Abstractboek Hartelijk welkom! Het Nederlands Netwerk voor Tabaksonderzoek (NNvT) is in 2013 opgericht om een platform te bieden aan het wetenschappelijk tabaksonderzoek in Nederland. Het is een netwerk van onderzoekers uit verschillende wetenschappelijk disciplines, van (pre-)klinisch onderzoek en farmacologie van nicotineverslaving tot epidemiologische studies en beleidsonderzoek. Het netwerk wordt mogelijk gemaakt door financiële ondersteuning van KWF Kankerbestrijding, de Nederlandse Hartstichting en het Longfonds. Jaarlijks wordt er een wetenschappelijk congres georganiseerd, waarvan dit het eerste is! Via de NNvT website kan je gericht zoeken naar individuele wetenschappers of collega wetenschappers die in Nederland actief zijn op het brede terrein van tabaksonderzoek ten behoeve van verbetering van de volksgezondheid. De aanvang van het congres is 9.30 u en de afsluiting is rond 18.00 u. In de ochtend spreken, onder leiding van dagvoorzitter Prof. dr. Onno van Schayck, hoogleraar preventieve geneeskunde: Prof. dr. De effectiviteit va n ta baksontmoediging hangt s amen met de omva ng en de kwaliteit va n het wetenschappelijk onderzoek op di t gebied. In deze presentatie wordt d eze Nederlandse wetenschappelijke output i n de a fgelopen 12 ja ar beschreven. De Nederlandse output wordt vergeleken met de rest va n Europa, zodat inzicht ontstaat i n de onderzoeksthema’s waar Nederland i n vergelijking met andere Europese l anden vóór- of achterloopt. De Nederlandse wetenschappelijke output is onderzocht voor de jaren 2000, 2004, 2008 en 2012 door mi ddel va n een i nhoudsanalyse va n de abstracts va n ti jdschriftpublicaties. Aa n de hand van tabaksgerelateerde zoektermen in het ti tel veld werden alle tijdschriftpublicaties die een Engelstalige a bstract bevatten geselecteerd uit PubMed, Web of Science, Ps ycInfo en Scopus. Na excludering va n a rtikelen die geen betrekking hadden op ta baksontmoediging (bijvoorbeeld bos branden, plantenonderzoek) en verwijdering van duplicaten bleven er 4,665 a rti kelen over voor codering. Belangrijke i ndelingen waren: epidemiologisch raamwerk (‘a gent, host, environment, vector’), soort product (‘nicotine, tobacco, s mokeless, electronic’’), studie focus (‘secondhand s moke, smoking initiation, smoking cessation, harm reduction’), ma a tregel (‘smoking ban, taxation, advertising, warnings, ca mpaigns’), en of de publicatie een va n de volgende onderwerpen betrof : gezondheidsschade, socio -economisch verschillen, farmacotherapie, fundamenteel onderzoek (‘basic s ci ence’). Het a a ntal tabaksgerelateerde publicaties i n de 31 Europese l anden nam toe va n 780 i n 2000 tot 1.499 i n 2012. In de pres entatie zal een gedetailleerde beschrijving worden gegeven va n i nhoudelijke veranderingen binnen het Nederlandse ta ba ksonderzoek, in vergelijking met de a ndere Europese landen. Op basis va n de resultaten va n deze bibliometrische s tudie zullen aanbevelingen worden gedaan voor een Nederlandse ta baksonderzoeksagenda .

Inleiding: Onda nks dat de s chadelijke gevolgen va n roken bekend zijn, rookt nog s teeds een aanzienlijk deel va n de Nederlandse bevolking. Wat zijn de oorzaken va n i ndividuele verschillen i n rookgedrag? Waarom beginnen sommige mens en wel met roken en anderen niet. En waarom kan de ene persoon gemakkelijk s toppen en begint de ander steeds opni euw met roken? Speelt erfelijke aanleg hier een rol bij, of wordt rookgedrag met name beïnvloed door de omgevi ng? Methode: Met behulp va n de gegevens va n tweelingen en hun familie l eden is onderzocht hoe groot de invloed va n erfelijke aanleg is op de individuele verschillen i n rookgedrag. In de afgelopen jaren is er ook veel onderzoek gedaan naar het i n kaart brengen va n de specifieke genen, onder a ndere door het doen va n genoom-wijde associatie s tudies. Discussie: Wa t i s er tot nu toe bekend met betrekking tot de rol va n erfelijke a anleg voor rookgedrag. Welke s tudies zijn er gedaan en hoe moeten we de resultaten interpreteren? Wa t zijn de volgende s tappen bij het i n kaart brengen va n de genetische aspecten van tabaksverslaving? En wat betekent dit voor mensen die willen stoppen met roken en voor hul pverleners?

De vooruitgang in kennis over terugval en s uccesvolle abstinentie bij s toppen met roken is i n de a fgelopen 20 ja ar ges tagneerd. Een oorzaak daar va n is dat er na het Relapse Prevention Model va n Marlatt nauwelijks nieuwe theoretische pers pectieven zijn ontwikkeld en getoetst. Basale vra gen als “hoe werkt de motivatie bij ex-rokers om abstinent te blijven” en “hoe kan het dat stoppen met roken steeds gemakkelijker wordt” worden door de bestaande modellen niet bevredigend bea ntwoord. Het beantwoorden va n deze vra gen kan de praktijk va n s toppen met roken echt vooruit helpen. De Leer Abs ti nentie Theorie (LAT) is ontwikkeld om meer inzicht te krijgen i n continue abstinentie en de processen die daar bij betrokken zijn. De kern va n de LAT l igt i n het leren va n ex-rokers over roken en niet-roken. Enerzijds leren exrokers concreet i n l eersituaties. Wat ze l eren i s afhankelijk va n de state of mind i n die situatie. Anderzijds leren ex-rokers op een a bstracter niveau over de voortgang naar hun einddoel va n gemakkelijke en continue a bstinentie: Op grond van verza melde informatie e creëren ze een beeld va n hun voortgang, dat bepalend is voor hun motivatie. De vaardigheden die ex-rokers gebruiken moeten dit leren ondersteunen: In de LAT s taat daarom het leren centraal, niet de abstinentie. Dit i mpliceert dat op grond va n de LAT rokers geen stoppoging doen maar s tarten met het afleren va n het roken en het a a nleren va n niet-roken. Zo heeft de LAT belangrijke consequenties voor het begeleiden va n stoppen met roken in de pra ktijk.

Tot welke ni euwe inzichten leiden zij en verdienen zij een plaats in de nieuwe ri chtlijn? Of niet. Oordeelt u zelf.

Economic evaluation of multiple web-based smoking cessation interventions

[email protected] Introduction Whi le evidence exists for their effectiveness, the current tobacco dependence treatment guideline pays l ittle a ttention to web-based interventions. As economic evaluations of these i nterventions are s till s carce, results will be discussed of two economic evaluations s tudies investigating the cost-effectiveness of web-based computer-tailored s moking cessation i nterventions. While up to now most of these i nterventions were text-based, we a lso investigated a n intervention delivered vi a vi deo, especially a imed to increase a ttention for these interventions among l ow SES s mokers. Methods Both economic evaluation s tudies were embedded in a ra ndomized controlled trial, in which 2099 a nd 414 Dutch a dult s mokers participated, respectively. Self-reported societal cost and health-related quality of life were assessed during a 12month follow-up period; behavior s pecific outcome measures were assessed during the 12-month follow-up measurement onl y. Both studies were conducted from a societal perspective. Uncertainty wa s accounted for by bootstrapping a nd s ensitivity a nalyses. Results Both cos t-effectiveness analyses, using s moking abstinence as outcome measure, were in favor of the web-based smoking ces sation interventions. However, cost-utility a nalyses, using quality of life as outcome measure, s howed results that were i n fa vor of the interventions received by the control groups. Discussion Whi le both web-based computer-tailored smoking cessation interventions s eemed to be most cost-effective, the cost-utility of the i nterventions received by the control groups was probably highest. These fi ndings will be discussed in l ight of potential challenges for the economic evaluation of lifestyle behavior change i nterventions, s uch as the i nterpretation of thes e findings without i nformation on s ociety’s willingness to pay per a dditional ex-smoker.

Inleiding Roken is een belangrijke ri sicofactor voor coronaire hartziekten. Problematisch i s dat meer dan 50% va n de hartpatiënten di e rookten voor zi ekenhuisopname, blijft roken of terugvalt. Omdat van laag i ntensieve s toppen-met-roken begeleidingsvormen de effectiviteit wordt betwijfeld, onderzoekt deze studie de (kosten -)effectiviteit va n i ntensieve begeleiding door s toppen-met-roken professionals gecombineerd met nicotinepleisters (NP). Afdelingsverpleegkundigen pa s ten hierbij de Vraag-Advies-Verwijsmethode (VAV) toe i n plaats va n zelf stopondersteuning te geven. Naast toetsing va n het overall effect werd nagegaan of effectivi teit verschilde in s ubgroepen qua s topmotivatie en opleidingsni veau. Methode Ca rdi ologiepatiënten die rookten werden ad random toegewezen aan telefonische coaching (TC, n=223), persoonlijke coa ching (PC, n=157) (beide gecombineerd met NP)of gebruikelijke stopondersteuning (UC, n=245). Pa tiënten werden geworven i n 8 verpleegafdelingen ca rdiologie. Resultaten De twa alf maanden i ntention-to-treat gedragsmeting laat zien dat TC en PC voora l effectief zijn bij patiënten met een laag opl eidingsniveau en een l age s topmotivatie (TC: OR 2.95, 95% CI 1.24-7.04, p=.02; FC: OR 5.05, 95% CI 2.01-12.69, p=.<001). De OR’s va n TC en PC vers childen niet significant va n elkaar. Voor hoog opgeleide gemotiveerde patiënten zijn de OR’s niet s i gnificant. TC bleek het meest kosteneffectief. Discussie Voora l bij lager opgeleide patiënten die twi jfelen over stoppen i s toepassen va n VAV gecombineerd met counseling (TC of PC) en NP zi nvol. Hoger opgeleide gemotiveerde patiënten stoppen va ker uit zichzelf; maar hoe ook hen te kunnen ondersteunen dient te worden onderzocht. De Richtlijn Ta baksverslaving en de Multidisciplinaire Richtlijn Hartrevalidatie worden deels onderschreven, maar laag gemotiveerde patiënten dienen zeker te worden doorverwezen naar stoppen-metroken gerelateerde counseling.

Can a NRT supported reduced smoking intervention remotivate therapy-resistant COPD smokers to stop smoking?

Introduction Smoki ng cessation is the most effective means of fa vorably modifying the course of COPD. However, ma ny s moking COPD pa ti ents have given up tryi ng a fter numerous failed attempts. A novel s trategy to re -motivate these therapy-resistant s mokers may be a reduced smoking i ntervention, a llowing participants to rebuild s elf-control towards quitting. Methods The REDUQ s tudy i s a multicenter RCT wi th 18 months follow-up, i ncluding 130 s moking COPD outpatients, who were not rea dy to quit. REDUQ patients received a n intensive reduction -to-quit program, combining counselling and NRT. Existing behavi oural change techniques as well as s pecific reduction techniques were a pplied in eight s mall-group sessions and four i ntermediate telephone contacts, delivered by pulmonary nurses. NRT was offered free of charge for 12 weeks, i n d osages i ni tially i ncreasing wi th progressive cigarette reduction, and then gra dually fa ding out. Control patients received a single i nformation meeting and a self-help manual with reduction strategies. As soon as patients in both groups expressed rea diness to quit, they were referred to an intensive smoking cessation program. Results Prel i minary a nalyses involving N=110 pa rticipants revealed that REDUQ intervention successfully i ncreased ≥50% s elf reported reduction a t 6 months (55.3% vs . 28.2%; p=.016), but failed to increase self -reported 7-days point prevalence a bs tinence a t both 6 months (16.7% vs . 12.5%; p=.54) a nd 12 months (14.8% vs . 12.5%; p=.72). Bi ochemically va lidated outcomes, i ncluding continuous a bstinence, a re presented a t the conference. Discussion Prel i minary outcomes suggest that a n i ntensive reduction-to-quit i ntervention for COPD s mokers unmotivated to quit, is i neffective in i ncreasing long-term abstinence rates, in spite of initially a chieving reduced s moking levels.

Stoppen met roken interventies voor rokers met een depressie: rijp voor de nieuwe richtlijn?

Inleiding In de huidige ri chtlijn voor de behandeling va n ta baksverslaving wordt op minimale wi jze aandacht besteed aan stoppenmet-roken interventies (SMRIs) voor s ubpopulaties. Een belangrijke groep is rokers met een depressie. Deze studie beoogt de effectiviteit te evalueren va n SMRIs voor rokers met een huidige depressie of depressieverleden. Methode Wi j voeren een meta a nalyse uit va n gerandomiseerde tri als volgens het Cochra ne protocol. Depressie was gedefinieerd als kl i nische depressie of depressieve s ymptomen. De uitkomstmaat was a bstinentie va n roken na tenminste 6 ma anden fol l ow-up en werd uitgedrukt als een relatieve kans (RR) op een positief behandelresultaat. De meta -analytisch gecombineerde RR’s werden met de Ma ntel-Haenszel methode geschat. Resultaten 49 tri a ls werden geïncludeerd. Meta-analyses lieten een positief effect zien voor het toevoegen va n een psychosociale ‘mood ma nagement’ component aan een standaard SMRI voor rokers met een huidige depressie (11 s tudies RR 1.47, 95%CI 1.13-1.92) of met een depressieverleden (13 s tudies RR 1.41, 95%CI 1.13-1.77) i n vergelijking met een s tandaard SMRI a l leen. Ook werd een voorzichtig te interpreteren positief effect gevonden voor het farmacologische middel bupropion bij rokers met een depressieverleden (4 studies RR 2.04, 95%CI 1.31-3.18) i n vergelijking met placebo. Er waren te weinig s tudies voor het evalueren va n de effectiviteit va n de a ndere antidepressiva en NRT. Discussie Het toevoegen va n een psychosociale ‘mood management’ component a an SMRIs vergroot de kans op abstinentie voor rokers met een depressie.

Impact of smoke-free legislation on perinatal and child health: systematic review and meta-analysis

Background Smoke-free l egislation has the potential to reduce the substantive disease burden associated with s econd -hand smoke expos ure, particularly in children. We investigated the impact of smoke-free l egislation on perinatal a nd child health. Methods We s earched 14 online databases (1975-2013), screened the World Health Organization (WHO) International Clinical Trials Regi stry Pl atform a nd relevant citations a nd references, and contacted an i nternational expert panel to i dentify a dditional s tudies. We i ncluded studies undertaken using the Cochrane Effective Pra ctice and Organisation of Ca re approved designs tha t reported associations between smoking bans i n public places a nd one or more predefined perinatal and paediatric hea lth i ndicators. Pri mary outcomes were: preterm birth, low birth weight, a nd hospital a ttendances for asthma. Effect es timates were pooled using ra ndom-effects meta-analysis. Findings We i dentified 11 eligible studies, i nvolvi ng >2,541,389 bi rths and 247,168 a sthma exacerbations. All employed interrupted ti me s eries designs. Five North-American s tudies described l ocal bans a nd s ix European studies described national bans. Ri s k of bias was high for one, moderate for six, a nd l ow for four studies. Smoke-free l egislation was a ssociated with reducti ons in preterm birth (4 s tudies, 1,366,862 s ubjects: -10.4% (95%CI -18.8;-2.0, p=0.016)) a nd a sthma hos pitalisations/emergency department vi sits (3 s tudies, 225,753 events: -10.1% (95%CI -15.2;-5.0), p<0.001). No s ignificant effect on low birth weight was i dentified (6 s tudies, >1,952,397 s ubjects: -1.7% (95%CI -5.1;1.6), p=0.31).

Interpretation Smoke-free l egislation is associated with s ubstantial reductions i n preterm births and hospital attendance for asthma exa cerbations. Together with the health benefits in adults, this work provides strong support for WHO recommendations to crea te smoke-free public places.

Young adult smokers’ behaviours and reactions on peer discouragement

Introduction Thi s study i nvestigates the impact of antismoking peer pressure on the smoking behavior and reactions of daily-smoking young a dults. Method An experiment (consisting of four conditions) was conducted among 59 daily-smoking young adults a ged 16–24 yea rs. Duri ng the session, the confederate a nd participant sat i n a camper va n a nd had to do a 30-min joint music task. The pa rti cipants’ smoking behavi or a nd reaction were observed. Result The fi ndings s howed that young adults s moked fewer ci garettes i n the presence of a nonsmoking model pressuring the young a dult not to s moke compared to a heavy-smoking model not using a ny pressure. At the s ame ti me, the results i ndicated that the total number of cigarettes smoked did not differ s ignificantly for nonsmoking peers verbally pressuring the young a dult not to s moke compared to nonsmoking peers not verbally pressuring the young adult. Furthermore, we focused on the two conditions regarding antismoking peer pressure: the 44 responses in the a ctive a nd 34 res ponses i n the passive condition were analyzed i n a discourse analyses. The participants i n the passive condition were more elaborative about s moking while i n the active condition participants s how more passive resistance. Open resistance does not significantly differ i n these two conditions but occurs, for both, primarily a t the third discouragement. Conclusion Our fi ndings indicate that the protective effect of peer influence merely l ies i n that the peer does not s moke. However, pa ssive discouragement s eems to be more effective in stimulating young adults to ta ke a n a ctive role in the dialogue with thei r peers a bout smoking.

A randomized controlled trial testing the efficacy of nicotine replacement therapy among adolescents: the role of the therapy compliance

Previ ous research among a dolescents showed limited evidence for the short-term efficacy of Ni cotine Replacement Thera py (NRT) and generally l ow medication compliance. Since higher compliance ra tes a re associated with i mproved a bs tinence ra tes, we examined i n a randomized controlled tri al 1) the efficacy of NRT and 2) the role of compliance. Pa rti ci pants (N=257, a ge: 16.7 1.13 yea rs ) a ttended an i nformation meeting followed by a 6- or 9-week treatment with ei ther a nicotine patch or a placebo patch. Data were required by means of 6 online questionnaires during treatment. Intention-to-treat analyses showed that the nicotine patch significantly i ncreased the chances of quitting 2 weeks after quit da te. At end-of-treatment we found that the efficacy of NRT was moderated by compliance, that i s, the nicotine patch s i gnificantly i ncreased the chances of quitting, but only i n high -compliant participants. No main effects of NRT, or a ny moderating effect of compliance were found for a bstinence a t 6 a nd 12 months follow-up. To go more deeply i nto the cours e of compliance, we s ubsequently investigated whether different adolescents’ NRT compliance tra jectories can be di s tinguished and whether these trajectories ca n be predicted by demographic factors, smoking (cessation) related factors a nd personality characteristics. Three trajectories were found, i .e. the ‘compliers’, the ‘moderate decreasers’ and the ‘s trong decreasers’. The differences in the development of these trajectories were predicted by three factors: the level of cons cientiousness a nd extraversion, and the number of previous quit attempts. Implications i n terms of personalityta rgeted interventions aiming to enhance compliance ra tes and suggestions for future research will be discussed.

Introduction Ci ga rette smoking a nd a ttention deficit hyperactivity disorder (ADHD) are highly co -morbid. Animal s tudies reported that expos ure to nicotine during adolescence ca n a ffect the developing brain and negatively i mpacts on attention. Ours is the fi rs t human s tudy exploring short- and long-term consequences of smoking during a dolescence on ADHD -symptoms.

Methods Da ta were a vailable for 1,987 a dult a nd 648 a dolescent monozygotic (MZ) twin pairs from the Netherlands Twin Register. Beca use MZ twi ns are genetically (a lmost) identical a nd (usually) grow up i n the same family, a causal effect of smoking on ADHD-s ymptoms can be i dentified with the discordant MZ co -twin design. Pa irs were classified as concordant/discordant for s moking initiation a nd compared on ADHD-symptoms using the ADHD i ndex (Conners) and the Youth Self Report a ttention s cale. Short-/long-term effects were investigated using data of multiple surveys from adolescence and/or a dulthood. Results Adul ts who never s moked scored significantly l ower on ADHD-symptoms than smokers. In 123 a dult pairs discordant for s moking, ADHD-symptoms i ncreased more from a dolescence to a dulthood i n the smoking versus the nonsmoking twin (p:0.033). In a dolescents, 71 pairs s cored s imilar on ADHD-symptoms when both didn’t s moke at a ge 15, while the smoking twi n tended to s core higher than his/her co-twin a fter becoming discordant at a ge 17 (p:0.079). In 21 pa irs discordant at a ge 15, the s moking twin tended to score higher than the co -twin (p:0.147), while both scored (similarly) high after becoming concordant s mokers a t age 17. Conclusion Res ults suggest that s moking during a dolescence ca uses a higher score on a ttention problems, lasting into adulthood .

Inleiding Ha rdcore rokers zijn rokers die al vele jaren roken (>15 ja ar) en niet van plan zijn te stoppen. Zij zi jn erg moeilijk te bereiken en te beïnvloeden door de huidige preventiemaatregelen. We onderzoeken of er in deze s pecifieke populatie verschillende s ubgroepen (profielen) te vi nden zijn. Zulke profielen zouden ons kunnen helpen hardcore rokers beter te bereiken en te beïnvl oeden. Methode In een eerder kwalitatief onderzoek hebben hardcore rokers i n focusgroepen voor- en nadelen va n roken en stoppen a a ngegeven. In de huidige studie hebben we deze voor- en nadelen onder 510 ha rdcore rokers i n een online survey ui tgevraagd. Vervolgens hebben we gemiddelde s cores op de voor-en nadelen berekend. Deze gemiddelden hebben we vervol gens gebruikt om met een latente profielen a nalyse verschillende profielen te onderscheiden. Resultaten We hebben drie profielen gevonden onder de hardcore rokers. Het meest voorkomende profiel (59%) bestond uit rokers di e erg ambivalent tegenover roken en s toppen s tonden. Zij zagen weinig voor- en nadelen va n zowel roken als stoppen. Het tweede profiel (36%) bestond uit rokers die relatief positief waren over s toppen. Rokers in dit profiel hadden veel na delen va n roken en zagen veel voordelen va n stoppen. Het derde profiel (5%) was juis t uitgesproken negatief over s toppen. Zij zagen weinig nadelen va n roken en ook weinig voordelen va n s toppen. Conclusie De res ultaten laten zien dat er onder hardcore rokers drie verschillende profielen te vi nden zijn. Kennis over de profielen ka n ons helpen bij het beter bereiken en beïnvl oeden van hardcore rokers.

Objective Toba cco control policies in Europe have been through major development over the past decade. We i nvestigated the effect of toba cco control policies on a dolescent smoking i nequalities in Europe. Methods Da ta of 77,629 15-16 yea r old adolescents from 13 European countries were obtained from the ESPAD 2003, 2007, 2011 da ta bases. We performed multilevel logistic regression a nalys es with daily s moking as the outcome va riable. Tobacco Control Policy Index (TCPI) measured a sum score for national tobacco control policy. TCPI b a nd TCPI t indicated respectively the between countries and over ti me associations between policies a nd s moking. Socioeconomic position (SEP) was based on pa ternal education and i nteraction with TCPI b and TCPI t was tested. Results In a ll s tudied countries, except Portugal, adolescent smoking prevalence rates were highest amon g l ow SEP respondents. TCPI b wa s i nversely a ssociated with s moking (OR=0.78 [0.64-0.93]). An i ncreasing TCPI t over ti me was not a ssociated with s moking rates (OR=0.95 [0.78-1.17]). Interaction between TCPI components a nd SEP wa s not significant. No evidence was found for a ti me l ag effect of TCPI, associations were similar when TCPI scores of up to four years previous were used in the regression models. Conclusion We found no evi dence for a n effect of tobacco control policies on smoking ra tes over ti me. H owever, countries with a s tronger tobacco control policy had l ower s moking ra tes. This indicates that tobacco control policies only have a long term effect. The between countries effect was similar a mong low a nd high SEP respondents, s uggesting that tobacco control pol icies did not i ncrease inequalities in a dolescent s moking.

Background Much ha s been achieved in tobacco control policy i n Europe over the past decades, but the progress has been marked by a l a rge degree of va riation between countries. Quantitative analyses of factors that influence the poli cy development process a re l acking. This study aims to provi de an insight in the political contextual factors that s haped tobacco control policy development. Methods Thi s study used data from 11 European countries from 1996 through 2010. The Tobacco Control Scale (TCS) was the policy outcome. Political contextual va riables included were political orientation a nd government effectiveness. Multilevel l inear regression modelling was used to investigate associations between the contextual va riables a nd the TCS , while controlling for gros s domestic product a nd overall s moking prevalence. Results In the a nalysis for the entire study period, we did not observe the expected associations between the TCS a nd political ori entation or government effectiveness. Analyses per year showed positive associations between a left political orientation a nd TCS from 1996 through 2003 a nd positive associations between government effectiveness a nd the TCS between 2001 a nd 2005. Further, we found that TCS va l ues from 2005 onwards were higher than would be expected based on these contextual political factors. Discussion The res ults s uggest that in the early s tages of tobacco control policy development, l eft governments had a pioneering role by i mpl ementing new policies when overall development was slow. Since a round 2002, i nternational factors l ike the Fra mework Convention on Tobacco Control (FCTC) s eem to have become the driving force behind tobacco control policy development.

Background Res piratory tra ct infections (RTIs) are the l eading ca use of child morbidity and mortality worldwide, a nd s econd -hand s moke exposure is a major risk factor. Increasing evidence s uggests important early-life population health benefits of s moke-free public environments. We investigated the i mpact of the English 2007 s moke-free l egislation on childhood hos pital-admissions for RTIs. Methods We us ed the Hospital Episode Statistics database to obtain nationwide data on unplanned hospital-admissions for acute RTIs a mong children (<15 years) from 2001-2012. Da ta were disaggregated by month, a ge-group, sex, a nd area, a nd linked to urba nisation level, region, deprivation index, a nd corresponding population estimates. Negative binomial regression a na lysis was performed, a djusting for confounders, seasonal variation, and population -size changes. Models accounted for underlying incidence trends, a nd allowed for s udden a nd gra dual changes following the s moke-free law. Findings We a nalysed 1,651,675 hos pital-admissions. The i ntroducti on of s moke -free l egi s l a ti on wa s fol l owed by a n i ns ta nt reducti on i n RTI a dmissions: -2.9% (95%CI: -4.0,-1.7), this mainly being a ttributable to a decrease in l ower RTI admissi ons : 10.8% (95%CI: -12.6,-9.1). In contrast, the reductions in a dmissions for upper RTIs were more incremental a nd s us ta i ned, a nd were therefore responsible for the majority of the estimated 24,098 RTI a dmissions cumula ti vel y prevented over the fi ve yea rs fol l owi ng the s moke -free l egi s l a ti on. Interpretation Introduction of national s moke-free legislation i n England was associated with approximately 5000/yea r fewer hospital a dmissions for RTIs among children. Whilst ca usality i s difficult to establish from this quasi-experimental analysis, when ta ken together with related work the findings s trengthen recommendations for global i mplementation of s moke-free public pl a ces.

Aanleiding Tegenwoordig mag er op steeds meer plekken niet meer gerookt worden, bijvoorbeeld i n openbare gebouwen, op werkplekken en i n de horeca. Daarnaast worden ook s teeds meer openbare ruimtes rookvrij, zoals treinstations. Een l ogische vervolgstap zou een rookverbod op schoolterreinen zijn i n het voortgezet onderwijs. Vaak s teken jongeren na melijk hier hun eerste sigaret op en raken ze verslaafd doordat ze erg gevoelig zijn voor nicotine. Momenteel zegt een kwa rt (26%) va n alle schoollocaties i n Nederlands een rookvrij s choolterrein te hebben. Rookvrije schoolterreinen zijn dus

nog ni et va nzelfsprekend. Dit onderzoek richt zich op het ontwikkelen van effectieve strategieën en methoden om s cholen te ondersteunen i n de a doptie-, implementatie- en continueringsfase bij het i nvoeren va n rookvrije schoolterreinen. Aanpak Deze ontwikkeling zal vormgegeven worden door 1) het omschrijven, wetenschappelijk onderbouwen en verder ontwi kkelen va n een aanpak die ontwikkeld is door de Alliantie Nederland Rookvrij!, 2) een landelijke procesevaluatie va n de a doptie-, i mplementatie- en continueringfasen bij scholen die al een rookvri j s choolterrein gerealiseerd hebben, 3) regi onaal: behoeftepeiling bij minder gemotiveerde scholen in drie GGD-regio’s, 4) regionaal: ontwikkelen en evalueren va n een a anpak om minder gemotiveerde scholen te bewegen tot a doptie en i mplementatie en 5) een l andelijke effectevaluatie (quasi experimenteel design) om te onderzoeken of minder jongeren beginnen met roken of op l atere l eeftijd pas beginnen met roken. Background Attendance of a behavioural therapy course fa cilitates s moking cessation. Disadvantaged smokers have been shown to a ttend l ess than their a dvantaged peers. This s tudy explores the underlying motivations, barriers a nd s ocial s upport of s mokers exhibiting different patterns of attendance a t a free smoking-cessation treatment program in a disadva ntaged nei ghbourhood of The Netherlands. Methods In 29 s mokers undertaking smoking cessation therapy i n a disadvantaged neighbourhood, qualitative interviews were compl eted, coded a nd a nalys ed. Motivations a nd barriers to a ttend a nd s ocial s upport, were analysed with reference to the Sel f-Determination Theory. Results We s a w two patterns of attendance: those who missed ≤ two s essions (“frequent a ttenders”), and > two s essions (“i nfrequent a ttenders”). These two groups differed in motivations a nd barriers to a ttend, a nd level of social support recei ved. Compa red with i nfrequent attenders, frequent a ttenders more often felt i ntrinsic motivation to attend (e.g. enjoyed a ttending), and named more s elf-determined extrinsic motivations to attend, such as making ti me for the course a nd wa nting to quit. Most of those mentioning i ntrinsic motivation did not mention a desire to quit a s a motiva tion for a ttendance. Frequent attenders mentioned no organizational barriers to a ttendance. Frequent attenders experienced more s oci al s upport. Conclusion We concl ude that motivation to a ttend, a s distinct from motivation to quit, is a n important factor i n a ttendance of s moking ces sation therapy courses i n disadvantaged areas. Some focus on i ncreasing motivation to a ttend ma y help to prevent pa rti cipants dropping out or missing sessions. Al so, increasing social s upport within the course may i mpact on attendance.

Objective Soci oeconomic s tatus (SES) and gender are known to be associated to the i ntention of s moking. The present exploratory s tudy a ims to i dentify gender differences in l ow SES a nd high SES children, since evi dence on this topic i s lacking. Design Da ta for this l ongitudinal study was gathered from a smoking prevention intervention study. A tota l of 1,801 chi ldren (aged 10-12 yea rs ) of 133 Dutch primary s chools completed a web-based questionnaire concerning their s moking i ntentions a nd other s moking related factors (e.g. a ttitude, s ocial i nfluence a nd s elf-efficacy) at baseline and one year follow-up. Logistic regression analyses were performed to a ssess differential effects to i dentify potential differences between SES a nd gender. Main results Sepa rate a nalyses of SES and gender were warranted, since a 3-way i nteraction effect was present (B=-1.63 (p=0.05)). High SES boys were more willing to initiate in smoking i f they ha d positive smoking a ttitudes, whereas high SES girls i ndicated to ha ve the i ntention to start s moking if they perceived l ess disadvantages (B=-0.86 (p=0.01)) of smoking. For l ow SES boys the perceived social norm (B=-0.72 (p=0.03)) was most s trongly a ssociated wi th their intention to s tart smoking. Low SES girls i ndicated to be most influenced to engage in smoking by the smoking behavior of people in their environment (B=1.71 (p<0.01)). Conclusion Thi s study i ndicated that different fa ctors are associated with the smoking i ntentions of boys and girls in high a nd l ow SES groups. More knowledge about this discrepancy i ncreases the opportunity to develop subgroup s pecific s moking prevention programs and may reduce youth s moking.

Background Wi th the general i ncrease in l ife expectancy a t birth in Europe, changes to the age distribution of dying occurred. This paper exa mines the role of smoking i n the two dimensions behind changes over ti me i n adult mortality, i .e. rectangularization of the s urvival curve (decline of premature deaths) and longevi ty extension (delay of the maximum a ge of death). We do so for ten European countries s ince the 1950s .

Methods For a l l-cause mortality a nd non-smoking-related mortality, we examined the trends in l ife expectancy, recta ngularization a nd l ongevity extension from 1950 to 2009. Thes e trends were correlated with indirectly estimated smoking prevalence. We s tudied the national s ex-specific populations from Denmark, Finland, France, West Germany, Italy, the Netherlands, Norwa y, Sweden, Switzerland and the United Ki ngdom, aged 50 a nd over. Results For a l l-cause mortality, rectangularization a ccelerated almost simultaneously a round 1980 a mong males. Both deceleration a nd a cceleration were observed a mong females. Trends in smoking prevalence correlated negatively wi th recta ngularization. A negative correlation was observed with l ongevity extension as well, although less marked. When cons idering only non-smoking-related mortality, rectangularization among males no longer a ccelerated a round 1980. Among females, differences between all-cause mortality a nd non-smoking-related mortality were smaller than a mong ma l es, but more visible for rectangularization than longevi ty extension. Rectangularization contributed l ess to the i ncrease i n l ife expectancy tha n longevity extension. For males, more so for n on-smoking-related mortality than for a ll-cause morta lity. Conclusions For both ma les a nd females, s moking affects the rectangularization of the survi val curve more than the s hift of the survival curve to the ri ght.

Provi ding an estimate of the future burden of smoking i s therefore important for toba cco control programs, updating a nd extending the descriptive smoking epidemic model, a nd the projection of allca us e mortality. Our objective is to project smoking-attributable mortality i nto the future for the Netherlands. Pa st trends i n lung cancer morta lity (1950-2010) a nd s moking prevalence i n Denmark, England & Wa les a nd the Netherlands a nd the generalities from the descriptive s moking epidemic model, a re used to formulate assumptions. We then extrapolate lung cancer mortality us i ng different extrapolation techniques. By a pplying a simplified and adapted version of the Peto-Lopez method to the future l ung ca ncer mortality, we obtain the future s moking-attributable mortality. The pa st trends showed clear convergence between men and women, a close link between the ti ming of the decline for women and the declining trend for men, a nd necessitated a cohort a pproach for the increase a nd a period a pproach for the decrease. The smoking-attributable mortality fra ction for men is expected to further decline to 14% i n 2050. For women, the fra ction will first increase to a maximum of 23% i n 2033, but then decline to 19%. Smoki ng-attributable mortality will remain important for the future, especially for women. The projected s mokinga ttri butable mortality fra ctions ca n easily be i ncorporated i n the projection of all-cause mortality.

The i ntake of nicotine by s moking ci garettes is modelled by a dynamical sys tem of d ifferential equations. The va riables a re the i nternal l evel of nicotine and the level of cra ving. The model i s based on the dynamics of neural receptors and the way they enhance cra ving. Li ghting of a ci garette is parametrised by a time-dependent Poisson process. The nicotine i ntake ra te i s a ssumed to be proportional with the parameter of this stochastic process. The effect of cra ving is damped by a control mecha nism i n which awareness of the risks of s moking and societal measures play a role. Fluctuations in this damping may ca us e transitions from smoking to non-smoking and vi ce versa. With the use of Monte Ca rlo simulation the effect of abrupt a nd gra dual cessation therapies are evaluated. The "missing=smoking" assumption: a fallacy in internet-based tobacco cessation intervention RCTs?

Introduction Mi s singness of follow-up data i n RCTs of s moking cessation interventions is a common yet potentially i mpactful nuisance. Internet-based RCTs often cope with higher a ttrition ra tes than face-to-face i nterventions. A common a pproach to dealing wi th missing smoking outcomes i s to recode missing data points as s moking (“missing=smoking”), assumed a ‘conservative’ a pproach compared to complete case a nalysis. We will s crutinize the assumption that missing=smoking yi elds conservative effect estimates in comparison to complete case a nalysis when data were generated by i nternet-based RCTs . Methods Da ta from 22 s tudies included i n a meta-analysis of i nternet-based smoking cessation interventions (Civljak, 2013), a ttri tion ra tes , relative a ttrition (difference i n a ttrition ra te between tri al arms), a nd associations with outcome were re-assessed under the missing=smoking assumption and under a complete case approach. Results The a veraged weighted a ttrition ra te across the 22 s tudies i s 51% a t follow-up. Missing=smoking relative ri sk (RR) results (RR=1.15 [1.00; 1.33]) were a lmost i dentical to results under complete case analysis (RR=1.14 [0.98; 1.32]). The a ttrition ra te wa s found to be s ignificantly negatively a ssociated with RR (R-Squared=0.24, p=0.03).The difference in RR under mi s sing=smoking a nd complete case a nalysis was strongly associated with the i nteraction attrition ra te × relative a ttrition (R-Squared=0.74, p<0.001). When a ttrition was relatively hi gher in the experimental arm, missing=smoking l ed to a l ower RR es timate than complete ca se analysis; however when a ttrition was relatively higher i n the control arm, missing=smoking l ed to a higher RR estimate than complete case analysis. This effect was stronger in studies with higher a ttrition ra tes.

Discussion Ba s ed on our meta-analyti c re-analysis of 22 i nternet-based smoking cessation interventions, we demonstrate the contrai ntuitive consequences of the missing=smoking assumption. Inleiding Hui sartsen en praktijkondersteuners (POH’s) ervaren negatieve reacties va n patiënten t.a.v. het stoppen met roken als ba rri ère bij de implementatie va n richtlijnen voor ta baksontmoediging. De s tudie ging na i n hoeverre huisartsen en POH’s deze ri chtlijn implementeren en hoe patiënten hierop reageren tijdens het consult. Methode We s electeerden 52 vi deoconsulten (20 huisarts, 32 POH) waarin professionals het gesprek over roken i nitieerden en we tra ns cribeerden gespreksfragmenten over roken. Gesprekseenheden va n professionals (5 A’s : Ask, Advise, Assess, Assist, Arrange) en pa tiënten (negatieve/positieve reacties t.a .v. s toppen) werden gecodeerd en geanalyseerd met sequentie a na lyses. Resultaten Indien de richtlijn werd geïmplementeerd, vroegen huisartsen relatief va ker naar de rookstatus va n patiënten dan POH’s (42.2% vs . 26.2%; p<0.000) en ondersteunden POH’s va ker een s toppoging (55.2% vs . 25.4%; p<0.000). Over het a lgemeen rea geerden rokers va ker negatief dan positief t.a .v. s toppen met roken (23.3% vs . 11.9%; p<0.000). De kans op een nega tieve reactie va n de patiënt was significant groter nadat POH’s vroegen naar de stopmotivatie (OR 3.61 95% BI 1.44 9.01) of na dat POH’s ondersteuning bood bij een stoppoging (OR 2.23 95% BI 1.43-3.48) i n vergelijking met elk andere voorga ande gesprekseenheid va n POH’s. In het laatste geval was de kans ook significant groter dat patiënten positief rea geerden t.a .v s toppen (OR 3.17 95% BI 1.74-5.76). Discussie Op ba sis va n de bevi ndingen a dviseren wij een Ask-Advise-Arrange a anpak voor de huisarts. Deze aanpak leidt tot de mi nste kans op negatieve reacties va n patiënten t.a.v. s toppen en sluit goed aan bij het takenpakket en de va ardigheden va n POH’s .

Young Adult Smoking in Peer Groups: An Experimental Observational Study

Introduction The a im of this experimental observational study i s to examine whether, in a group setting (same-sex triads), passive peer i nfluence (imitation) i n the context of homogeneous a nd heterogeneous (contradictory) behavior of peer models affects young a dults’ smoking behavior. Methods An experiment was conducted a mong 48 daily-smoking college a nd university s tudents aged 17–25. Pa rti cipants had to compl ete a 30-min music task wi th two same-sex confederates. We tested the following three conditions: (a) neither of the confederates is s moking, (b) one confederate is smoking and the other is not, and (c) both confederates a re s moking. The pri ma ry outcome tested was the total number of cigarettes smoked during the task. Results Students i n the condition with two smoking peer models and in the condition with one s moking peer model and one nons moking peer model smoked significantly more ci garettes than those i n the condition with two nonsmoking peer models. However, results for the condition with two smoking peer models did not differ significantly from the condition wi th one smoking peer model a nd one nonsmoking peer model. Conclusion Our fi ndings show that i n a group setting, the impact of the homogeneity of smoking peers on young a dults’ smoking behavi or is not greater than the i mpact of the heterogeneity of s moking and nonsmoking peers. This would suggest that the s moking peer in the group has a greater i mpact on the daily-smoking young a dult, thus reducing or even eliminating the protecti ve effect of the nonsmoking peer model.

Methoden en resultaten Ik za l uitgaan va n een conceptueel schema waarin het vóórkomen van (mee-)roken i n Nederland centraal staat. Vers chillende factoren beïnvloeden de prevalentie va n roken, waaronder het gevoerde tabaksbeleid. Op haar beurt bepaalt deze prevalentie op l ange termijn de hoeveelheid ziekte- en s terftelast als gevolg va n roken i n Nederland. Inleiding Na ti onale en i nternationale monitoring va n roken i s van belang om 1) ontwikkelingen in het rookgedrag te kunnen volgen, 2) te kunnen vergelijken met a ndere l anden, 3) ri sicogroepen te kunnen i dentificeren en 4) het draagvlak voor bel eidsmaatregelen vast te stellen. Methode en Resultaten In de presentatie wordt een overzicht gegeven va n verschillende monitors die op dit moment i n Nederland worden ui tgevoerd op het gebied va n roken, bij zowel volwassenen (Continu Onderzoek Rookgewoonten, gezondheidsenquête CBS) a l s jongeren (Roken Jeugd Monitor, Peilstationsonderzoek/ESPAD, HBSC studie). Verschillen tussen de s tudies worden bel icht en enkele bevi ndingen worden gepresenteerd. De presentatie eindigt met een korte blik op de toekomst: de i ntegratie va n bestaande door VWS gefinancierde monitors i n één landelijke leefstijlmonitor. Discussie In de discussie kan ingegaan worden op de voor- en nadelen va n het nieuwe monitoringssysteem. Kan de landelijke l eefstijlmonitor voldoende i nformatie bieden? Waar kunnen eventueel gaten vallen? En hoe worden deze opgevuld?

Inleiding Internationaal vergelijkend onderzoek naar ta baksbeleid i s van groot belang om nationale cijfers in perspectief te plaatsen. Ook ka n een landenvergelijkend onderzoeksdesign worden ingezet om de impact va n nationaal ta baksbeleid aan te tonen. Methode Het Nederlandse ta baksbeleid wordt vergeleken met meer dan 20 a ndere l anden wereldwijd in het International Tobacco Control Policy Eva luation Project (of ITC Project) en met de 26 a ndere EU-lidstaten i n de Eurobarometer. Het ITC Project i s een l ongitudinaal vra genlijstonderzoek onder ongeveer 2,000 rokers per l and. De Eurobarometer is e en cross-sectioneel populatie-onderzoek onder ongeveer 1,000 mensen per land. Resultaten Enkele belangrijke landenvergelijkende resultaten uit het ITC Project en de Eurobarometer worden gepresenteerd. Het percentage rokers is in Nederland lager dan gemiddeld i n Europa. Ui t beide studies blijkt dat in Nederland minder s teun i s voor ta ba ksbeleid dan i n andere landen. Analyses op basis va n data van het ITC Project s uggereren dat het Nederlandse ta ba ksbeleid minder effectief i s in het s timuleren va n stoppen met roken dan dat in a ndere landen. Nederlandse rokers l i jken zich bovendien minder bewust te zijn va n de schade va n roken dan rokers i n a ndere l anden.

Discussie: Hoewel data va n de Eurobarometer l aten zien dat i n Nederland minder wordt gerookt dan in a ndere Europese l a nden, l ijkt het erop dat het Nederlandse tabaksbeleid minder effectief is i n het stimuleren va n stoppen met roken dan het bel eid i n andere landen. Dit kan komen door een minder s trikte i nvoering van het beleid, maar zou ook voort kunnen komen uit culturele verschillen.

Inleiding Deze studie ging na wat het effect was va n de vergoeding va n het s toppen-met-rokenprogramma i n 2011 op het aantal s toppen-met-rokenmiddelen dat voorgeschreven werd door huisartsen en verstrekt door a pothekers en op de prevalentie va n roken. Methode Dri e l andelijk representatieve databases werden gebruikt om i nzicht te krijgen in het a antal voorschriften va n stoppen-metrokenmiddelen i n de huisartspraktijk (LINH NIVEL), verstrekkingen door apothekers (SFK) en prevalentie va n roken (COR STIVORO) i n de periode 2001-2012. Da ta werden i n kaart gebracht door lijngrafieken en gesegmenteerde regressieanalyses. Resultaten Kort na da t het stoppen-met-rokenprogramma in 2011 werd vergoed steeg het totaal a antal voorschriften door de huisarts s i gnificant met 6,3 per 1000 rokers (95% BI 2,9-9,8; p=.001). Ook het totaal aantal verstrekkingen door de apotheker steeg i n di t jaar significant met 17,3 per 1000 rokers (95% BI 12.5-22.0, p<.000). In datzelfde jaar daalde de prevalentie va n roken s i gnificant met 2,9% (95% BI 4,6-1,1; p=.002). Toen in 2012 het stoppen-met-rokenprogramma niet meer vergoed werd, da a lde het aantal verstrekkingen met 21,6 per 1000 rokers (95% BI 26,0-17,2; p<.000) en s teeg de prevalentie va n roken met 1,2% (95% BI 0,5-2,8; p=.156). Discussie De vergoeding va n het stoppen-met-rokenprogramma va nuit de basiszorgverzekering in 2011 gi ng samen met een toename va n het a antal voorschriften en verstrekkingen van stoppen-met-rokenmedicatie in de eerste lijn en een afname va n het a a ntal rokers.

Cha nges i n smoking prevalence due to tobacco control policies or other i nterventions s trongly i nfluence health outcomes. Thes e in turn ca n va ry over ti me. Modeling tools a re necessary to dynamically quantify the varyi ng impact of such policies in order to help health policy makers s et priorities i n their policy a genda. For thi s purpose s everal simulation models have been developed and applied to the situation i n the Netherlands and in other countries. Such s imulation models combine data from different sources to make projections of the effects of policy a nd intervention s cenarios in a population. There are also other modeling tools which e.g. can specifically s how the potential for reduction i n i nequalities i n s moking-related mortality due to changes i n s moking prevalence. In thi s presentation the DYNAMO-HIA, the SimSmoke and the Chronic Disease Model will be introduced. We will illustrate thei r data needs and some technical considerations (e.g. the i mportance of model assumptions and specifications or taking i nto a ccount time since quitting s moking). Their different outcome measures/outputs will also be presented.

Speaking Words of Wisdom: Wat ouders kunnen doen om hun kinderen van het roken te weerhouden

Inleiding Eerder onderzoek wijst uit dat preventieprogramma’s gericht op ouders veelbelovend zouden kunnen zijn in het verlagen va n de prevalentie va n roken. Het doel va n dit vi jfjarige onderzoek was om meer inzicht te krijgen i n de rol va n ouders in rel a tie tot het rookgedrag va n hun kinderen. Methoden Voor di t onderzoek zijn de volgende studies gedaan: zes l ongitudinale vra genlijststudies onder 428 Nederlandse gezinnen, een l ongitudinale vra genlijststudie onder 6,522 Amerikaanse jongeren en twee observatie-experimentele studies onder jonge kinderen. Resultaten Ui t de verschillende studies blijkt dat de kwaliteit en frequentie va n de rookspecifieke communicatie gerelateerd i s aan het rookgedrag va n jongeren. Wa nneer ouders op een opbouwende en respectvolle wijze praatten over roken, bleek de kans kl einer dat zij rokende kinderen hadden. Wanneer ze echter veel over roken praatten hadden ze juist een grotere kans om rokende kinderen te hebben. Ook blijkt dat ouders de kans op roken kunnen verminderen door regels op te leggen. Als ki nderen films bekijken waarin wordt gerookt, dan gaan ze zelf ook s neller roken. Door kinderen te verbieden naar deze fi l ms te kijken, kunnen ouders de kans dat hun kinderen beginnen met roken verkleinen. Ui t de twee observatie experimentele studies blijkt dat kinderen al op jonge leeftijd ideeën over rok en hebben ontwikkeld.

Conclusies In het kader va n preventie gericht op ouders s tellen we voor dat de doelgroep zich verder zou moeten uitbreiden naar (ouders va n) jonge kinderen. Tot slot zouden ouders ondersteund moeten worden in hoe ze rookspecifieke opvoedingsstrategieën s uccesvol kunnen inzetten.

Inleiding Veel programma’s gericht op de preventie va n rookgedrag va n adolescenten zijn niet effectief. Dit zou kunnen doordat preventieprogramma’s va ak op middelbare scholen worden uitgevoerd, i n een periode waarin een groot deel van de ki nderen al erva ring op doet met roken. Daarnaast worden bij deze programma’s ouders vaak niet betrokken. Het preventieprogramma Rookvrije Ki ds richt zich op kinderen in de l eeftijd va n 9-11 jaar die nog niet roken en het programma wordt s a men met de ouders thuis uitgevoerd. De huidige studie evalueert de effectiviteit va n dit programma op beginnen met roken en rookgerelateerde cognities met behulp van een Randomized Controlled Trial (RCT). Methode De i nterventiegroep (n=728) kreeg vi jf magazines thuis gestuurd die zich ri chten op het verbeteren va n ouderlijke va a rdigheden om met kinderen over roken te kunnen praten. De controlegroep (n=750) kreeg magazines die bestonden uit i nformatie gebaseerd op feiten va n roken en tabaksgebruik. De k inderen werden op 5 meetmomenten geïntervi ewd (ba seline, 6, 12, 24 & 36 ma a nden). Resultaten Na 6 ma a nden had het programma i nvloed op een aantal rookspecifieke cognities en op een a antal onderdelen van rooks pecifieke opvoeding. Na 36 ma anden werd geen effect gevonden op beginnen met roken (I: 10.8% versus C: 12%, p >.05). Voor rookspecifieke cognities werd na 36 maanden a lleen een effect op sociale norm va n moeder gevonden. Conclusie Gebaseerd op bevindingen afkomstig van een RCT naar de effectiviteit va n het preventieprogramma Rookvri je Ki ds, lijkt het erop da t preventie voor kinderen i n de leeftijd va n 9-11 ja ar te vroeg is om te voorkomen dat jongeren ti jdens de a dolescentie beginnen met roken.

Inleiding Rokende ouders vormen een grote subgroep binnen de populatie va n vol wassen rokers (20-40%). Kinderen va n rokende ouders worden va ak blootgesteld aan tabaksrook. Hierdoor komen ze niet alleen in aanraking met de negatieve gezondheidseffecten va n passief roken, maar hebben ze ook een verhoogd risico om zelf te beginnen met roken. De huidige s tudie evalueerde de effectiviteit va n een telefonisch stoppen -met-roken coachingstraject voor rokende ouders. Methode Rokende ouders werden geworven via basisscholen. Vervolgens werden ouders at ra ndom toegewezen aan telefonische coa ching (n=256) of een controle groep (standaard zelfhulp brochure) (n=256). Er wa ren drie meetmomenten waarop aan ouders en kinderen werd gevraagd een vra genlijst in te vullen (baseline, 3-maanden postmeting, 12-maanden follow-up). Resultaten Ouders die deelnamen aan telefonische coaching hadden een grotere kans om te stoppen met roken in vergelijking met ouders die een zelfhulp brochure ontvingen (gestopt na 12-maanden: 34.0% vs . 18.0%, p<.001). Da arnaast waren ouders di e deelnamen aan telefonische coaching meer geneigd om nicotinevervangers te gebruiken (48.1% vs . 20.9%, p<.001), rookten ze minder sigaretten per dag (10.4 vs . 13.1; p<.001) en waren ze meer geneigd om naar aanleiding va n de i nterventie een volledig rookverbod in huis te i mplementeren (41.6% vs . 26.9%; p<.01). Conclusie Ba s isscholen zijn een potentieel belangrijke setting om rokende ouders in contact te b rengen met s toppen-met-roken i nterventies. Telefonische coaching is een zeer effectieve methode om ouders te ondersteunen bij het stoppen met roken. Zel fs bij ouders die niet gestopt zijn had telefonische coaching positieve effecten zoals het i nvoeren va n een volledig rookverbod in huis.

Wa t kunnen we l eren va n de praktijk als het gaat om de rol va n ouders bij de preventie va n roken? Hoe kan worden i ngespeeld op a ctuele onderwerpen zoals de verhoging va n de l eeftijd voor de verkoop va n ta bak? Eerder onderzoek laat zien dat ouders invl oed kunnen uitoefenen op het voorkomen va n (mee)roken door hun kind. Deze kennis i s toegepast i n opvoedingsadviezen, interventies en ca mpagnes gericht op vrouwen vóór, ti jdens en na de zwa ngerschap en ouders met opgroeiende kinderen. Ouders krijgen informatie over rokenopvoeding a fzonderlijk of in s a menhang met andere leefstijlthema's aangeboden. Opvoedingsadviezen zijn bijvoorbeeld: niet of zo min mogelijk roken

i n het bijzijn va n de kinderen, het uitdragen va n een a nti-rooknorm en a f en toe een constructief gesprek voeren met de ki nderen over niet-roken. Onderzoek rondom opvoedcampagnes l aat zi en dat ouders roken een belangrijk onderwerp vi nden, dat zi j openstaan voor pra ktische tips om hun kind te beschermen tegen (mee)roken en er ook wat mee doen. Lastiger wordt het als kinderen ouder worden, dan neemt het vertrouwen i n de eigen opvoedingsstrategieën a f. Ouders verwachten va n overheid, s cholen en verkopers dat zi j zi ch inzetten om hun kind te beschermen tegen (mee)roken. Ouders ondersteunen de verhoging van de l eeftijdsgrens voor de verkoop van tabak en alcohol naar 18 ja ar. Op i nitiatief va n het ministerie va n VWS is een meerjaren ca mpagne gestart gericht op het verstevi gen van de sociale norm 'niet roken en niet drinken' onder de 18. De campagne ri cht zi ch op ouders, jongeren en omgevi ng en wordt de komende ja ren verder ingevuld.

Het gebruik va n elektronische sigaretten komt op i n Nederland. Elektronische sigaretten of e -sigaretten zijn a pparaatjes op ba tterijen voor het toedienen va n nicotine die zijn ontworpen om op vergelijkbare wi jze als tra ditionele sigaretten te worden gebruikt. Deze producten zi jn bedoeld om gebruikers s nel va n nicotine te voorzien zonder hen bloot te stellen aan de vel e s chadelijke chemicaliën die zich i n tra ditionele sigaretten bevinden. Hoewel relatief veilig vergeleken met een tra di tionele sigaret, brengt gebruik va n het product wel degelijk gezondheidsrisico' met zich mee en is het mogelijk zelfs een opstapje voor jongeren om sigaretten te gaan roken. Bovendien is er nog niet genoeg bewijs om de claims te onderbouwen dat het product zou helpen om te stoppen met roken. Deze situatie roept de vra ag op hoe we ons tot de es i garet moeten verhouden. In deze sessie bespreken we i n vi er presentaties het gebruik en de gebruikers va n e -sigaretten, ni euwe cijfers over (redenen voor) gebruik in Nederland uit de International Tobacco Control (ITC) s tudie, de samenstelling en met gebruik samenhangende gezondheidsrisico’s en, tot slot, de zoektocht naar een passende wettelijke kader voor regul ering.

Resultaten en discussie In Nederland gebruikte tussen 1 en 7% va n de inwoners wel eens een e -sigaret, vooral huidige- en ex-rokers. Ci jfers over het gebruik door jongeren zijn niet beschikbaar. In de VS, waar de e -sigaret eerder op de markt verscheen, verviervoudigde tus s en 2009 en 2010 het aantal volwassenen dat de e -sigaret wel eens gebruikte. Het aantal jongeren dat de e -sigaret probeerde verdubbelde tussen 2011 en 2012 (tot 6,8% onder 11-15-jarigen en 10% onder 15-18-jarigen). Voorstanders zien de e-sigaret als een middel dat het stoppen met roken ondersteunt of als een gezonder a lternatief dat a ls s cha debeperkende methode kan worden ingezet. De e -sigaret kan de hunkering naar sigaretten en ontwenningsverschijnselen bij het s toppen met roken verminderen. Verschillende s tudies tonen een a fname va n ta ba ksgebruik aan of complete abstinentie. Een beperking va n veel va n deze studies is de veelal korte follow-up periode. Tegenstanders zien de e -sigaret als een ga teway naar het roken va n de conventionele s igaret, met name voor jongeren, en wi jzen erop dat de e-sigaret het roken weer normaliseert. Aa n het eind va n deze presentatie wordt bediscussieerd welk a dvi es door gezondheidszorgprofessionals aangaande gebruik va n de e -sigaret gegeven kan worden, de huidige onzekerheden over veiligheid, kwaliteit en effectivi teit a ls s topondersteuner in aanmerking nemende.

Introduction E-ci ga rettes are battery operated nicotine delivery devices used in a similar fashion as conventional ci garettes. Here we des cribe the current health and safety issues of e-cigarette use. Method A l i terature research was performed to i nvestigate the health a nd s afety i nformation of e -cigarettes. In addition, a new risk a s sessment method was used to a ssess the risk of exposure of propyl ene glycol and glycerol, the two major components i n the a mpules of e -cigarettes. Results and Discussion The ma jor health concern of e -cigarettes is nicotine because it ca n be l ethal to a dults with a n a cute intake of 30 to 60 mg, whi le a n a cute intake of 10 mg of nicotine i s lethal for children. Nicotine is a highly a ddictive substance a nd s ome evi dence s uggests that i t may a lso be a tumor promoter. Risk assessment of propylene glycol and glycerol present i n e -cigarettes tes ted were found to be a t levels for risk of i rritation of the respiratory tra ct. E-cigarettes may also contain other toxic s ubstances suspected of being harmful to humans (diethylene glycol, nitrosamines). Exposure i nformation (users, duration of va pi ng session, number of va ping sessions per day, etc.) is needed to better assess the potential health risks of e ci ga rettes.The majority of products do not have i nstructions or health/safety wa rning labels. Currently, e-cigarettes are ra rel y regulated and do not fall under the conditions of tobacco laws and because of this, they ca n be sold anywhere from s ma ll kiosks and stores to online. Finally, the manufacture, quality control, sales and advertisement of e -cigarettes remains unregulated.

Introduction In mos t European countries, electronic ci garettes (e-cigarettes) are becoming popular a lternatives for regular ci garettes. However, little is known about how many people use them in the Netherlands a nd why. Methods Da ta from the International Tobacco Control (ITC) Netherlands Survey 2008-2013 were a nalyzed. Smokers a ged 15 yea rs a nd older were i nterviewed using online surveys i n 2008 (n = 2,224), 2010 (n = 1,723), a nd 2013 (n = 1,531).

Results Awa reness of e -cigarettes decreased between 2008 (97%) a nd 2013 (86%) while ‘ever use’ increased (15% and 24% of res pondents who were a ware, respectively). ‘Current use’ decreased between 2008 (25% of ‘ever users’) a nd 2010 (8%) and i ncreased again between 2010 a nd 2013 (23%). E-ci garettes were mainly used for health-related reasons, as respondents bel ieved they a re less harmful than regular ci garettes (79%), to cut down on regular ci garettes (71%) or to help them to quit s moking (66%). Discussion E-ci ga rette use is increasing among Dutch s mokers. Respondents indicated they were mainly used to reduce the harm of regul ar cigarettes or with the i ntention to reduce or quit smoking. Future l ongitudinal research can help to clarify whether e-ci garette use indeed results i n i ncreased smoking cessation rates.

De i nvloed va n de elektronische sigaret op de volksgezondheid zal in belangrijke mate a fhangen va n de regulering va n het product. In deze presentatie staat daarom de juridische status va n de elektronische s igaret i n Nederland centraal. Het voor de elektronische sigaret toe te passen wettelijke kader i s vanaf de i ntroductie va n het product op de Nederlandse ma rkt i n 2007 punt va n discussie geweest. In de eerste plaats zal daarom kort worden ingegaan op de totstandkoming va n de huidige status va n de elektronische sigaret als regulier consumentenproduct. Hierbij zal i n het bijzonder a andacht worden besteed aan de marketing van het product als alternatief genotsmiddel (in plaats va n stoppen-met-roken middel), a l smede aan de krappe definitie va n tabaksproducten i n de Nederlandse Tabakswet. Vervol gens zullen de recente ontwikkelingen op het Europese niveau i n het kader va n de aangepaste Ta ba ksproductenrichtlijn worden besproken. De opties die op dit moment op ta fel liggen, zijn de regulering van de ni cotinehoudende elektronische sigaret als een geneesmiddel (zoals bepleit door de Europese Commissie en de Raad va n Mi ni sters) of onder een aangepast tabaksproductenregime (zoals voorgesteld door het Europees Parlement). Tot s l ot zullen deze ontwikkelingen (en de huidige juridische situatie) worden beschouwd i n het licht va n de risico’s en ka nsen waarmee elektronische sigaretten veelal worden geassocieerd. In deze context zal ook het gebrek aan verdere regul ering betreffende de verkoop en/of het gebruik van de nicotinevrije elektronische sigaret (i nclusief de zogenaamde Shi shapen) besproken worden.

In een i nteractieve s essie en aan de ha nd va n voorbeelden en kleine opdrachten worden ti ps en tricks besproken over het s chrijven va n titel en a bstract va n een a rtikel. De workshop is vooral gericht op jonge onderzoekers die met hun eerste a rtikelen bezig zijn maar i nteressant voor gevorderde onderzoekers die de i mpact va n hun publicaties willen vergroten.
