddelde en een drank met grotere hoeveelheid koolhydraten. Natuurlijk waren er weer verschillen in glucose en insuline gehalte, maar er was geen verschil in prestatie! Het nut van drinken van koolhydraatrijke sportdrank TIJDENS een wedstrijd, is uiterst zinvol (mits langer dan 45 minuten), het drinken van vocht voor een wedstrijd/training ook; maar die BEHOEFT GEEN koolhydraten te bevatten! In een eerdere mailing meldde ik jullie: Hitte? Minder drinken en meer zout! Daar zou ik aan toe willen voegen: en koolhydraten in de bidon…. In een onderzoek kom men namelijk aantonen dat bij warme omstandigheden ( 35 graden) met zo’n drankje (met maltodextrines) de tijd tot uitputting (testen op 60 en 73% van de VO2max) met 1.5% en 13.5% toenam. De verklaring ligt niet op metabool, maar op centraal vlak; mogelijk dus een direct effect in de hersenen/hypofyse. Maltodextrines? Ook die zitten in Roosvicee. Maar mijn ervaring met snoeiheet weer, is dat dan een commerciëel sportdrankje onontbeerlijk is voor de smaak. gebruikten de baanrenners meer anaërobe energie dan de wegrenners. Dat ging echter met een groter vermoeidheidsgevoel bij de baanrenners gepaard. In een zuurstofarme omgeving getest, daalde de zuurstofopname met 16% ibij weg- en baanrenners. Toch was er geen verandering in maximale “power output”, moeheidsgevoel of laktaat gehalte, t.o.v. het testen onder normale zuurstof-omstandigheden. Onder dit soort omstanddigheden compenseren wegrenners een maximale inspanning (sprint), door meer gebruik te maken van het anäerobe energie-systeem, waardoor net zoals bij de baanrenners het maximale kortdurende vermogen grotendeel gehandhaafd blijft. Wat is het effect van bepaalde interval-trainingen bij (al) goed getrainde wegrenners? Voor-, na 2 en na 4 weken training, werden 38 renners ( gemiddeld 25 jaar oud, 75 kg wegend; VO2 max = 64.5 ml/kg) getest. Dat betrof een maximaal test (PPO), een volhoudtest Tmax (belasting conform maximale VO2max) en een tijdrit (TT40). Stel iemand haalt 400 Watt; de Tmax is dan de tijd die hij dit, na inrijden, maximaal kan volhouden; bv. 5 minuten; 60% Tamx betekent dan 60% van die 5 minuten op 400 Watt intervallen. Er werden 4 verschillende trainingsgroepen gevormd, 3 van de vier deden naast hun duurtrainingen ook specifieke interval-trainingen ( G1: N = 8; 8 × 60% van Tmax; G2: N = 9; 8 × 60% van Tmax met herstel op 65% van HRmax; G3: N = 10, 12 × 30 s op 175% van PPO, met 4 tot 5 minl; GCON, N = 11). Alle groepen verbeterden TT40 tijd (4.4 tot 5.8%) en hun Wattage ( PPO, +3.0 tot +6.2%) significant meer bij de intervalgroepen dan de controle (GCON: 0.9 en +1.1%); Bovendien namen in G1 (+5.4%) en in G2(+8.1%) de maximale zuurstofopname meer toe dan bij de controle (GCON: +1.0%). Purpose: This study was designed to quantify and describe the intensity profile of cross-country mountainbiking races using heart rate (HR) recorded during competitions. Methods: Nine mountain bikers participated in four cross-country circuit races of international and national levels. Each cyclist was tested before the competitions to determine lactate threshold (LT), the onset of blood lactate accumulation (OBLA4), and the relationship between percentage of maximum HR and percentage of O2max. Results: To control for intersubject variability, only the five off-road cyclists who completed all four competitions were included in the statistical analysis. The four races' mean absolute and relative time expressed in percentage of race duration (147 ± 15 min) spent in the EASYZONE (HR below LT) were 27 ± 16 min and 18 ± 10%, in the MODERATEZONE (HR between LT and OBLA4) were 75 ± 19 min and 51 ± 9%, and in the HARDZONE (HR above OBLA4) were 44 ± 21 min and 31 ± 16%. The average HR was 171 ± 6 beats·min-1, corresponding to 90 ± 3% of maximum (84 ± 3% of O2max). Conclusion: This study shows that cross-country events are conducted at very high intensity, especially at the start of the race. Coaches must take into account the distribution of the effort and the high exercise intensity characteristic of mountain-biking cross-country events when prescribing specific training programs. Purpose: To assess the effects of the Rotor (ROT), a new pedaling system that makes each pedal independent from the other so that cranks are no longer fixed at 180°, on endurance cycling performance. Methods: Following a randomized design, eight subjects (noncyclists; age (mean ± SEM): 22 ± 1 yr; O2max: 51.8 ± 1.0 mL·kg-1·min-1) performed two bicycle-ergometer tests on separate days, one with the conventional pedaling system (CON) and the other one with ROT. Starting at 75 W, the power output was increased by 25 W at 3-min intervals until volitional exhaustion. Gas exchange parameters and blood lactate were measured for every 3-min interval. Results: At exercise intensities between 60 and 90% O2max, delta efficiency (DE) was significantly higher in ROT than in CON (24.4 ± 1.9% vs 21.1 ± 1.1%, respectively). Conclusions: Although more research is needed, especially with trained riders, the Rotor system might improve delta efficiency during endurance cycling. Other performance determinants (O2max, maximal power output) do not seem to be changed compared with the conventional system. Hematological indices in elite cyclists Y. O Schumacher, R Jankovits, D Bültermann, A Schmid, A Berg Background: Blood testing is a major concern in sports. Sporting federations have introduced cutoff values for hematological variables to limit blood manipulations. Do date, no reference margins have been established to adjust single- or multivariable blood tests to the exercise adapted blood cell system of athletes. We studied hemoglobin (Hb), hematocrit (Hct), blood cell variables and changes in vascular volumes in male and female national team cyclists to evaluate the influence of exercise on these variables and the results of blood tests, and to estimate normal ranges in athletes for blood tests. Methods: 1628 blood samples of 169 male and 55 female athletes were considered. Samples were analyzed automatically, blood cell indices and vascular volumes were calculated. Overall averages and seasonal differences were estimated. Results: Hb-average was: 15.4 ± 0.8 g dL1 for male and 13.9 ± 0.7 g dL1 for female cyclists, Hct: 45 ± 2.9% and 40.7 ± 2.7%. Blood variables and vascular volumes showed significant seasonal changes. 1-6% of all samples were found above the currently used limits for blood testing. This is in accordance with the data from the normal population. Nevertheless, EPO misuse or blood manipulations cannot totally be excluded. Conclusion: Athletes display seasonal adaptations in their blood profile which should be considered in testing regulations. The presented data might be used as reference values for indirect single- and multivariable blood tests. Effects of L-tyrosine and carbohydrate ingestion on endurance exercise performance Troy D. Groeiremming door sporten? Adolescent athletes may be at risk of restricted growth and delayed maturation when intense training is combined with insufficient energy intake. Because catch-up growth commonly occurs when training is reduced or ceases, final adult stature may not be compromised. However, in athletes who have long-term, clinically delayed maturation, catch-up growth may be incomplete. Charting individual growth patterns assists in the identification of athletes at increased risk of reduced growth. This then allows physicians, coaches, and athletic trainers to modify the training environment, specifically reducing training intensity and increasing energy intakes. Elite young athletes undertake training programs of progressive intensity at an early age and eventually compete at the national and international level in sports such as gymnastics, swimming, soccer, tennis, and dancing during their teenage years.1 For instance, talented young female gymnasts often commence training at age 5 or 6 and train more than 20 to 30 hours per week year-round throughout childhood and adolescence.2 Young tennis players may spend up to 6 hours per day in court play and practice, and some adolescent runners train up to 80 miles per week for marathons.3 Short stature and delayed maturation in some young athletes may reflect sport-specific selection practices, but some evidence suggests that prolonged, intensive physical training combined with insufficient energy intake may reduce growth and delay maturation. Other concerns include poor dietary habits, increased physical loading and repetitive trauma to open epiphyses, and psychological and emotional stress. Performance Enhancement With Maintenance of Resting Immune Status After Intensified Cycle Training *Rudolph H. Dressendorfer, PhD, PT; *Stewart R. Petersen, PhD; *Shona E. However, it is not established whether intensified endurance training alters resting immune status or increases the risk of upper respiratory infection (URI). Purpose: This study examined the effect of intensified endurance training for performance enhancement on resting immune status in nine healthy, male competitive cyclists. Design: Data were collected during 4 weeks of usual training (baseline), followed by prescribed cycle training that consisted of volume-building at customary training intensity (V phase, 6 weeks), unaccustomed very high intensity interval training at 100% maximal heart rate (I phase, 18 days), and an unloading taper (U phase, 10 days). Methods: The main performance criterion was a simulated 20 km time-trial. Aerobic capacity measures included power output at ventilatory threshold (POTvent) and maximal oxygen uptake (VO2max). Markers of immune status (lymphocyte subset counts, serum cytokine levels, and new URI cases) and physiological indicators of training stress (cycling economy, 24-hour urinary cortisol excretion, and serum testosterone concentration) were evaluated in the rested state, 36 to 44 hours postexercise, during baseline, and after each training phase. Results: Time-trial performance, POTvent, VO2max, and cycling economy improved significantly (p < 0.001) after the V phase, and remained higher than baseline (p < 0.001) after the I and U phases. As compared with the V phase, performance time was faster after the U phase (p < 0.01). In contrast, lymphocyte counts, cytokine levels, incidence of URI, cortisol excretion, and serum testosterone concentration were not significantly different from baseline in any phase.

Conclusions: Cycling efficiency and performance improved while resting immune status was maintained throughout the 10-week training program. This study provides encouraging data in support of immunological robustness during intensified endurance training. Effects of Intranasal Budesonide on Symptoms, Quality of Life, and Performance in Elite Athletes With Allergic Rhinoconjunctivitis *Constance H. Katelaris, MD, PhD; *Fiona M. Burke, RN; †Karen Byth, PhD Objective: To assess change in symptoms, quality of life (QOL), and performance ability before, during, and after treatment with budesonide in a group of Olympic and Paralympic athletes with seasonal allergic rhinoconjunctivitis (SAR/C). Design: Because budesonide has already been proven to be an effective and well-tolerated treatment of SAR/C, 1 an open-label treatment format was used. Setting: The study was community-based with participating athletes preparing for Olympic competition. Participants: Olympic and Paralympic athletes were screened for the presence of SAR/C using history and positive skin test results for pollen allergens. Interventions: All were offered treatment with intranasal budesonide, applied to each nostril, once daily for eight weeks. Outcome Measurements: Symptom and medication diaries were completed before treatment and after 4 and 8 weeks of treatment. Similarly, Quality of Life (QOL) was measured with the Rhinoconjunctivitis Quality of Life Questionnaire. As a secondary outcome measure, the ability to train and compete was assessed using a performance diary. Results: Of the 236 athletes eligible for the study, 145 (61%) agreed to participate. Forty-six percent of the athletes who were dispensed treatment did not return questionnaires. For those returning questionnaires, scores between baseline (week 0) and weeks 4 and 8 were calculated for total symptoms, QOL, and performance scores. There were statistically significant improvements in symptoms, QOL, and performance scores in athletes who used intranasal budesonide. Conclusion: SAR/C is a common condition and has demonstrable negative effects on athletes. Better education of coaches and athletes is necessary to ensure that the condition is correctly diagnosed and treated, with safe, effective, permitted medication. Setting: The study was carried out at the South African Ironman triathlon. Participants:

Thirty-eight athletes competing in the triathlon were given salt tablets to ingest during the race. Data collected from these athletes [salt intake group (SI)] were compared with data from athletes not given salt [no salt group (NS)]. Interventions: Salt tablets were given to the SI group to provide approximately 700 mg/h of sodium. Main Outcome Measurements: Serum sodium, hemoglobin, and hematocrit were measured at race registration and after the race. Weights were measured before and after the race. Members of SI were retrospectively matched to subjects in NS for 1) weight change and 2) pre-race [Na]. Results: The SI group developed a 3.3-kg weight loss (p < 0.0001) and significantly increased their [Na] (Δ[Na] 1.52 mmol/L; p = 0.005). When matched for weight change during the race, SI increased their [Na] compared with NS (mean 1.52 versus 0.04 mmol/L), but this did not reach statistical significance (p = 0.08). When matched for pre-race [Na], SI had a significantly smaller percent body weight loss than NS (-4.3% versus -5.1%; p = 0.04). There was no significant difference in the increase of [Na] in both groups (1.57 versus 0.84 mmol/L). PV increased equally in both groups. None of the subjects finished the race with [Na] < 135 mmol/L. Conclusions: Sodium ingestion was associated with a decrease in the extent of weight loss during the race. There was no evidence that sodium ingestion significantly influenced changes in [Na] or PV more than fluid replacement alone in the Ironman triathletes in this study. Sodium supplementation was not necessary to prevent the development of hyponatremia in these athletes who lost weight, indicating that they had only partially replaced their fluid and other losses during the Ironman triathlon. Zernicke, PhD Objective: Ankle joint complex dynamics developed during volleyball spike jumps take-offs and landings were quantified to assess potential relations between these joint dynamics and patellar tendinopathy. Design: Three-dimensional kinematic data provided information about movements of the lower limbs, while the kinetic data permitted analysis of ground reaction forces as players took-off and landed from full-speed spike jumps. Setting: Simulated volleyball court with net in a biomechanics research laboratory. Participants: 10 members of the Canadian Men's National Volleyball Team. From history and physical examination, 3 of the 10 players had patellar tendon pain associated with activity and were diagnosed with patellar tendinopathy at the time of the study. Investigators were blinded about the injury status of the players. Interventions: None. Main Outcome Measures: Three-dimensional kinematics and joint moments of the ankle, knee, and hip joints. Results: Our analysis revealed that maximal external tibial rotation occurred at or near maximal dorsiflexion while maximal internal tibial rotation coincided with maximal plantarflexion. The plantarflexion moment was 3 to 10 times greater than all the other moments measured, with the maximal plantarflexor moment being calculated at 0.4 BWm (360 Nm). In blinded logistic regression analyses, we found one of the dynamics variables (inversion moment during the landing of the spike jump) was a significant predictor of patellar tendinopathy.

Conclusions: Coupling the results of the current analysis of ankle joint complex dynamics with previously reported results of knee joint dynamics related to patellar tendinopathy suggests that a cluster of variables linked to patellar tendinopathy includes: high ankle inversion-eversion moments, high external tibial rotation and plantarflexion moments, large vertical ground reaction forces, and high rate of knee extensor moment development. Problems include invalid screening tests, failure to understand the problems of intersex, the discriminatory singling out of women based only on laboratory results, and the stigmatization and emotional trauma experienced by individuals screened positive. Genuine sex-impostors have not been uncovered by laboratory-based genetic testing; however, gender verification procedures have resulted in substantial harm to a number of women athletes born with relatively rare genetic abnormalities. Individuals with sex-related genetic abnormalities raised as females have no unfair physical advantage and should not be excluded or stigmatized, including those with 5-α-steroid-reductase deficiency, partial or complete androgen insensitivity, and chromosomal mosaicism. In 1990, the International Amateur Athletics Federation (IAAF) called for ending genetic screening of female athletes and in 1992 adopted an approach designed to prevent only male impostors from competing. The IAAF recommended that the "medical delegate" have the ultimate authority in all medical matters, including the authority to arrange for the determination of the gender of the competitor if that approach is judged necessary. The new policy advocated by the IAAF, and conditionally adopted by the International Olympic Committee, protects the rights and privacy of athletes while safeguarding fairness of competition, and the American Medical Association recommends that it become the permanent approach. Purpose: This study investigated cardiovascular and hemodynamic responses during the transition from rest to electrical stimulation-induced leg cycling exercise (ES-LCE) in individuals with chronic paraplegia (PARA). Methods: Ten PARA (T4-T9; ASIA A) participated in this study. Heart rate (HR), mean arterial pressure (MAP), stroke volume (SV), and cardiac output () were measured on a beat-to-beat basis at rest and during the first 60 s of ES-LCE. Results: PARA exhibited two discrete MAP responses during ES-LCE. Those with high thoracic lesions (HIGH: T4-T6, N = 5) responded to ES-LCE with a significant rise in MAP (maxΔ 8.3 ± 3.6 mm Hg), whereas MAP did not exhibit any sustained change from resting values during ES-LCE in those subjects with lower lesions (LOW: T7-T9, N = 5). In HIGH PARA, the immediate increase in MAP corresponded to a decrease in HR (maxΔ 6.8 ± 3.1 b·min-1), which returned toward resting levels by the end of 60 s. In contrast, for LOW PARA there was no change in HR from resting levels during transition to ES-LCE. In both subgroups, SV and were not significantly increased during ESLCE. Conclusion: These results suggest that the on-transient responses of MAP during ES-LCE in HIGH PARA elicited reflex changes in HR via the arterial baroreflex, whereas in LOW PARA, an unchanged HR from rest was likely due to a constant MAP during ES-LCE. Isoflavones (ISO) appear to reduce the inflammatory pathway through their role as a tyrosine kinase inhibitor. Delayed onset muscle soreness (DOMS) develops after intense exercise and has been associated with an inflammatory response. Therefore, we hypothesized that physical parameters associated with DOMS could be decreased via the modulation of the inflammatory response by supplementing subjects with either FO or ISO. Methods: 22 subjects were recruited and randomly assigned to one of three treatment groups: FO (1.8 g of omega-3 fatty acids·d-1), ISO (120 mg soy isolate·d-1), or placebo (PL) (Western fat blend and/or wheat flour). All treatment groups received 100-IU vitamin E·d-1 to minimize lipid peroxidation of more highly unsaturated fatty acids. Subjects were supplemented 30 d before the exercise and during the week of testing and were instructed to refrain from unusual exercise. DOMS was induced by 50 maximal isokinetic eccentric elbow flexion contractions. Strength parameters, pain, arm circumference, and relaxed arm angle (RANG) were measured at 48, 72, and 168 h post exercise. Cortisol, creatine kinase (CK), interleukin-6 (IL-6), tumor necrosis factor (TNFα), malondialdehyde (MDA), and serum iron were measured before supplementation, after supplementation, and post exercise. Results: Significant decreases were observed in RANG and strength 48 h postexercise among all groups, and there were significant increases in pain and arm circumference. There were no significant changes among all groups from baseline at 168 h (7 d) post exercise. There were no significant treatment effects between groups for the physical parameters or for cortisol, CK, IL-6, TNFα, MDA, or serum iron. Conclusion: These data indicate FO or ISO, at the doses supplemented, were not effective in ameliorating DOMS with the above-cited protocol. Purpose: The purpose of the present study was to determine the effect of a prolonged bout of exercise in freezing cold conditions on saliva immunoglobulin A (s-IgA) responses in endurance-trained males. Methods: Using a randomized cross-over design, 15 trained male cyclists cycled for 2 h on a stationary ergometer at 70% O2max in an environmental chamber on one occasion at a temperature of -6.4 ± 0.1°C (cold) and on another occasion at a temperature of 19.8 ± 0.2°C (control). Trials began at 12:30 h to avoid the fall in s-IgA concentration that occurs during the morning hours. Unstimulated whole-saliva samples were collected over a 2-min period at preexercise, postexercise, and 2-h postexercise. The s-IgA concentration was determined using a sandwich-type ELISA method. Results: Saliva flow rate decreased postexercise by 31%, returning to preexercise levels by the 2-h postexercise collection (main effect of time: P < 0.01). The decrease in saliva flow rate postexercise in the control trial (39% compared with 22% on cold trial) approached significance (interaction: P = 0.08) and may have accounted for the corresponding increase in s-IgA concentration postexercise in the control trial (s-IgA concentration: control preexercise; 91 ± 12; postexercise; 110 ± 13 mg·L-1; P < 0.05). Saliva IgA secretion rate decreased postexercise by 19.5% returning to preexercise levels by 2-h postexercise measure (main effect of time: P < 0.05). Conclusions:

These data show that performing a bout of prolonged exercise results in a reduction in s-IgA secretion rate. Additionally, these data demonstrate that performing prolonged exercise in freezing cold conditions does not influence saliva flow rate or s-IgA secretion rate responses to prolonged exercise. Purpose: This study was designed to examine the effects of cycling position (seated or standing) during level-ground and uphill cycling on gross external efficiency (GE) and economy (EC). Methods: Eight well-trained cyclists performed in a randomized order five trials of 6-min duration at 75% of peak power output either on a velodrome or during the ascent of a hill in seated or standing position. GE and EC were calculated by using the mechanical power output that was measured by crankset (SRM) and energy consumption by a portable gas analyzer (Cosmed K4b2). In addition, each subject performed three 30-s maximal sprints on a laboratory-based cycle ergometer or in the field either in seated or standing position. Results: GE and EC were, respectively, 22.4 ± 1.5% (CV = 5.6%) and 4.69 ± 0.33 kJ·L-1 (CV = 5.7%) and were not different between level seated, uphill seated, or uphill standing conditions. Heart rate was significantly (P < 0.05) higher in standing position. In the uphill cycling trials, minute ventilation was higher (P < 0.05) in standing than in seated position. The average 30-s power output was higher (P < 0.01) in standing (803 ± 103 W) than in seated position (635 ± 123 W) or on the stationary ergometer (603 ± 81 W). Conclusion: Gradient or body position appears to have a negligible effect on external efficiency in field-based high-intensity cycling exercise. Greater short-term power can be produced in standing position, presumably due to a greater force developed per revolution. However, the technical features of the standing position may be one of the most determining factors affecting the metabolic responses. Relationship of physical activity to eating behaviors and weight loss in women JAKICIC, J. M., R. R. WING, and C. WINTERS-HART. Purpose: To examine whether change in physical activity is associated with compliance to changes in dietary intake and eating behaviors in an 18-month behavioral weight loss program, and to examine the contribution of exercise to weight loss when these other weight loss behaviors are also considered. Methods: Data from 104 subjects who completed an 18-month behavioral weight loss program were analyzed in this study. All subjects were prescribed a reduced energy (1200-1500 kcal·d-1) and fat (20 to 30%) diet, and exercise progressed from 100 to 200 min·wk-1. Subjects attended group behavioral lessons throughout the study. Weight, physical activity, energy intake, and weight loss eating behaviors were assessed at 0 and 18 months. Results: Body weight decreased 7.8 ± 7.5 kg and body mass index decreased 2.8 ± 2.7 kg·m-2 from 0 to 18 months (P < 0.05). Total energy intake (kcal·d-1) and macronutrient intake (g·d-1) decreased, whereas physical activity and eating behaviors associated with weight loss increased from 0 to 18 months (P < 0.05). Change in physical activity was significantly correlated with weight loss (r = 0.33), reductions in energy intake (r = 0.20), and improvements in eating behaviors associated with weight loss (r = 0.24) (P < 0.05). Regression analysis indicated that change in physical activity significantly improved weight loss after changes in energy intake and weight loss eating behaviors were considered in the analysis, with R2 significantly improving by approximately 0.04 (P < 0.05). However, results from multiple regression showed weight loss was influence more by

changes in eating behaviors than changes in physical activity (R2 = 0.17 vs R2 = 0.04). Conclusions: The combination of changes in eating and physical activity behaviors can improve long-term weight loss compared with either behavior alone. Interventions targeting both behaviors are recommended for improving long-term weight loss. Autonomic adaptations to intensive and overload training periods: a laboratory study PICHOT, V., T. Sports Exerc., Vol. 34, No. 10, pp. 1660-1666, 2002. Purpose: Looking for practical and reliable markers of fatigue is of particular interest in elite sports. One possible marker might be the autonomic nervous system activity, known to be well affected by physical exercise and that can be easily assessed by heart rate variability. Methods: We designed a laboratory study to follow six sedentary subjects (32.7 ± 5.0 yr) going successively through 2 months of intensive physical training and 1 month of overload training on cycloergometer followed by 2 wk of recovery. Maximal power output over 5 min (Plim5′), O2peak and standard indices of heart rate variability were monitored all along the protocol. Results: During the intensive training period, physical performance increased significantly (O2peak: +20.2%, P < 0.01; Plim5′: +26.4%, P < 0.0001) as well as most of the indices of heart rate variability (mean RR, Ptot, HF, rMSSD, pNN50, SDNNIDX, SDNN, all P < 0.05) with a significant shift in the autonomic nervous system toward a predominance of its parasympathetic arm (LF/HF, LFnu, HFnu, P < 0.01). During the overload training period, there was a stagnation of the parasympathetic indices associated to a progressive increase in sympathetic activity (LF/HF, P < 0.05). During the week of recovery, there was a sudden significant rebound of the parasympathetic activity (mean RR, HF, pNN50, rMSSD, all P < 0.05). After 7 wk of recovery, all heart rate variability indices tended to return to the prestudy values. Conclusion: Autonomic nervous system status depends on cumulated physical fatigue due to increased training loads. Therefore, heart rate variability analysis appears to be an appropriate tool to monitor the effects of physical training loads on performance and fitness, and could eventually be used to prevent overtraining states.

Exercise endurance 1, 3, and 6 h after caffeine ingestion in caffeine users and nonusers Douglas G. Bell and Tom M. McLellan Defence R&D Canada-Toronto, Toronto, Ontario, Canada M3M 3B9 The purpose of the present study was to examine the duration of caffeine's ergogenic effect and whether it differs between users and nonusers of the drug. Twenty-one subjects (13 caffeine users and 8 nonusers) completed six randomized exercise rides to exhaustion at 80% of maximal oxygen consumption after ingesting either a placebo or 5 mg/kg of caffeine. Exercise to exhaustion was completed once per week at either 1, 3, or 6 h after placebo or drug ingestion. Exercise time to exhaustion differed between users and nonusers with the ergogenic effect being greater and lasting longer in nonusers. For the nonusers, exercise times 1, 3, and 6 h after caffeine ingestion were 32.7 ± 8.4, 32.1 ± 8.6, and 31.7 ± 12.0 min, respectively, and these values were each significantly greater than the corresponding placebo values of 24.2 ± 6.4, 25.8 ± 9.0, and 23.2 ± 7.1 min. For caffeine users, exercise times 1, 3, and 6 h after caffeine ingestion were 27.4 ± 7.2, 28.1 ± 7.8, and 24.5 ± 7.6 min, respectively. Only exercise times 1 and 3 h after drug ingestion were significantly greater than the respective placebo trials of 23.3 ± 6.5, 23.2 ± 7.1, and 23.5 ± 5.7 min. In conclusion, both the duration and magnitude of the ergogenic effect that followed a 5 mg/kg dose of caffeine were greater in the nonusers compared with the users.

After 2.5 ± 0.1 h of intense cycling to deplete the muscle glycogen stores, subjects (n = 7) received, using a rank-ordered design, a CHO-Pro (80 g CHO, 28 g Pro, 6 g fat), LCHO (80 g CHO, 6 g fat), or HCHO (108 g CHO, 6 g fat) supplement immediately after exercise (10 min) and 2 h postexercise. Before exercise and during 4 h of recovery, muscle glycogen of the vastus lateralis was determined periodically by nuclear magnetic resonance spectroscopy. Exercise significantly reduced the muscle glycogen stores (final concentrations: 40.9 ± 5.9 mmol/l CHO-Pro, 41.9 ± 5.7 mmol/l HCHO, 40.7 ± 5.0 mmol/l LCHO). After 240 min of recovery, muscle glycogen was significantly greater for the CHO-Pro treatment (88.8 ± 4.4 mmol/l) when compared with the LCHO (70.0 ± 4.0 mmol/l; P = 0.004) and HCHO (75.5 ± 2.8 mmol/l; P = 0.013) treatments. Glycogen storage did not differ significantly between the LCHO and HCHO treatments. There were no significant differences in the plasma insulin responses among treatments, although plasma glucose was significantly lower during the CHO-Pro treatment. These results suggest that a CHOPro supplement is more effective for the rapid replenishment of muscle glycogen after exercise than a CHO supplement of equal CHO or caloric content.

In fact, intracellular glutamine concentration may not be compromised when plasma levels are decreased postexercise. In addition, a number of recent intervention studies with glutamine feeding demonstrate that, although the plasma concentration of glutamine is kept constant during and after acute, strenuous exercise, glutamine supplementation does not abolish the postexercise decrease in in vitro cellular immunity, including low lymphocyte number, impaired lymphocyte proliferation, impaired natural killer and lymphokine-activated killer cell activity, as well as low production rate and concentration of salivary IgA. It is concluded that, although the glutamine hypothesis may explain immunodepression related to other stressful conditions such as trauma and burn, plasma glutamine concentration is not likely to play a mechanistic role in exercise-induced immunodepression. Published online May 3, 2002; 10.1152/japplphysiol.00982.2001 The metabolic cost of producing submaximal cycling power has been reported to vary with pedaling rate. Pedaling rate, however, governs two physiological phenomena known to influence metabolic cost and efficiency: muscle shortening velocity and the frequency of muscle activation and relaxation. The purpose of this investigation was to determine the relative influence of those two phenomena on metabolic cost during submaximal cycling. Nine trained male cyclists performed submaximal cycling at power outputs intended to elicit 30, 60, and 90% of their individual lactate threshold at four pedaling rates (40, 60, 80, 100 rpm) with three different crank lengths (145, 170, and 195 mm). The combination of four pedaling rates and three crank lengths produced 12 pedal speeds ranging from 0.61 to 2.04 m/s. Metabolic cost was determined by indirect calorimetery, and power output and pedaling rate were recorded. A stepwise multiple linear regression procedure selected mechanical power output, pedal speed, and pedal speed squared as the main determinants of metabolic cost (R2 = 0.99 ± 0.01). Neither pedaling rate nor crank length significantly contributed to the regression model. The cost of unloaded cycling and delta efficiency were 150 metabolic watts and 24.7%, respectively, when data from all crank lengths and pedal speeds were included in a regression. Those values increased with increasing pedal speed and ranged from a low of 73 ± 7 metabolic watts and 22.1 ± 0.3% (145-mm cranks, 40 rpm) to a high of 297 ± 23 metabolic watts and 26.6 ± 0.7% (195-mm cranks, 100 rpm). These results suggest that mechanical power output and pedal speed, a marker for muscle shortening velocity, are the main determinants of metabolic cost during submaximal cycling, whereas pedaling rate (i.e., activation-relaxation rate) does not significantly contribute to metabolic cost. Assessing the efficacy of exercise training in patients with chronic disease PELAGIA KOUFAKI; PATRICK F. NASH; THOMAS H. MERCER Purpose: The purpose of this study was to evaluate the meaningfulness of exercise training responses in patients with end stage renal disease (ESRD). Methods: Eighteen ESRD patients [(mean ± SD); 54.3 ± 17.1 yr] completed a training regime progressing to accumulate 40 min of stationary cycling, three times per week for 6 months. O2peak determined via incremental cycle ergometer protocol, and O2 kinetics determined from a transition from unloaded pedalling to an exercise intensity corresponding to 90% of VT, were assessed at baseline and at 3 and 6 months of training. Results: Repeated measures analysis of variance revealed significant changes (P < 0.05) on the time factor for O2peak, O2-VT, and O2 kinetics. Post hoc analysis revealed that O2peak and O2 kinetics significantly (P < 0.05) improved at 3 months of training with no further improvements thereafter. Analysis of individual subject response data revealed that after 3 months of training, 61% of the patients improved O2peak by greater than the standard error of measurement (SEM = 0.07 L·min–1). At 6 months of training, 89% of the patients improved by more than the SEM O2 kinetics improved by more than the SEM (12.3 s) at 3 months of training in 55% of the patients, with no increase in the number of patients exhibiting faster time constants after 6 months of training. Conclusion: Although conventional statistical analyses indicate that exercise training favorably alters O2peak and oxygen uptake kinetics of patients with end stage renal disease (ESRD), it is apparent that considerable interindividual variability exists in the response to training.

Consideration of the SEM data underscores the heterogeneity of adaptive response in this patient group and may be valuable in assessing the efficacy of therapeutic exercise rehabilitation. Published online May 31, 2002; 10.1152/japplphysiol.00224.2002 This study investigated the effect of long-term high-fat sucrose (HFS) or low-fat complexcarbohydrate (LFCC) diet consumption on leptin, insulin, fat cell size, energy intake, and markers of activity to ascertain the role that leptin plays in long-term energy balance in a model of dietinduced obesity. Female Fischer 344 rats were fed either a HFS or LFCC diet ad libitum for a period of 20 mo. Measurements of leptin concentration, insulin concentration, and adipocyte size were performed at 2 wk, 2 mo, 6 mo, and 20 mo. Body weight and energy intake were measured weekly for calculation of feed efficiency. Body temperature and activity levels were assessed over a 5-day period after 12 mo of the dietary intervention. Plasma leptin and insulin concentrations were significantly elevated within 2 wk of HFS diet consumption and remained elevated throughout the course of the study. After 2 mo, the adipocytes of the HFS group were significantly larger and continued to increase in size throughout the course of the study. A significant correlation was noted between leptin and adipocyte cell size (r = 0.96, P < 0.01). However, despite elevated leptin, energy intake was similar, and the HFS group weighed significantly more than the LFCC group, as a result of a higher feed efficiency. There were no significant differences in body temperature or activity levels between the groups. These results demonstrate that a HFS diet causes hyperleptinemia and hyperinsulinemia before adipocyte size is increased and suggests that leptin resistance may be present or, alternatively, that leptin does not to play a major role in the longterm regulation of energy intake or activity levels in this model. Purpose: A “J”-shaped model has been proposed to describe the relationship between physical activity and risk of upper-respiratory tract infection (URTI). However, little epidemiologic evidence is available to support the contention that moderately active individuals are at lowest risk of URTI. This investigation examined differences in URTI risk between physically inactive and moderately active adults. Methods: Observational study of 547 healthy adults (49% women) aged 20–70 yr reported URTI events at 90-d intervals over 12-month of follow-up (5 evaluations). Three 24-h physical activity recalls per evaluation were obtained and averaged to quantify total moderate-vigorous activity (3.0 metabolic equivalents [MET]). Associations between URTI and physical activity levels were estimated using incidence rate ratios (IRR) derived using Poisson regression while adjusting for a number of potential confounders including age, education, anxiety, cynicism, and selected dietary factors. Results: Men and women reported 1.2 (1.4) and 1.2 (1.2) URTI events per year, respectively (mean [(SD]). Adjusting for gender and potential confounders, the IRR for less than 3.93, 3.94– 7.15, 7.16–11.95, and 11.96 MET-h·d–1 among men, and less than 2.38, 2.39–4.09, 4.10–6.24, and 6.25 MET-h·d–1 among women, were 1.00 (referent), 0.87 (95% confidence interval [CI], 0.70–1.07), 0.88 (95% CI, 0.71–1.09), and 0.77 (95% CI, 0.62–0.95), respectively (Ptrend = 0.03). This effect was stronger in men (Ptrend = 0.03) than women (Ptrend = 0.17), although at similar expenditure levels (6-7 MET-h·d–1), risk was reduced by about 20% in men and women. Risk reduction was most pronounced in the fall of the year (Ptrend = 0.02). Conclusions: These data support the hypothesis that moderate levels of physical activity are associated with a reduced risk for URTI. It was hypothesized that the reduction of high-energy phosphates in muscle after repeated sprints is smaller in women than in men. Fifteen healthy and physically active women and men with an average age of 25 yr (range of 19-42 yr) performed three 30-s cycle sprints (Wingate test) with 20 min of rest between sprints. Repeated blood and muscle samples were obtained. Freeze-dried pooled muscle fibers of types I and II were analyzed for high-energy phosphates and their breakdown products and for glycogen. Accumulation of plasma ATP breakdown products, plasma catecholamines, and blood lactate, as well as glycogen reduction in type I fibers, was all lower in women than in men during sprint exercise. Repeated sprints induced smaller reduction of ATP and smaller accumulation of IMP and inosine in women than in men in type II muscle fibers, with no gender differences in changes of ATP and its breakdown products during the bouts of exercise themselves. This indicates that the smaller ATP reduction in women than in men during repeated sprints was created during recovery periods between the sprint exercises and that women possess a faster recovery of ATP via reamination of IMP during these recovery periods.

Jeukendrup Time course of performance changes and fatigue markers during intensified training in trained cyclists J Appl Physiol 93: 947-956, 2002. Published online April 5, 2002; 10.1152 To study the cumulative effects of exercise stress and subsequent recovery on performance changes and fatigue indicators, the training of eight endurance cyclists was systematically controlled and monitored for a 6-wk period. Subjects completed 2 wk of normal (N), intensified (ITP), and recovery training. A significant decline in maximal power output (N = 338 ± 17 W, ITP = 319 ± 17 W) and a significant increase in time to complete a simulated time trial (N = 59.4 ± 1.9 min, ITP = 65.3 ± 2.6 min) occurred after ITP in conjunction with a 29% increase in global mood disturbance. The decline in performance was associated with a 9.3% reduction in maximal heart rate, a 5% reduction in maximal oxygen uptake, and an 8.6% increase in perception of effort. Despite the large reductions in performance, no changes were observed in substrate utilization, cycling efficiency, and lactate, plasma urea, ammonia, and catecholamine concentrations. These findings indicate that a state of overreaching can already be induced after 7 days of intensified training with limited recovery. Of dit daadwerkelijk voordelen heeft voor de gezondheid, is echter nog maar de vraag. Een studie door Senior onderzoeker David Bender van het departement of biochemistry and molecular biology van de University College of London, heeft uitgewezen dat vitaminepillen soms meer kwaad dan goed doen. Zo is bewezen dat een hoge dosis caroteen wordt geassocieerd met een lager risico op long-, prostaat-, en andere kankers. Maar anti-oxidanten als caroteen, vormen vrije radicalen die in weefsels kunnen doordringen en daar schade aan kunnen richten. Twee studies, een onder Finse rokers en een onder asbestslachtoffers in de Verenigde Staten, hebben zelfs onlangs bewezen dat de groep mensen die caroteen kreeg toegediend vaker stierf aan long-, en andere vormen van kanker dan de mensen die een placebo kregen. Negatieve verschijnselen van een hoge dosis van de anti oxidant vitamine C zijn eveneens vastgesteld. De conclusie luidt: een dagelijks multivitamine tablet is alleen nuttig in geval van een slechte voeding. Voor het overige doen vitaminesupplementen ons geen goed, met uitzondering van vitamine D voor ouderen en foliumzuur voor zwangeren. Newer compounds are coming on the market capable of boosting red blood cell concentration and thus improving aerobic activities in particular. Erythropoietin (EPO) has been used extensively in the past by the athletic community in this role. Its main disadvantages are the side effects due to increased viscosity, and the recent development of a blood test for drug testing methods. Two new methods of increasing red blood cell concentration for use in medicine are hemoglobin oxygen carriers and perfluorocarbons, each having a different structure, but which allow oxygen to be delivered to the tissues. Hemoglobin oxygen carriers physically alter the hemoglobin molecule by several methods so that complications such as renal toxicity are obviated. Perfluorocarbons belong to a group of synthetic compounds containing hydrocarbons to which fluorine is added. Evidence is available to suggest that athletes are already adapting these newer molecules for their benefit.

In many cases, the prescription of IBAs (Inhalatie B2.s.m.m.) to this population has been made on empirical grounds. Beginning with the 2002 Winter Games, athletes will be required to submit to the IOC Medical Commission clinical and laboratory evidence that justifies the use of this medication. The eucapnic voluntary hyperpnea test will be used to assess individuals who have not satisfied an independent medical panel of the need to use an IBA. In a randomized crossover study of the effect of adaptation to exercise in the heat on endurance performance at room temperature, nine highly-trained male cyclists performed a simulated 40-km time trial at 20°C on an air-braked ergometer, preceded by 7 d of self-selected training on a cycle ergometer in the heat (90 min.d-1 at 37°C and 50% humidity) or 7 d of control training (90 min.d1 at 20°C and 50% humidity) at similar perceived effort. A 2-wk washout period separated these acclimation periods, during which athletes replicated training recorded in the two weeks before their first acclimation period. The measures of acclimation were changes in heart rate, core temperature, whole-body sweat rate, and perceived exertion during a ride at fixed intensity on the first and last day of each acclimation period; an additional measure was the hematocrit on the day after each acclimation period. The observed mean effect of training in the heat on performance time in the 40-km time trial was 0.4% (95% confidence interval -1.5 to 2.2%). The chances that the true effect on performance is beneficial/trivial/harmful are 35/54/11%. Only one-third of the cyclists showed substantial changes in perceived exertion and heart rate consistent with heat acclimation, and the individual differences in heat acclimation did not relate consistently to performance in the time trial. In conclusion, moderately intense training in the heat produced only modest variable heat acclimation and only the possibility of a worthwhile enhancement of performance. Highly trained individuals probably need a more marked stimulus to achieve substantial heat adaptation and effects on performance. KEYWORDS: athlete, endurance, ergogenic, heat acclimation. Extra Aminozuren en eiwitten nodig? Digestion of dietary protein yields amino acids, which, after being absorbed, are available for metabolism and the growth and repair of all tissues in the body. Because many athletes are purposefully trying to add more muscle, most believe the protein content of the diet should be considerably higher than that of non-athletes. It is true that on average, athletes require slightly more protein (1.2-1.5 g/kg body weight; 0.5-0.7 grams/lb) than do less-active people (0.8-1.0 g/kg body weight; 0.4-0.5 grams/lb). However, it is far from true that in order to meet this additional need, athletes must rely upon protein and amino acid supplements. Further, there is simply no scientific evidence to support the idea that the protein or amino acids in supplements are more effective for athletes than protein in ordinary foodshe best scientific studies have been unable to show that branched-chain amino acid ingestion during exercise benefits performance9,10. In fact, a potential side effect of amino acid ingestion during exercise is excess ammonia accumulation (a by-product of amino acid breakdown), which could contribute to fatigue11. We can only conclude on the basis of published research that ingesting amino acids (and likely protein) during exercise has no effect on exercise performance. Although future publications may provide a different perspective, it is hard to imagine how ingesting small quantities of protein or amino acids during exercise could benefit performance.

De (sport)voeding bevat bij voorkeur 20-30% vet, ongeveer 1 gr/kg lichaamsgewicht. Zeer vetarm dieet kan ongezond zijn voor een sporter en zelfs de prestatie ongunstig beïnvloeden. Dit berust op een tekort aan calorieen, mineralen en essentiele vetzuren. Op de Universiteit van Buffalo., deed men onderzoek met 25 hardlopers, 12 mannen en 13 vrouwen. Het dieet vareerde van 16%, 31% en 44% vet. In de praktijk bleef de voeding achter bij het energie verbruik, maar dit was het sterkst bij de vetarme voeding. In de periode van vetarme voeding bleek men 19% minder calorieen te gebruken dan in de periode met het andere dieet, vooral de vrouwen kwamen dan tot slechts 60% van de natuurlijke behoefte. Overgaande op meer vet in het dieet nam een uithoudingstest op 80% van de VO2max met 14% toe. Op een hoger vet% (44%) was er echter geen aanvullende verbetering van de prestatie.

Een meta-analyse naar het effect van KT op de (basis) bloeddruk geeft een reductie te zien van 2% in de systolische waarde en 4% in de diastolische waarde.(Kelley) Zijn bepalingen van het lactaat gehalte, belangrijk voor monitoring van wielrenners? In een onderzoek (Nichols, Phares en Buono) met 13 wat oudere vrouwelijke wielrenners (USA; gemiddeld 47 jaar oud) werd gekeken naar VO2max , lactaat drempel (LT) en naar prestaties bij een tijdritten, 13.5 km en 20 km. De VO2max was gemiddeld 50,6 ml/kg, erg hoog in vergelijking met andere sporters; de VO2max was matig (r=-0.67) gecorreleerd met de leeftijd. De lactaatwaarde in het bloed (BLC) was significant hoger dan op de LT (2.86 mmol ), zowel op de 13.5 km (7.59 mmol) als op de 20 km tijdrit (6.99 mmol ). De LT lag gemiddeld op 168 Watt en op 66% van de VO2max. The VO2, die met een BLC van 4.0 mmol, overeenkwam, lag op 72% van de VO2max. Zoals te verwachten, was de tijdrit sterk gecorreleerd met de VO2max (r=-0.85); een regressie analyse liet zien dat de power output (Watt) op de LT de beste voorspellende waarde had. (13.5 km: r2 = 0.83; 20 km: r2 = 0.78). De hartslag (HR) op de LT (die lag op 88% van de HRmax) was significant lager dan tijdens de tijdritten (op 92-94% van HRmax). Een conclusie luidt dat afstemmen van trainings intensiteit op de hartslag van de LT, een te lage inschatting geeft van de wedstrijd-intensiteit. Opvallend dat tijdens het congres in Groningen ook Erik Breukink aangaf dat wanneer hij een tijdrit reed op de – op grond van de testen aanbevolen – hartslag, hij te langzaam naar zijn mogelijkheden reed; daarom reed hij sindsdien zonder hartslagmeter maar volledig op zijn gevoel.

Hyperbare zuurstoftoediening na intensieve belastingen met spierschade, heeft geen nuttig effect op het herstel.

Hoewel het aantal belangrijke verwondingen relatief laag is (99 personen van de 20.769 personen in een studie), lopen bij het mountainbiken vrouwen 2 keer zo vaak een blessure op. Onder de blessures komen breuken bij vrouwen uit op 45% , tegenover 21% bij mannen. Ook bij andere sporten (bv. Voetbal) is het bekend dat vrouwen grotere risico’s op een blessure (bv. een kruisbandscheur) hebben.

Een hoog ferritine komt bij veel wielrenners voor als gevolg van ijzer-suppletie. Dit vindt bij 90% van de elite-wielrenners plaats. Het is niet zonder risico. Ijzertoediening moet daarom niet routinematig, maar slechts op geleide van de ferritine-spiegels plaats vinden. Vitamine E wordt vaak genoemd als middel om beschadiging (radicalen) bij sporten te voorkomen. In een goed opgezette studie, waarbij ook spierbiopsie werd gedaan, was echter geen enkel verschil t.o.v. een placebo.

Longfunktie na inspanningen (20 en km tijdrit) is verminderd. In een onderzoek met 16 wielrenners was de Maximale inspiratoirie zuurstofdruk (P0), gemeten 2 min na een 20- en 40-km tijdrit, 18% , resp. 13% verminderd en bleef onder de uitgangswaarde (voor de test) na 30 min. Door ademhalingsoefeningen was deze reductie 14%, resp. 6%. Ook was er een significante verbetering in 20- and 40-km tijdrit met 3.8 ± 1.7% en 4.6 ± 1.9%. Deze gegeven duiden op een ademhalingsspiermoeheid na inspanningen. Ademhalingsoefeningen hebben daarop, en ook op het prestatie vermogen een gunstige invloed.

Recent overleed een 28 jarige vrouw na een vliegreis, na haar bezoek aan de olympische spelen. De doodsoorzaak was longembolie t.g.v. diepe veneuze trombose (DVT) tijdens de vliegreis, ook wel economy class syndroom genoemd. Bijzonder risico lopen met name vrouwen, die homo- of heterozygoot zijn voor factor V Leiden, een puntmutatie in het factor V gen. Gebruikelijk is dan om voor een vliegreis 1 dosis laagmoleculair heparine te spuiten (faxiparine 2000 E) . Ongeveer 5% van de Europeanen is drager van factor V Leiden en 2% is drager van een protombine variant, beiden predisponerend voor DVT. Minder vaak voorkomende risicofactoren zijn deficientie van antithrombin III, of proteine C of S, of van een abnormaal fibrinogeen. Ook de pil, en met name de 3e generatie pil, verhoogt het risico. Gecombineerde risico factoren kunnen vooral bij langere vluchten leiden tot DVT. Weinig bewegen en onvoldoende hydratie tijdens de vlucht (maar ook in een lange bus- of treinreis) versterken de stolling. Jaarlijks krijgen in de USA 30.000 personen DVT tijdens een vliegreis! Bij de verkoop van vliegticket zou een sinasprilletje gegeven moeten worden……

Krijg je tijdens een tijdrit buiten met een intensiteit, afgestemd op de hartfrekwentie van de in het lab bepaalde MLLS, dezelfde laktaatwaarden als in het lab?

10 Wielrenners moesten op verschillende dagen een aantal testen doen in het lab en een 5 en 10 km tijdrit buiten. De MLLS in het lab lag gemiddeld op een hartslag van 90% van die tijdens een 5 km tijdrit en op 88% van de HFM. Daarmee is een goede inschatting te maken van de MLLS door een 5 km tijdrit maximaal te rijden.

Na afloop van een intensieve training of wedstrijd doen de benen soms dagen lang zeer: DOMS (delayed inset muscle soreness) In een onderzoek was de maximale belastbaarheid tijdens een test, daar niet afhankelijk van; wel leidde het eerder tot hogere laktaatwaarden.

Krachttraining in een onderzoek bij 21 wielrensters, leidde wel tot een toename in spierkracht, maar er was geen enkel fysiologisch, noch ergometrisch verschil met een controle-groep bij een 1 uur durende maximaal-test op de fiets.

Warming-up, voorafgaande aan een uithoudings test (120% van maximaal vermogen) op de fiets, had metabole effect, w.o. lagere laktaat-waarden. Echter het resultaat (volhou-tijd) werd niet beïnvloedt door voorafgaande warming-up.

Professionele wielrenners hebben een maximaal vermogen dat varieert tussen 370 en 570 Watt, en een maximale zuurstofopname van 4.4 to 6.4 L/min; hun vermogen op het omslagpunt (OBLA, LT, of MLLS genoemd) varieert van 300 tot 500 Watt. Bij een etappe-wedtrijd zitten renners vaak 30 tot 100 minuten per dag boven hun omslagpunt te rijden.

Toen Indurain zich voorbereidde op het werelduurrecord, wer er veel onderzoek met hem gedaan. Hij was toen 30 jaar, bij een lengte van 188 cm en woog 81 kg.

Zijn Wmax was 572 Watt, en op het omslagpunt 505 Watt, waarbij hij 52.88 km/h reed; daarbij een gemiddelde hartslag van 183/min. De VO2 was daarbij 5,65 l/min. Hij verbeterde het record tot 53,040 km, wat –geschat – gemideld 509.5 Watt vereiste.

De bloeddrukregulatie vindt plaats in de vaten, hart, nieren, bijnieren en hersenen. Het neurohumerale systeem speelt daarbij een centrale rol, via een interactief communicatie-netwerk, waardoor ook lokale effecten (o.a. groeihormoon) tot uitdrukking komen. Het RAAS komt in vele organen voor. Bij de acute stimulatie (circulerend RAAS) leidt dit dit tot vaso-constrictie in de coronair vaten, tot een positief chronotroop effect en tot bijnierschors (aldosteron-sceretie) stimulering, waardoor zout en vocht retentie optreedt. Bij de chronische stimulatie (lokale RAAS) leidt dit tot verdikking van de vaatwanden of een plaque-ruptuur, tot glomerulosclerose en in het hart tot hypertrofie en fobrose, verstoring van de diastolisch funktie en ritme-stoornissen. Vandaar dat ACE-remmers en AII-antagonisten naast bloeddrukdaling ook tot vermindering van de incidentie van hart-infarct en nier-insufficientie leidt. Net zoals het RAAS, heeft de sympaticus algemene (adrenaline via de bijnier) als lokale (noradrenaline) werkingen. Dit leidt tot een positief inotroop effect, bronchodilatatie en vasoconstrictie c.q. dilatatie. Sympaticus en RAAS grijpen op tal van manieren in elkaar in. Zo is voor een verhoogde renine productie, bv. bij een ACE-remmer of AII-antagonist (via het terugkoppelings mechanisme) sympaticus activiteit noodzakelijk; anderzijds daalt renine productie door een betablokker. Bij ACE-remmers of AII-antagonisten bij een decompensatio cordis daalt de sympaticus-activiteit. De combinatie van betablokker en ACEremmer, resp. AII-antagonist lijkt daarmee een ideale door het potentiërend effect. Een intensieve training leidt tot een stimulering van de sympaticus en een stijging van de plasma renine-activiteit en daardoor tot een verhoogd angiotensine-II. Na de training is de sympaticus juist onderdrukt en overheerst de parasympaticus ( bradycardie en toename R-R.variabiliteit). Er zijn aanwijzingen dat na training in rust het RAAS-systeem bij een hartinsufficiëntie minder geactiveerd is.

Een recente publicatie in het D.Z.fur Sportmedizin laat zien dat bij sporters vaak een magnesiumtekort aanwezig is; dat is de algemene praktijk moeilijk te meten (loading dose; urine-excretie et.) en leidt tot klachten zoals spierkrampen. http://www.zeitschrift-sportmedizin.de/zeitschr.htm Tenzij je een goede bananen-eter (magnesium!!) bent, en nooit spierkrampen (na afloop), zou je eens kunnen starten met 100 mg elementair magnesium per dag.

Tijdens krachttraining in series, moet de pauze tussen de series langer worden om tot een optimaal effect te leiden. Tijdens series met zware belastingen, is een reductie van de last of het aantal herhalingen nodig om een eoptimaal effect te krijgen.

Formoterol – op de dopinglijst – had toegediend voorafgaande aan een Wingate-test en voorafgaande aan een maximaal-test geen ergogeen effect op deze waarden. Het staat dus ten onrechte op de doping-lijst.

Wat is het nut van speciale sportkleding, die het zweten zou bevorderen, zoals door merken als Coolmax geclaimed? Daartoe werden een aantal wielrenners getest die met modern materiaal of met katoen gekleed werden. Er bleek geen enkel effect in huid, lichaamstemperatuur en ook geen effect op zuurstofverbruik en hartfrekwentie aanwezig. Kortom, het nut van dergelijke kleding is bepaald nog niet aangetoond. Bij inspannings gerelateerde hoofdpijn, niet samenhangend met te duiden oorzaken zoals bv. de helmrand die in de nek drukt, is aanvullende diagnostiek zoals een CT-scan noodzakelijk. Anecdotisch is de casus van een patiente met HST die van orale op pleister-oestrogenen therapie overstapte en altijd hoofpijn tijdens sporten kreeg: door de toegenomen huiddoorbloeding nam de resorptie ook toe, met hoofdpijn als gevolg. In Nederland is neuropsycholoog Matser, de eerste geweest die gewezen heeft op de risico's van hersenbeschadiging door sport (boksen, voetbal). Helaas lopen ook wielrenners nogal eens een hersenschudding op. Bijna de helft van alle baanrenners, en meer dan 10% van alle wegrenners

liep al eens een hersenschudding op. Bij een val op een mountainbike komt men in 40% van de gevallen op zijn hoofd terecht. Hersenschudding wordt volgens de AAN (american academy of neurology) ingedeeld in 3 vormen: 1. Niet bewusteloos gweest; symptomen (duizelingen e.d.) korter dan 15 minuten 2. Idem, maar symptomen meer dan 15 minuten 3. Wel bewusteloos. Een sporter met type 1, mag als er er geen symptomen meer zijn, weer verder sporten. Bij de andere 2 types, niet en bij type 3 is melding op een eerste hulp van een ziekenhuis nodig. Ook moet er na type 2 en 3, 1 tot 2 weken niet gesport worden. Effect KH-drank op RPE (Borg-schaal). Ook het cortisol/glucose gehalte ligt gunstiger RPE ligt lager,alleen bij fietsers niet tijdens lopen

Precooling heeft gunstig effect op kortdurende prestatie ( 70sec) Power output: liep op van 580 naar 605 Watt.

Van Inosine wordt beweerd dat het ergogeen werkt via toename 2,3-DPG verhoging; in een studie naar het effect hiervan, kon dit niet bevestigd worden.

Een Sportdrank bevordert iha (duur)prestatie; reflux tijdens sporten neemt bij drinken van sportdrank echter toe m.n. bij hardlopen.

Efect sport en activiteit: Effect bij vrouw groter; bij intensive sportbeoefening RR van 0.5 (cardiovasculaire morbiditeit).

Strain (fysische krachten) wordt op de botten uitgeoefend tijdens belastingen zoals lopen en fietsen. Getest werde druk, compressie en shearing (scheren) mbv elektrodes in het bot. Grootste strain werd uitgeoefend tijdens hardlopen, maar tijdens fietsen is er nauweijks strain in het onderbeen. Om osteoporosis te voorkomen is hardlopen veel gunstiger dan fietsen.

Power output was lower in front suspension bikes compared to dual suspension on paved and gravel course. Bij screening: ECG-afwijkingen bij sporters; 60% is niet afwijkend; 5% heeft belangrijke structurele cardiovasculaire afwijkingen. Hoe groter het hart, hoe vaker afwijkend het ECG; ook bij mannen, jongeren en duursport vaker afwijkend ECG.

Placebo effect van koolhydraten: Er was een 4.3% snellere tijd in een tijdrit, indien KH drank en dit wel tevoren verteld werd, t.o.v. Personen met een KH-drank, maar dit niet wisten (kon ook placebo ziijn. Kortom: ook placebo's werken !! (neuspleisters...)

Met pacemaker mag geen contact sport gedaan worden.. Met ICD [intraventrivulaire defibrillator] geen intensieve sport en pas als een half jaar geen ritme stoornis. Bij krachttrainingen geven kortdurende belastingen een geringere bloeddrukstijging dan langer durende belastingen. Probeer daarom bij legg-press of leg-extension bij voorkeur slechts enkele seconden de contractie vol te houden. Na anabolica-gebruik, herstellen eventuele biochemische veranderingen (leverfunkties, lipiden) snel; veranderingen aan het hart (concentrische hypertrofie) kunnen echter veel langer blijven bestaan. Bij duur-atleten ontstaan niet alleen veranderingen aan het hart (grootte /dikte wanden) , maar ook aan de perifere vaten, in de vorm van een hypertrofie van bv. de wanden van de a.femoralis communis. De variabiliteit (test) in de VO2max bedraagt wel 5%! Kleine veranderingen in de conditie van een individu zijn dus niet betrouwbaar met de VO2max te meten. Bovendien kan het prestatie vermogen toenemen zonder verandering in de VO2 max. Volgens de ACSM bedraagt de formule: VO2 (ml/min) = (7 maal lichaamsgewicht) + (10,8 maal Wmax). De WHO beschouwt RR 130-140/85-90 als hoog-normaal en 140/90 als grenswaarde, voor alle leeftijden. Het voorstel luidt om bij ergometrie een systolische RR vann 180 bij 100 Watt als grenswaarde te nemen, waarboven sprake is van hypertensie.

Als fietsen met hardlopen wordt vergeleken, dan ligt de anaërobe drempel bij hardlopen op een (veel) hogere waarde van de hartslag, vaak wel meer dan 10 hartslagen. De maximale lactaat waarden blijven tijdens hartlopen echter lager. Bij (mannelijke) fietsers wordt een verhoogd voorkomen van impotentie vermeld. Dit wordt verklaard door een hogere druk in het kruis. Aan de Universiteit van Keulen werd als (omgekeerd evenredige) maat voor de druk, de zuurstofdruk in de penis gemeten tijdens het rijden op een race fiets. Na enkele minuten op het race zadel daalt de druk al van 61 mm Hg naar 20 mm Hg. Als de fietser ging staan, herstelde de druk zich meteen. Als de fietser na een langer durende test-rit van de fiets ging, herstelde de druk na een tiental minuten. Polymorfisme in de de genen, die voor , NOS3, EPOR en ADRA2 coderen, blijken te determineren voor prestaties in duursport. De correlatie van het ACE gen is echter zwak. Polymorfisme betekent dat een bepaalde volgorde in het DNA molecule bij meer dan 1% van de bevolking voorkomt. Langdurige supplementatie met Creatine leidt tot een geringe zwelling van cellen; dit remt de eiwit-afbraak en de leucine-oxydatie. Dit zijn dus gunstige, anabole effecten. Zinkprotoporfyrine is een zeer geschikte parameter in het bloed om ijzergebrek bij duursporters aan te tonen. Bij chronische enkel-klachten worden fracturen van de processus lateralis tali (“snowboard injury”) vaak miskend. Alleen met CT-scan zijn deze goed aantoonbaar. De opvatting dat bij intensieve inspanning “melkzuur” wordt aangemaakt, dat bijdraagt aan de “verzuring”, is biochemisch volstrekt onjuist. Bij de afbraak van glucose komen 2 pyruvaat moleculen vrij; vervolgens worden 2 pyruvaat moleculen omgezet in 2 moluculen lactaat. De “verzuring” treedt wel op, maar wordt veroorzaakt door de afbraak van de energie-leverancier ATP; daarbij ontstaan ADP en waterstofionen. ATP wordt weer geresynthetiseerd door de afbraak van vetten en glucose. Pas als tijdens intensieve belastingen de afbraak van ATP, de resynthese van ATP overheerst , treedt er “verzuring” op. Lactaat moleculen kunnen zelfs voor de energiestofwisseling gebruikt worden, zoals we ook uit de cardiologie kennen (bij coronair ischemie of een infarct); deze lactaat-stofwisseling kan daardoor verzuring juist tegengaan! A Star is born and made……. Erfelijkheid speelt voor de helft mee bij het prestatievermogen van de sporter. Natuurlijk spelen ook andere factoren zoals maatschappelijke ondersteuning (ouders, relatie, werk) en eigen mentaliteit een rol, naast het traiinings programma. Door onderzoek van de gebroeders Brouchard , o.a. met tweelingen, is duidelijk geworden dat ook de respons op training afhankelijk is van erfelijke factoren. Onderzoekers richten zich nu vooral op het ontdekken van specifieke DNAvolgorde in de chromosomen en met name het DNA dat aanwezig is in de mitochondria. Trouwen 2 topsporters (Andrew Agassi en Steffie Graf), dan is succes verzekerd….. Hoge doses vitamine C (1 tot 3 gram dd) is in staat spierpijn t.g.v. intensieve ongebruikelijke spierbelastingen (bv. bergop) te reduceren. De natuurlijke variatie in een tijdrit van 60 km bij wielrenners is zeer klein (1.3%). Dit betekent een variatie in het te meten maximaal vermogen van hooguit 2.5% . Aangezien de meetfout bij ergometrie al 5% bedraagt, zijn dit soort metingen niet in staat bij een gezonde renner “de actuele conditie” te meten, wel om bij evidente conditie-achteruitgang dit middels ergometrie te bevestigen. Lumbago komt vaak voor bij sporters en in de algemene praktijk. Het recidiveert regelmatig en heeft i.t.t. tot de vaak gepubliceerde opvatting, bij velen een hardnekkig beloop. Rugpijn is in de eerste lijn een goede tweede na banale verkoudheid. Tachtig tot negentig procent van alle personen heeft wel eens een periode met rugklachten en in de helft van deze gevallen komt de rugpijn terug in de daaropvolgende 3 jaren. Decennia lang waren bedrust, gebruik van sederende pijnstilling en operatief ingrijpen de peilers van de behandeling. De huidige aanpak is echter gebaseerd op deconditionering van het pijnpatroon, activering en beweging. Dat zou een gunstig effect hebben op de paralumbale spieren. In een studie met 186 patiënten met acute, nietspecifieke rugpijn, herstelden de personen die 2 dagen bedrust moesten houden minder snel dan de controle groep die zo veel mogelijk de dagelijkse bezigheden moesten continueren. Uit vroeger onderzoek bleek dat 90% binnen 6 tot 12 weken van zijn rugklachten verlost is. Recenter

onderzoek toont dat dat een te gunstige voorstelling van zaken is. Met name het recidiverende karakter en het niet volledig pijnvrij zijn na maanden, is opvallend. In de herstel-fase zijn stabiliserende oefeningen nodig, eventueel via een fysiotherapeut, met correctie van spierspanning, verder versterking van rug- en buikspieren en soms de rest van de bewegingsketen, maar vaak moet ook het foutief gebruik van de rug gecorrigeerd worden. Als de acute rugpijn verdwenen is, kan de rugfunktie nog gestoord zijn, de basis voor een recidief. Als de rugfunktie echter hersteld is en geen klachten meer optreden tijdens sportspecifieke bewegingen, kan weer aan wedstrijden worden deelgenomen. De complicaties bij een maximaal test bedragen per 10.000 tests: 1 hartinfarct, 2 maal ventrikelfibrilleren of andere ernstige ritme-stoornissen, 0.3 doden en 3 maal hospitalisatie; In totaal 6 belangrijke complicaties per 10.000 tests. Bij een maximaal test worden daarom een defibrillator en arts aanwezig geacht. Acute hartdood en hartinfarct zijn ernstige complicaties tijdens sportbeoefening. Artsen zijn (nog?) niet in staat het risico door screening tot nul te reduceren. Naast vals-negatieve uitslagen, treden er ook vals-positieve uitslagen op (10-25%). Bestrijden en behandelen van coronaire risico factoren en voorlichting over prodromale verschijnselen, zijn de belangrijkste doelstellingen. Inspannings ergometrie is onvoldoende in staat elke acute hartdood te voorspellen. Het grote merendeel (> 90%) van alle gevallen van acute hartdood wordt veroorzaakt door coronaria lijden. Door intensieve inspanningen kunnen fissuren in de coronair vaten of in een bestaande plaque optreden. Op een geruptureerde plaque kan tijdens sporten trombus-vorming optreden; hoewel na het sporten aggregratie remming optreedt, is er tijdens de inspanning namelijk een toename van het aantal trombocyten. In een oud litteken van een infarct kan tijden sporten een fatale ritmestoornis optreden. Bij personen tussen 30 en 65 jaar, onbekend met coronaria lijden, wordt het risico op acute hartdood geschat, op 1 per 18.000 personen per jaar. Bij langerdurende inspanningen zoals marathons lijkt het risico ook wat groter. Het risico is ook vergroot bij rokers, diabeten, obesen, en minder getrainden, maar ook op latere leeftijd. Akupunkuur: zinvolle aanvulling in revalidatie na voorste-kruisband operatie: In de revalidatie na een VKB-operatie, staat het herstellen van de quadriceps-musculatuur centraal. Bij 12 patiënten werd het effect van acupunctuur op 2 pijn punten getest; er volgde een significante stijging van de maximaalkracht met 18%. Deze veranderingen werden niet gezien bij 8 controle die “nep-acupunctuur” kregen toegediend. Acupunctuur bevordert de neuromusculaire prikkelbaarheid en draagt daarmee bij aan een kortere herstelfase. Ruptuur van de biceps-origo aan tuber ischiadicum: Niet vaak voorkomend, en ook als gevolg van een banaal trauma, leidt dit trauma tot grote gevolgen. Een fors hematoom, maar vooral een sterk verminderd kracht in het bovenbeen en een onzeker gevoel. De m.biceps werkt namelijk als een co-factor (agonist) voor de vastus lateralis en is belangrijk voor een goed coördinatie in het been. Behandeling is zo nodig operatief (pees wordt verankerd), afhankelijk van het sport-niveau en de belasting. Ooit een hartinfarct gehad en nu weer op de ski-piste? Dan is de kans op een plotse hartdood in de sneeuw met 90% verhoogd. Kreatine-inname leidt zoals bekend tot gewichtstoename door vocht retentie; maar ook tot stijging van vrije vetzuren in het bloed, alsook het groeihormoon. Spierpijn, in de vorm van drukpijn en pijn bij bewegingspijn, berust op sensibilisering van de noziceptoren in de spieren, door in de spier aangemaakte stoffen zoals Bradykinine, Serotonine en Prostaglandine. Laktaat speelt daarin geen rol! Desondanks hebben cyclo-oxygenase remmers (NSAID’s) weinig effect hierop. In deze geprikkelde nociceptoren, ontstaat in de daarop volgende (rust)fase, een sterke overgevoeligheid van de betrokken spieren. Als de spierklachten langer bestaan, leidt dit via de nociceptoren tot een sterke prikkelbaarheid van de de sensibele ruggemerg-neuronen. Dit geeft nog meer pijn en leidt tot een uitbreiding van het betreffende spiergebied. Vaak is er echter ook nog reffered pain, waardoor de primaire lokalisatie verkeerd ingeschat kan worden. De sensibele zenuwbanen staan onder invloed van remmende factoren, die vrijkomen tijdens duursport. Dit verklaart het mechanisme dat sporters tijdens het sporten een lesie krijgen, die pas na het sporten duidelijk wordt, zoals b.v. een vermoeidheidsfractuur.

Het beleid bij enkelband letsels is de laatste 20 jaar nogal eens aan verandering onderhevig geweest. Aktief funktionele behandeling is de laatste jaren meer benadrukt dan passieve stabilisering. Het is opvallend dat het subjectieve gevoel van instabiliteit een goede diagnostische betekenis heeft. Neuromusculaire revalidatie wordt bevorderd door orthesen van het spronggewricht en door proprioceptieve oefentherapie. Operatieve behandeling komt pas in aanmerking bij “ second stage” distorsie en bij chronische instabiliteit. Vooral door orthesen heeft de enkel meer tijd bij een inversie-trauma met een adekwate respons te reageren. Bij een vers trauma, moet de behandeling in principe funktioneel (oefeningen) zijn. Indien geen duidelijke, ernstige instabiliteit aantoonbaar is, kan daarnaast worden volstaan met 2 weken orthese of tape behandeling. Bij herhaling, gaat het om een re-ruptuur of om een “second stage”ruptuur. Als de patiënt anamnestisch aangeeft dat er stabiliteit aanwezig was, gaat het zeer waarschijnlijk om een re-ruptuur, te behandelen als een eerste distorsie. Bij chronische kapselband insufficiëntie, is er voortdurend een onzeker gevoel. Van belang is dan ter preventie een funktioneel preventieve orthese of een tape-verband te gebruiken. Als dit onvoldoende effect heeft, is een operatieve stabilisatie te overwegen. Verkorting van de hamstrings wordt vaak getest in rugligging, door het been heffen; die mogelijkheid is dan een maat voor de rekbaarheid. Door de zwaartekracht, verandert echter de vochthoeveelheid in het been en dit heeft een zodanige invloed, dat de rekbaarheid te hoog wordt ingeschat. Men voorkomt dit door te testen in zij-ligging. Imigran geeft goede resultaten bij mannen met hoofdpijn door hoogteziekte (A-022). Golfen is een leuke sport voor ouderen. De belasting voor het lichaam is gering en bij 2 spelletjes per week haal je net 2000 kcal. De hartfrekwentie blijft daarbij ver onder je omslagpunt. Goed ter ontspanning of als revalidatie na een hartinfarkt (A-106), maar onvoldoende om een gezonde fysieke prikkels te geven (B-099). Uitslag van Hematocriet is afhankelijk van jaargetijde. Keul a.o. vonden onder bijna 100 sporters, in nov-febr. bij bijna 15% van de onderzochten een hematocriet van boven de 50. Tussen maart en oktober betrof het 10% van de onderzochten. Schmidt onderzocht 14 renners. Ook hij vond een verloop in het seizoen. Echter geen enkele renner had een waarde boven de 50 en gemiddeld was de waarde 45% . In het topvoetbal is de fysieke belasting groot. Vooral middenveldspelers moeten een goed uithoudingsvermogen bezitten. In een onderzoek bleek dat in de afgelopen 10 jaar er geen veranderingen opgetreden zijn in te meten parameters zoals (start)snelheid, hartfunktie en vetpercentage (A-111). Vaak wordt gesuggereerd dat alleen Krachttraining leidt tot een verdikte hartspier met een verkleining van het hartvolume, de concentrische linker ventrikel hypertrofie. Bij echocardiografisch onderzoek van verschillende soorten sporters kon deze voorkeur voor krachtsporters niet bevestigd worden. Ook bij duursporters komt de concentrische vorm voor. Wel bleek bij krachtsporters die anabolica gebruiken de wand/volume index met 20% toe te nemen. Een kwartiertje na een inspanning is de bloeddruk meestal lager dan vooraf. Na anaërobe inspanningen is dat gemiddeld systolisch 12 mm, en diastolisch 6 mm. Na äerobe inspanning, ligt dat gemiddeld op 9, respectievelijk 3 mm Hg (C-016). De dood op het sportveld is dramatisch. Een Duitse analyse van niet-traumatische aandoeningen van overlijden tijdens of meteen na het sporten, verzameld n.a.v. obductie rapporten gedurende 20 jaar, laat zien dat het bijna allemaal mannen zijn (95%), gemiddeld 55 jaar oud. Het hartinfarct en coronairsclerose vormen 78%, verder overlijdt 4% t.g.v. acute forward en 2% t.g.v. acute myocarditis. Van alle gevallen betreft het in 20% voetbal (gemiddeld 47 jaar oud) , 16% tijdens zwemmen (gemiddeld 61 jaar oud), 10% tijdens fietsen (gemiddeld 65 jaar), 10% tijdens joggen (gemiddels 55 jaar), 7% tijdens tennis (gemiddeld 51 jaar), evenals kegelen 7% (gemiddeld 58 jaar). Vergelijking met overlijden tijdens sex laat zien dat hier de helft minder doden bij vallen en dat de gemiddelde leeftijd hierbij 61 jaar is (C-020; C-48). Kortom…….. Het Brugada-syndroom bestaat uit syncope, c.q. acute hartdood, waarbij in het ECG een RBTB en persisterende ST-elevaties in V1-3 bij normale QT-tijd aanwezig zijn en waarbij het sporten een trigger tot een calamiteit vormt (C-025).

Beta-blokkers hebben i.h.a. als eigenschap de prestatiecapaciteit van een duurprestatie ongunstig te beïnvloeden. Met Nebivolol (Nebilet) blijkt de VO2max echter onveranderd (C-040). Sportblessures komen tegenwoordig bij kinderen meer voor dan vroeger. Enerzijds hangt dat samen met de tijdgeest meer risico’s te nemen en het het karakter van trendsports zoals skaten. Ook is de coördinatie van kinderen tegenwoordig slechter, omdat ze minder bewegen; ze worden door de ouders overal heen gebracht met de auto, maar ook zijn de bewegingsmogelijkheden zeker in de grote steden - veel minder. En dan speelt bij een selecte groep ook nog mee dat de trainingsintensiteit en het aantal wedstrijden toe is genomen, waardoor eerder overbelasting blessures zoals epiphysiolysis, rugafwijkingen en aseptische necrosen optreden. Blessure preventie bestaat daarom uit dragen van beschermingsmiddelen (helm, knie en elleboogbeschermers), training afstemmen op de beperkte belastbaarheid van kinderen en bevorderen van natuurlijke bewegingen in spelvorm (D-001). Op een conferentie in 1997 in Hong Kong over dit onderwerp werden de volgende aanbevelingen gedaan. Zolang kinderen nog groeien moeten ze niet meer dan 16-20 uur per week sporten. Onder de 10 jaar is het aan te raden niet eenzijdig te sporten, maar verschillende sporten te bedrijven. Boxen is aan kinderen te ontraden. Onder de 18 jaar moeten kinderen niet langer dan 10 km hardlopen. Kinderen, ouder dan 10 jaar, die op topsport gericht zijn, moeten met behulp van een coach en arts begeleid worden (D-011). De “schokdempende” hardloopschoen, b.v. door een tussenzool, is al weer helemaal uit. Hij bleek juist eerder blessures te kunnen veroorzaken! De eerste stoot bij bodemcontact moet juist krachtig zijn, voor optimale voetafwikkeling (D-045). Blessures bij hardlopers komen vaak voor, wel 30% zou regelmatig klachten hebben. Vooral achillespeesblessures (20% van de mannen), en fasciitis plantaris (10% bij vrouwen) komen veel voor. Knieklachten komt voor bij 12%. Gemiddeld duren blessures 5 maanden en bijna 60% wordt behandeld met fysiotherapie, 20% krijgt inlays (D-054). Mer een nieuw apparaat, de Swiss DolorClast, gebaseerd op schokgolf-therapie, worden zeer goede resultaten vermeld bij blessures, zoals van de achillespees, apexitis patellae en adductorentendinopathie (D-066). Inline-skating is geen ongevaarlijke sport, zoals ze inmiddels ook in Amsterdam (tunneldrama…)weten. Voorals de distale radius-fractuur komt vaak voor. Maar ook fracturen aan middenhandsbeentjes en enkeldistorsies. Goede bescherming voorkomt de ergste schaafwonden (D-073). Spondylolysis, c.q spondylolisthesis komt vooral bij speerwerpers voor. Twintig jaar na hun carriere werden 20 speerwerpers onderzocht. Tien hadden een spondylolistesis, drie hadden een spondylolyse (D-092). Hartrevalidatie geschiedt vaak met de vuistregel “intensiteit conform HF = 180 min leeftijd”. Bij personen ouder dan 70 jaar, blijkt dit in de praktijk goed te funktioneren, bij jongeren is de HF echter vaak lager (A-132). Na intensieve, anaërobe belastingen zijn er onder invloed van Catecholaminen veranderingen in de leucocyten. In de eerste fase, het eerste kwartier, is er een toename van NK-cellen, CD8-cellen en monocyten. Daarna volgt de vertraagde reactie, met een maximum na ongeveer 12 uur, met een toename van neutrofiele leuco’s, monocyten, CD4-cellen en B-lymfocyten (B-005) Overall genomen blijkt dan bij regelmatige duurtrainingen de ratio CD4/CD8, die omgekeerd correleert met weerstand tegen infecties, toe te nemen, ook bij ouderen (B-010). Na intensieve, interval belastingen is er een acute fase-reactie met stijging van Interleukine-6 en CRP (B-011). Bij vrouwen die intensief (duur)sporten, is osteoporose of zelfs een stress-fractuur een reëel gevaar. Klassiek bij deze vrouwen is de trias Osteoporose, Cyclusstoornis (amenorrhoe) en eetstoornis (anorexia nervosa). Mechanismen daarbij zijn een insufficiënte voeding en een verstoring van de hypothalame-gonadale as (B-012). Na korte intensieve belastingen, stijgt het leptine-gehalte, wat meespeelt bij de verminderde eetlust en mogelijk een factor is bij vermagering bij regelmatig sporten (B-018). Leptine speelt mogelijk een belangrijke rol bij diabeten met overgewicht (B-104).

Sporten is een stressor. Daarbij wordt HSP70, een heatshock-eiwit aangemaakt als beschermend en aanpasend eiwit voor de gestresde cellen (B-060). Sporten tijdens warm weer geeft exta stress, de “hitte-stress”. Daarbij is er een sterkere stijging van leuco’s, een sterkere stijging van hartfrekwentie, en van laktaat-gehalte. Inspanningen kosten meer zuurstof (VO2-verbruik) en er is een sterkere stijging van de hormonen Cortisol en hgH (B-029). Voor overtraining is er geen bewijzende diagnostische test. Symptomen die op overtraining duiden zijn: meer dan gewoonlijke spierpijn na de inspanning (“zware benen”), moelijk slapen, meer zweten en een gevoel van zware inspanning tijdens lichte hersteltrainingen. (B-062). Vaccinatie (griepvaccin, hepatitis A etc.) bij sporters geeft significant minder bijwerkingen naar mate de gevaccineerden gewoonlijk gewend is, meer uren per week te sporten (B-009). Bij voorafgaande regelmatige duursport, komt de immuunreactie sneller en intensiever op gang (B032). Anderzijds wanneer tijdens de vaccinatie intensief doorgetrained wordt, is de immuunreactie verminderd (B-031). Vitamine C (1 gram) blijkt (significant) de stijging van Laktaat en Noradrenaline tijdens een ergometrie tegen te gaan (B-040). Intensiteit:% VO2max 40 Vetten 96 KH-aeroob KH-anaer.

Energie-productie afhankelijk van Intensiteit (B-053) Krachttraining met (sub)maximale belasting is zwaar. Daarbij worden vaak hoge laktaatwaarden van wel 14 mmol/l gezien. Ook kan de systolische bloeddruk stijgen tot 290, en kan bij jongeren een hartslag van 190 gehaald worden (B-075). Ijzertoediening bij latent tekort (laag ferritine ) geeft significante verbetering van de VO2max. Hartgens a.o. deden onderzoek naar het effect van inhalatie, in supratherapeutische dosis, van een Beta-sympaticomimeticum bij gezonde sporters. Toegediend voor de inspanning verbeterde niet alleen de longfunktie (FEV1), maar ook verbeterde een duurtest (volhoudtijd) op de ergometer met 2%. Hartgens a.o. Invloed van Salbutamol op de duurprestatie van sporters. Geneeskunde en Sport 32 (1999), 5, 9-15. N.D.’s of tewel nasal dilators, worden een aantal jaren al gebruikt door sporters met de hoop en de illusie er beter door te presteren. Uit veel onderzoek blijkt inmiddels dat er geen effect van te verwachten is behalve de psychologische kick. Het oppervlak van de neusgaten bedraagt slechts eentiende van dat van de keel en neusademhaling speelt tijdens sport een zeer bescheiden rol. Daarentegen bij paarden berust de ademhaling voornamelijk op neusademhaling. Pleisters? Iets voor paarden! Door alcoholgebruik daalt bij mannen de testosteronspiegel. Opvallend is echter dat bij vrouwen juist een stijging optreedt door (fors) alcoholgebruik, zelfs zodanig dat de Testosteron/Epitestosteron ratio stijgt, waardoor zij bij een dopingcontrole positief kan worden! Gebruik je vitamine-preparaten, ijzer en anti-oxydantia zoals Vitamine E of Selenium? Er zijn aanwijzingen dat de combinatie minder werkzaam is als alles tegelijk wordt ingenomen; neem daarom b.v. de vitamines en ijzer 's avonds en de anti-oxydantia 's ochtends. Horwitt, Do the metals in vitamin-mineral supplements produce undesirable oxidation? Am. J. Clin. Nutr. 65:1571, 1997 Het gebruik van Q10 is helemaal achterhaald. Er zijn zelfs aanwijzingen dat het aantal vrije radicalen en celschade toeneemt bij sterk intensieve belastingen, als je dit middel gebruikt. Malm et al. Supplementation with ubiquinone-10 causes damaging during intense exercise. Acta.Physiol.Scand. 1996; 157: 511-512. Recent is aangetoond dat in veel sporten, maar ook t.a.v. het onderwijs, de geboortemaand mede bepalend is voor de kans op succes. Geboren in de maanden voorafgaand aan de "peildatum"

betekent dan "ongelukkig geboren". Deze factor geldt b.v. bij ijshockey en beroepsvoetbal. Het is echter niet aantoonbaar bij hockey en tafeltennis. Het geboortemaand-effect wordt verklaart vanuit het feit dat personen, geboren in de maanden voorafgaand aan de peildatum, altijd een ontwikkelingsachterstand hebben t.o.v. hun oudere "klasgenoten". Sinds kort staan dextropropoxyfeen (Depronal <(R)>) en tramadol (Tramal <(R)>, Tramadol <(R)>, Theradol <(R)> en Tramagetic <(R)>) niet meer op de lijst van verboden producten. Bovendien is voor Insuline een verklaring van noodzakelijkheid door een specialist noodzakelijk. Ook nieuw is dat Selegeline (Eldepryl) nu op de lijst van verboden producten staat. De Diuretica (plaspillen) staan wel weer op de dopinglijst. In 1994 is in de VS de Dietary Supplement Health and Education Act aangenomen. Door deze wet kunnen producten zoalsa melatonin, DHEA, pregnenolone, progesterone, and androstenedione vrij gekocht in health food stores, in apotheken en supermarkten. Vooral DHEA is erg populair omdat het leidt tot een stijging van testosteron en daardoor de spierkracht kan toenemen. Uit onderzoek blijkt dat vooral de sportprestaties van vrouwen boven de 40 en mannen boven de 50 gunstig beïnvloedt kunnen worden; maar er zijn gezondheidsrisico's aan het gebruik ; zo kunnen er prostaatproblemen optreden en bij een dopingcontrole kan het testosteron gehalte te hoog blijken, alhoewel in de praktijk dit wel meevalt. Gebruik van dit soort middelen is echter Russische Roulette voor het endocriene systeem Bosy, et al., The effect of oral dehydroepiandrosterone (DHEA) on the urine testosterone/epitestosterone (T/E) ratio in human male volunteers. J. Anal. Toxicol. 22(6):455459, 1998. In de periode 1987-1994, de periode vooraf gaande aan EPO, werden er in Vlaanderen 4.500 renners getest. Bijna 8% was positief. Veel gebruikt werden toen de amfetamines, codeïne en de anabolen. Inspanningsastma komt veel voor bij astmatici; het hangt drinken tijdens het sporten.

nogal eens samen met onvoldoende

Veel wielrenners klagen na het fietsen van langere afstanden over gevoelloosheod van "het genitaal". Dat komt omdat de bloedtoevoer naar dat gebied wordt afgekneld, vooral bij een zit op harde kleinere zadels. Vier procent van de wielrenners is (hierdoor) impotent, in vergelijking met twee procent bij zwemmers. Bloedneuzen tijdens het fietsen? Ze ontstaan meestal door te droge slijmvliezen. Soms t.g.v. te weinig drinken onderweg. Als dat niet de oorzaak is, helpt het gebruik van zout-neusdruppels, voor de start. · Maagklachten komen vaak bij wielrenners voor. Maagzuur, misselijkheid, boeren en maagpijn zijn de klachten. Meestal wordt het veroorzaakt door een functiestoornis van de maag. Het plantaardig preparaat Iberogast geeft een goede bescherming daartegen. In een groot prospectief onderzoek, werd gekeken naar de voorspellende waarde van inspanningstesten. Tijdens sport kregen 62 personen een acuut hartvaat-incident, waarvan 54 maal een hartinfarct en 8 maal acute hartdood. Slechts bij 11 personen was tevoren het isnpanningsonderzoek afwijkend geweest. Desondanks adviseren Amerikaanse cardiologen dit onderzoek aan sporters, ouder dan 40 (mannen), respectievelijk 50 (vrouwen) of bij meer dan 1 cardiovasculaire risico factor. En dit tweejarlijks herhalen. Grenzen aan vermogen: Het prestatie vermogen van oudere (duur)sporters is vooral verminderd door verlaging van de maximale hartslag, waardoor de VO2max daalt. Ook de lagere creatine fosfaat spiegels, de verminderde dichtheid van de mitochondria (mogelijk te bestrijden met antioxydantia) en verlaging van de totale glycogeen voorraad in de spieren spelen een rol. Bergop: Staan of Zitten? Staand fietsen berg-op kost meer energie en gaat gepaard met een hogere hartslag, bij gelijk blijvende snelheid in vergelijking met zittend fietsen. Fiets daarom berg-op bij voorkeur zittend. Bij

steilere hellingen (> 10%) is er geen energetisch verschil meer tussen zitten of staan. Fietsers bkijken dan echter een beter gevoel in de benen te hebben als ze staan. Daarom heeft bij steilere hellingen staan de voorkeur. Cadance: Aan de universiteit van Wisconsin en Wyoming deed men het volgende onderzoek. Acht wielrenners moesten op 80% van hun VO2max een half uur fietsen. Ofwel met een een cadans van 50 /minuur en een grote plaat, of met een cadans van 100 en een klein verzet. De snelheid bleef daarbij gelijk. Er waren geen verschillen in zuurstofverbruik, hartfrekwentie of lactaat vorming. Echter bij de hogere cadans werd er relatief meer vet verbruikt. Bij de lagere cadans werd er veel meer glycogeen verbruikt, vooral in de FT-vezels. Daarom hebben hogere trapfrekwenties, tot een bepaald optimum, de voorkeur. Een infuus met dextraan is een methode (niet toegestaan volgens de doping regels) om het totale bloedvolume te verhogen. Het effect op het bloedvolume is gelijk aan het effect van meerdere intensieve trainingen, waarbij het totale bloedvolume met 10% toeneemt, mits er voor en tijdens de training gedronken wordt. Voorafgaan aan een belangrijke wedstrijd leidt tapering met korte, intensievere interval-training tot deze gunstige veranderingen. Bij sub-elite wielrenners in de VS werden VO2max waarden gemeten tussen 4.5 en 5 liter/min , per kg 63-70 ml. Bij dames gemiddeld 3.4 l/min, per kg 52 ml. Belangrijkste bepalende factoren bij een tijdrit op de baan, zijn de VO2max, vetgehalte en anäeroob vermogen. Tijdens de 4 km op de baan speelt het anäeroob vermogen minimaal voor 25% mee. Wie goed wil presteren, moet zich de dag tevoren niet over de kop rijden. Als het trainings volume wordt gereduceerd tot 30%, blijft 10 tot 28 dagen de VO2 max en de maximale snelheid gehandhaafd. Tapering met hoge-intensiteits trainingen, met volume reductie (tot de helft) leidt tot verbetering in het prestatie vermogen. Aan de universiteit van Freiburg, bij Prof.Keul, werd bij wielrenners onderzoek gedaan naar het effect van vitamine E als anti-oxydans. Als gevolg daarvan waren de CPK spiegels, als indicatie voor spierschade, lager dan bij niet-gebruikers. In een korte fiets-test van 30 minuten waren er geen verschillen in prestatie-vermogen tussen gebruikers en niet-gebruikers. Beter presteren? Adem dan tijdens de inspanningen met de mond open om maximale zuurstofhoeveelheden binnen te krijgen. Ook hyperventilatie oefeningen, ’s avonds een tiental minuten, heeft effect op maximaal prestatie-vermogen. Voorafgaande aan een Wingate-test moesten proefpersonen 15 minuten hyperventileren. De peakpower steeg daardoor van 12.3 naar 12.6 Watt/kg, de gemiddelde power steeg van 8.5 naar 8.9 Watt/kg. Ook het laktaat gehalte was na afloop hoger (1,0 mmol/l). Met een SRM-apparaat werden 8 professionele wielrenners (Telekom) gedurende een etappewedstrijd, 5 dagen lang vervolgd. Gemiddeld was de belasting 234 Watt; de gemiddelde hartfrekwentie was 147 en de snelheid 40,1 km/u. Percentage van de tijd die boven de anäerobe drempel werd gereden, bedroeg 21%. Met de Polar Own Zone, bv. op de Smart Edge, bereken je op basis van de hartslag variabiliteit het omslagpunt. Nadere analyse geeft aan dat minder getrainden hierdoor te hoog uitkomen, goed getrainden te laag uitkomen. Mountainbiken met hart-patiënten Gedurende 6 maanden gingen patiënten 1.5 uur per week mountain biken en 1 uur per week op een hometrainer. Na die 6 maanden steeg de Wmax van 150 naar 157 Watt. Cholesterol daalde van 204 naar 197; LDL daalde van 137 naar 130; TG daalden van 133 naar 129 en HDL steeg van 41 naar 43, alles niet significant. Bij de behandeling van overgewicht, komen in aanmerking: Vermindering energie-uptake, meer lichaamsbeweging, stress management en verandering van eetgedrag. Bij vervolgen van een groep adipeuzen, kwamen de volgende resultaten naar voren: Na 1 jaar was men gemiddeld 8% vermagerd en bijna 30% haalde het streefgewicht. Na 2 jaar zijn deze cijfers 7% en 16%. Na 3 jaarweegt men nog maar 3% minder dan de uitgangswaarde.

De zuurstofdruk in de penis vermindert tijdens fietsen op een race-zadel. Er werden 4 zadels uitgetest. Alle zadels leiden tot een daling van de druk. Smalle, gepolsterde zadels leiden tot de sterkste daling. Bij zadels met “body geometrie”, smalle gepolsterde zadels met een uitparing in het midden treedt iets minder druk-daling op. Nog minder drukdaling treedt op bij ongepolsterde brede zadels en het minste effect is er bij bredere gepolsterde zadels. Bredere zadels geven een bredere, iets betere steun aan de zitbeenderen waardoor er minder druk op de zenuwen en bloedvaten in het kruis komt. Hoewel tijdens het fietsen, de gemiddelde scrotum temperatuur stijgt (van 33.3 naar 33.6), bleken er bij onderzoek van 51 fietsers, die gemiddeld 500 km/wk fietsten, geen sperma veranderingen of afwijkingen voor te komen. Tijdens het wegseizoen treden gunstige immunologische veranderingen op bij fietsers. De fagocytose en de monocytaire activiteit stijgen namelijk. Tijdens de WK Triple-Triathlon 2000, werden de energetische data van 6 deelnmers geévalueerd. Gemiddeld was de totale duur 51 uren. Per uur werden er 293 kcal ingenomen en 378 ml gedronken. Gemiddels gewichtsverlies was 2.2 kg. De gemiddelde koolhydraat inname was 47 gram/uur. De ervaringen bevestigen nogmaals dat meer dan 8000 kcal per dag innemen, onvermijdelijk leidt tot ernstige gastro-intestinale klachten. Glycerol (dosis 1.5 g/kg in vloeistof 15 ml/kg) leidt tot hyperhydratie en daardoor tot toename van het bloedplasma-volume. Het nuttig effect werd bevestigd in een test, waarbij de uithoudingstijd met meer dan 10% toename bij een 10% lagere hartslag.
