The Supporter agrees to provide support for the named activity by means of an unrestricted educational grant to [name of organization] in the amount of $[#]. CONDITIONS 1. Purpose of Activity: This activity is for scientific and educational purposes only and will not promote the Supporter’s products, directly or indirectly. 2. Control, Content, and Selection of Presenters and Moderators: Sponsor is ultimately responsible for control of content and selection of presenters, moderators, committee members, and others involved in the activity. Supporter, or its agents, will respond only to Sponsor-initiated or Joint Sponsor-initiated requests for suggestions of presenters or sources of possible presenters. Sponsor will make selection of presenter(s) and others involved in planning based on balance and independence. There will be no “scripting,” emphasis, or influence on content by the Supporter or its agents. 3. Disclosure of Financial Relationships: Sponsor will ensure disclosure to the audience of (a) Supporter funding and (b) any significant relationship between the Sponsor and the Supporter (e.g., grant recipient) or between individual speakers or moderators and the Supporter. 4. Ancillary Promotional Activities: No promotional activities will be permitted concurrently in the same room or obligate path as the accredited educational activity. No product advertisement will be permitted concurrently in the room during the time the accredited educational activity takes place. 5. Objectivity and Balance: Sponsor and Joint Sponsor will make every effort to ensure that data regarding the Supporter’s products (or competing products) are objectively selected and presented, with favorable and unfavorable information and balanced discussion of prevailing information on the product(s) and/or alternative treatments. 6. Limitation of Data: Sponsor and Joint Sponsor will ensure, to the extent possible, disclosure of limitations of data, e.g., ongoing research, interim analyses, preliminary data, or unsupported opinion. 7. Discussion of Unapproved Uses: Sponsor and Joint Sponsor may include presenters that will discuss off-label use of products. 8. Opportunities for Debate: Sponsor and Joint Sponsor will ensure opportunities for questioning or scientific debate within the educational activity.

Independence of Sponsor in the use of Contributed Funds: a) Funds should be in the form of an unrestricted educational grant made payable to [name of Sponsor or Joint Sponsor]. b) All other support associated with this activity (e.g., distributing brochures, preparing slides) must be given with the full knowledge and approval of Sponsor. c) No other funds from the Supporter will be paid to the program director, faculty, or others involved with the activity (additional honoraria, extra social events, etc.) as compensation for this program. Termination: Supporter, Joint Sponsor or Sponsor shall have the right to terminate this Agreement for any reason on thirty- (30) days written notice to the other parties. In the event of such termination, Supporter shall be responsible for paying to Sponsor those amounts actually spent, or irrevocably committed, by Sponsor with respect to the activity. Any overpayments or any other amounts in possession of the Sponsor will be reimbursed to the Supporter within thirty days of termination. In case of early termination by either Sponsor or Supporter, either party shall be able to use completed curriculum and educational materials developed pursuant to this agreement, provided that the Supporter does not use Sponsor’s name in any activity or promotion without prior written permission of Sponsor.
The Supporter agrees to: 1) abide by all requirements of the Accreditation Council for Continuing Medical Education (ACCME) Standards of Commercial Support for Continuing Medical Education; 2) abide by all requirements of Sponsor’s Commercial Support Policy, and 2) pay the unrestricted educational grant within 30 days of signing this Letter of Agreement. The Sponsor and Joint Sponsor agree to: 1) abide by the ACCME Standards for Commercial Support of Continuing Medical Education; 2) acknowledge educational support from the commercial company in program materials, 3) furnish the Supporter activity reports concerning the expenditure of funds provided by the grant, and 4) apply any unanticipated funds remaining from program sessions to related program activities.

6. Sponsor dan Bersama Sponsor setuju untuk: 1) mematuhi Standar ACCME untuk Dukungan Komersial Pendidikan Kedokteran Berkelanjutan; 2) mengakui dukungan pendidikan dari perusahaan komersial pada materi program, 3) memberikan kegiatan Supporter laporan mengenai pengeluaran dana yang disediakan oleh hibah, dan 4) menerapkan dana tidak terduga yang tersisa dari sesi program untuk kegiatan program yang terkait.
