Het belang van de ‘ideale houding’ om rugpijn te voorkomen op de werkplek is bij velen ingebakken als vuur blussen met water. Maar klopt dit wel? In het projec Spa(ine)@work t onderzoeken we welke fysieke en mentale factoren of welke combinaties ervan aanleiding geven tot problemen op de werkplek. We richten ons op mensen met ontstekingsgebonden rugklachten (axiale spondyloartritis, ziekte van Bechterew) omdat net zij het erg moeilijk hebben om optimaal te functioneren op de werkvloer. Zinvol en succesvol werk bepaalt mee de kwaliteit van leven. Daarom 'kijken' we niet enkel toe, maar zullen we ook uitzoeken welke behandeling de best aanpak biedt voor de individuele factoren die optimaal functioneren op het werk verhinderen.. EN A common advise of care givers (but also of colleagues, partner and friends) who focus on ergonomics at work. The importance of the ‘ideal posture’ to tackle spinal pain crosses the mind as naturally as extinguishing fire with water. But is this true? In the Spa(ine)@work project we investigate physical and mental factors or a combination thereof in the occurrence of work-related problems. We focus on patients with inflammatory spinal conditions (axial spondyloarthritis, ankylosing spondylitis) because they experience serious difficulties to perform optimally at the workplace. A meaningful and successful job significantly adds to the quality of life. Therefore, we do not only ‘study’, but also put findings into practice to evaluate which tailored treatment program is best to optimize work participation.

Identification of risk factors associated with the progression of knee-osteoarthritis: a 4 year prospective longitudinal study

[email protected] 29.000 € NL Osteoartrose (OA) van de knie is een vaak voorkomende chronische degeneratieve gewrichtsaandoening, die gepaard gaat met pijn, spierzwakte, stijfheid en instabiliteit, alsook beperkingen in fysieke activiteiten. Preventie van OA is dus belangrijk maar daarvoor is kennis van de risicofactoren in de progressie en/of het ontstaan van osteoartrose van primordiaal belang. In de literatuur zijn er fragmentarisch reeds verschillende risicofactoren voor progressie van artrose beschreven, onder meer neuromusculaire factoren (vb. spierkracht, spiermassa) en biomechanische factoren (knieënstand, laxiteit, gewrichtsbelasting). De relatie tussen die verschillende factoren en hun relatief belang is echter nog steeds onduidelijk. Het doel van dit project is dan ook om neuromusculaire, biomechanische en symptomatische risicofactoren voor progressie van knieartrose te identificeren en hun onderlinge relatie en relatief belang te bepalen in een 4 jaar durende prospectieve longitudinale studie. De uiteindelijke, finale uitkomst van dit project is het opstellen van geoptimaliseerde behandelstrategieën gericht op de belangrijkste en beïnvloedbare risicofactoren voor progressie van knieartrose. EN Osteoarthritis of the knee is a common chronic degenerative joint disease. Patients often complain about pain, muscle weakness, stiffness and instability, as well as limitations in physical activities. This may ultimately lead to a loss of independence and reduced quality of life. Prevention of knee osteoarthritis should therefore be one of the main objectives in health care. Knowledge of risk factors in the progression and/or the occurrence of osteoarthritis is therefore of paramount importance. In the literature, several risk factors for progression have already been described including factors of neuromuscular (eg muscle strength, muscle mass, muscle activation patterns), and biomechanical nature (knee alignment, laxity, joint impact). The relationship between these factors and their relative importance is still unclear. The aim of this project is therefore to identify neuromuscular, biomechanical (structural) and symptomatic risk factors for progression of knee osteoarthritis and to determine their mutual relationship and relative importance in a 4-year prospective longitudinal study. The ultimate outcome of this project is the development of optimized treatment strategies focused on the major and changeable risk factors for progression of knee osteoarthritis.

Imaging and biomolecular biomarkers in systemic sclerosis- a tissue-based approach

[email protected] 33.000 € FR Le Service de Rhumatologie des Cliniques Universitaires Saint-Luc a coordonné de nombreuses initiatives autour de la sclérose systémique (SSc), notamment la mise-en-route d’une cohorte de patients (BSSC ou Belgian Systemic Sclerosis Cohort). La SSc est une affection particulièrement grave puisque, dans les cas très sévères de l’affection, la mortalité atteint près de 50% à 10 ans. L’étiologie reste inconnue. Au sein de la famille des affections rhumatismales, même des maladies inflammatoires rhumatismales, la SSc est sans aucun doute l’affection qui se traite avec le plus de difficulté. Dans les formes sévères, l’inflammation suivie d’une cicatrisation (appelée également fibrose) peut concerner non seulement la peau, mais également différents organes comme les poumons, les reins et le tube digestif. Elle peut ainsi occasionner beaucoup de dommages irréversibles. Notre projet a pour but d’identifier des mécanismes importants de l’inflammation et de la fibrose, voire de nouvelles molécules qui pourront par la suite faire l’objet d’un traitement ciblé. L’imagerie « corps entier » réalisée chez des patients souffrant d’une forme diffuse de la maladie permettra de quantifier l’atteinte cutanée et du système musculo–squelettique et de valider des nouveaux biomarqueurs d’imagerie. Des biopsies cutanées seront réalisées et analysées en utilisant des techniques de laboratoire très performantes afin de démasquer les signatures moléculaires, c’est-à-dire les gènes et les protéines qui sont sur-exprimés ou sous-exprimés, de découvrir ainsi des nouvelles cibles thérapeutiques et de développer des traitements ciblés. Nous espérons que ce projet de recherche réalisé en collaboration avec la KUL générera de vraies découvertes. EN The rheumatology department of the Cliniques Universitaires Saint-Luc has coordinated several projects in systemic sclerosis (SSc) since the initiation of the Belgian Systemic Sclerosis Cohort (BSSC ou Belgian System Sclerosis Cohort) in 2006. SSc is by far the most demanding systemic rheumatic disease. The severe form of the disease is characterized by intense inflammation, followed by a scaring (so-called fibrotic) process, involving not only the skin and the subcutaneous tissue but also other major organs, such as the lungs, the oesophagus, the gut and the kidneys, thereby leading to organ failure in the worst case scenario. The cause of SSc is unknown and the mechanisms underlying inflammation and fibrosis are largely unraveled. Our project is aimed at finding new molecular pathways playing a pathogenic role which could then be specifically targeted. Total body imaging performed in patients with extensive disease will allow us to score skin and musculoskeletal involvement and to validate new imaging biomarkers. Deep skin biopsies will be performed and analyzed using very potent lab techniques to unmask molecular signatures, i. e. the genes and proteins that are either over-expressed or down-regulated. The relevance of these inflammatory and fibrotic pathways will be further tested in animal models and cell cultures. We expect that this joint venture between the KUL and the UCL will lead to true discoveries.

Er resten vele vragen omtrent het ontstaansmechanisme van deze ziekte die nog te vaak fataal verloopt. We zullen in dit onderzoek beeldvorming uitvoeren van de huid en de onderhuidse weefsels van patiënten in een vroeg ziektestadium. We gebruiken echografie en nucleaire magnetische resonantie en onderzoeken de correlaties tussen de afwijkingen op beeldvorming en de klinische bevindingen ter hoogte van de huid en de onderhuidse weefsels. We zullen gerichte biopsies uitvoeren van de betrokken regio’s en deze verder onderzoeken door middel van histopathologie, eiwit- en genexpressie analyse. Zo trachten we signaalwegen te identificeren die een afwijkende activiteit vertonen in systeemsclerosepatiënten. Voortgezet onderzoek in diermodellen kan deze mechanismen verder ontrafelen en de therapeutische mogelijkheden ervan onderzoeken. b) In het proces van verbindweefseling van de huid of van inwendige organen, dat kan optreden bij systeemsclerose, blijken de twee ontwikkelingsbiologische signaalwegen Wnt en BMP signaalweg een rol te spelen. In dit doctoraatswerk bestuderen we de BMP signaalweg in longfibrose bij muizen, en tonen aan dat deze signaalweg een beschermende rol speelt. We tonen verder aan dat natuurlijke antagonisten van de Wnt signaalweg in vitro het proces van verbindweefseling tegengaan. Met dit werk dragen we bij tot de groeiende evidentie voor een cruciale rol van deze signaalwegen bij het proces van verbindweefseling. EN a) Systemic sclerosis is a rare, serious systemic disease that affects small vessels and leads to fibrosis of the skin or internal organs such as the lungs. Many open questions remain, regarding the causative mechanisms of the disease, and effective targeted therapies are still lacking, resulting in substantial mortality. In this research project, we plan to investigate patients with early disease with different imaging modalities (ultrasound and magnetic resonance imaging). We will evaluate correlations between clinical findings in the skin and the subcutaneous tissues on one hand, and imaging abnormalities on the other hand. We will perform targeted biopsies of the affected sites and perform histopathologic , as well as protein and gene expression analysis, aiming to identify signaling pathways that are active in early disease. Continued research in animal models will explore in more detail the disease mechanisms and evaluate their therapeutic potential. b) Systemic sclerosis is a rare, serious systemic disease that affects small vessels and leads to fibrosis of the skin or internal organs such as the lungs. Many open questions remain, regarding the causative mechanisms of the disease, and effective targeted therapies are still lacking, resulting in substantial mortality. The BMP and the WNT signaling pathway are two developmental signaling pathways that are hypothesized to play a role in the process of

fibrosis. In this doctoral thesis, we studied the BMP pathway in the process of lung fibrosis in mice, and we have shown that this pathway has a protective effect. Furthermore, we showed that natural antagonists of the WNT signaling pathway counteract fibrosis in vitro. With this work, we further add strength to the growing evidence that stresses the central role of these pathways in the process of fibrosis. 5)

Er zijn op dit ogenblik geen therapieën voorhanden die het natuurlijk verloop van de ziekte kunnen afremmen. Wanneer in de toekomst echter effectieve behandelingen voorhanden zullen zijn, dan is het belangrijk te weten wie ernstige orgaancomplicaties zal ontwikkelen. Deze studie heeft als doel een predictief model op te bouwen (gebaseerd op klinische systeemsclerose gerelateerde kenmerken, nagelriemkarakteristieken en serologische merkers) dat vroegtijdig voorspelt welke patiënten ernstige orgaancomplicaties zullen ontwikkelen. EN The clinical expression and course of systemic sclerosis (SSc) may be coupled with serious morbidity and mortality. At this very moment no therapies are at hand that have proven through randomized controlled trials to halt the natural evolution of the disease or to efficaciously treat organ complications. If ever preventive therapy for organ involvement would be available it would be opportune to know which patients are prone to develop certain organ complications. Ideally treatment could be initiated timely in such a population. This study has the aim to develop a model (based on clinical SSc characteristics, nailfoldvideocapillaroscopic characteristics and serologic markers) which predicts which patient will develop severe organ involvement in SSc. 6)

NL Dit project heeft als doel een interventieprogramma te ontwikkelen voor de implementatie van intensieve combinatie behandelingsstrategieën bij beginnende reumatoïde artritis in de dagelijkse klinische praktijk. Het project bestaat uit drie fasen. Tijdens de eerste fase zullen twee studies worden uitgevoerd: een vragenlijstonderzoek om de relatieve belangrijkheid van de hinderpalen te onderzoeken en een literatuurstudie om mogelijke implementatiestrategieën te identificeren. In een tweede fase zal een symposium georganiseerd worden om de bewustwording te vergroten en om een werkgroep voor het ontwikkelingsproces te vormen. Tijdens het ontwikkelingsproces, de derde fase, zullen verschillende mogelijke strategieën vertaald worden in een leverbaar programma, wat zal leiden tot een stappenplan voor de implementatie en evaluatie om zorgverleners te ondersteunen en het voorschrijven van intensieve combinatie behandelingsstrategieën te vereenvoudigen. EN The objective of the project is to develop an intervention program for the implementation of intensive combination treatment strategies for early rheumatoid arthritis in daily clinical practice. The study project will be subdivided in three phases. In the first phase two studies will be conducted: a survey study to examine the relative importance of the barriers and a systematic review to identify possible implementation strategies. In the second phase, an invitational conference will be organized for increasing awareness and recruiting participants for the program development process. During the program development process, the third phase, different possible strategies will be translated into a deliverable program resulting in the actual design of an implementation and evaluation plan for supporting healthcare professionals and facilitating the prescription of intensive combination treatment strategies. 7)
Doctoraatsthesis: The role of the infrapatellar fat pad in knee osteoarthritis

[email protected] 3.500 € NL Artrose is een gewrichtsaandoening met een belangrijke persoonlijke, sociale en financiële impact op mensen. Er bestaan nog steeds geen geneesmiddelen die deze aandoening kunnen voorkomen of tegengaan. Het is daarom belangrijk om verder onderzoek te verrichten dat mogelijks tot nieuwe therapieën kan leiden. Deze thesis voorziet bewijs voor een rol van knievet in het ontstaan van artrose in de knie en tevens geeft het belangrijke aanwijzingen dat lipidenverlagende middelen zoals fibraten, ontsteking en destructie in een gewricht kunnen tegengaan. Op basis van bestaande literatuur stelden we de hypothese dat lipidenverlagende middelen niet alleen via hun gunstige invloeden op het gewricht zelf, maar ook door de preventie van systemische aandoeningen zoals aderverkalking, een verstoord lipidenprofiel of algemeen verhoogde ontsteking, artrose kunnen voorkomen of genezen. In een epidemiologische studie vonden wij alvast dat statinegebruikers een verminderde kans hadden op ontwikkeling en verergering van artrose. De mogelijkheid om lipidenverlagende middelen te gebruiken ter preventie of genezing van artrose lijkt erg beloftevol en dient verder te worden onderzocht.

EN Osteoarthritis is a joint disease with significant personal, social and financial impact on people. There are still no drugs available that prevent or stop the progression of this disease. It is therefore important to investigate the disease process of osteoarthritis and to find new targets for treatment. In this thesis, we demonstrate that fat tissue within the knee joint acts as an additional joint tissue contributing to the osteoarthritis disease process in the knee, and that it can be a target for drugs to treat patient with osteoarthritis. We also performed culture experiments with cells to show that certain lipid lowering drugs inhibit inflammatory and destructive processes in knee fat, but also other joint tissues. In addition, we performed the first study that demonstrates that people that are using lipid lowering drugs, are less likely to develop osteoarthritis of the knee. Based on these studies, this thesis indicates that lipid lowering drugs, such as statins, may prevent or slow down the progression of osteoarthritis. 8)

[email protected] 3.500 € Een goede medische controle van Reumatoïde Artritis betekent niet noodzakelijk dat de patiënt zich goed voelt. Dus de ‘objectieve’ beoordelingsparameters van de ziekte en behandeling komen dus niet noodzakelijk overeen met hoe de patiënt de ziekte en behandeling ervaart en hoe hij zich voelt. Daarom wordt de zorg voor patiënten met Reumatoïde Artritis (RA) best ook benaderd vanuit een niet-medische, psychosociale invalshoek, met aandacht voor onder meer emoties en omgang met de ziekte, het functioneren op het werk en binnen het gezin, de steun van familie en vrienden, etc. De ultieme vraag is wat bijdraagt tot het zich wel of niet goed voelen. Wij stellen daarom een kwalitatief onderzoeksproject voor waarbij patiënten met beginnende RA via individuele interviews de kans krijgen om in eigen woorden te vertellen wat zij verwachten van hun antireumatische behandeling en belangrijk vinden in het beginstadium van hun ziekte. We leggen de focus op het beginstadium, omdat wij vermoeden dat het vroegtijdig herkennen van signalen van ‘zich niet goed voelen’ kan leiden tot het voorkomen van psychosociale problemen op lange termijn. In eerste instantie zou dit onderzoek kunnen leiden tot een meer optimale keuze van vragenlijsten voor de dagelijkse klinische praktijk en bij het beoordelen van toekomstige therapeutische interventies . Het ultieme doel is het verhogen van de patiëntgerichte aanpak, met aandacht voor de even belangrijke niet-medische invalshoek in de behandeling voor patiënten met beginnende RA.

[email protected] 3.500 € NL Tot op heden is het gebruik van oefentherapie bij spondylarthropathie patiënten nog maar weinig wetenschappelijk onderbouwd. In relatie tot medicatiegebruik is er nog minder evidence based literatuur te vinden. Met deze studie willen we dus op een wetenschappelijke manier aantonen dat oefentherapie bij deze patiënten, zoals algemeen aangenomen, maar onvoldoende evidence based, nuttig en efficiënt is. Doel van de studie is nagaan of het consequent uitvoeren van een thuisoefenprogramma, waarin zowel oefeningen ter verbetering van lenigheid en kracht zijn opgenomen alsook oefeningen ter verbetering van cardiovasculaire parameters en uithouding, een invloed heeft op een aantal ziekte- en fysieke parameters, zoals o.a. lenigheid, medicatiegebruik en levenskwaliteit van spondylarthropathie patiënten die behandeld worden met een biological. Tevens zal worden nagegaan hoe de ‘compliance’ is van de patiënten bij het zelfstandig uitvoeren van een thuisoefenprogramma aan de hand vaneen dagboek dat door de patiënten wordt bijgehouden. EN Thus far, the use of exercise therapy for spondyloarthritis patients is only limited based on scientific evidence. In relation to the use of medication this evidence is even poorer. The aim of this study is to deliver scientific prove for the general accepted benefit and efficiency of a home exercise program for these patients. This study’s objective is to document that participating consistently in a home exercise program, including strength and flexibility exercises as well as cardiovascular and endurance exercises, influences disease and physical parameters such as flexibility, medication use and quality of life of spondyloarthritis patients, when treated with a biological. The compliance of performing the home exercises will be documented by the use of a patient diary.

[email protected] 100.000 € FR La maladie d’Alzheimer (MA) est la démence sénile de loin la plus fréquente. Elle est caractérisée par un ensemble de symptômes associés au développement de deux lésions caractéristiques dans le cerveau des patients: les plaques séniles et les dégénérescences neurofibrillaires. Les plaques séniles se forment par l’accumulation d’un petit peptide, le peptide -amyloïde ou A, issu du clivage de son précurseur, l’APP. Les formes héréditaires de la maladie sont notamment liées à de mutations dans le gène de l’APP. Nous analysons les relations existant entre la structure, le métabolisme et la fonction de l’APP. Nos travaux récents ont identifié des motifs dans l’APP contrôlant sa structure et régulant la production d’ A. Ces mêmes motifs permettent son association avec des partenaires impliqués dans la maladie. Ceci offre des perspectives intéressantes pour le développement futur d’approches thérapeutiques visant à bloquer l’apparition et la progression de la maladie d’Alzheimer. EN Alzheimer’s disease (AD) represents the most frequent form of dementia. It is characterized by clinical symptoms associated to the progressive development of two typical lesions in the patient’s brain: the senile plaques and the neurofibrillary tangles. Senile plaques are formed by the accumulation and deposition of a small peptide, the amyloid peptide or A produced by the cleavage of its precursor called APP. Inherited forms of the disease are linked in particular to mutations in APP gene. We study the relationships existing between APP structure, processing and function. Our recent works identified motifs present in APP that control its structure and regulate Aproduction. The same motifs allow its association to partner proteins involved in AD. This provides interesting targets for the development of therapeutic approaches aiming at blocking the onset and progression of the disease.

NL De Neuromotorische Revalidatie Onderzoeksgroep houdt zich bezig met wetenschappelijk onderzoek naar de verbetering van revalidatiemogelijkheden bij mensen met neurologische aandoeningen zoals de ziekte van Parkinson. Het bekroonde project streefde naar een optimale revalidatiestrategie voor de behandeling van problemen tijdens het stappen en tijdens belangrijke bewegingen van de bovenste ledematen (zoals schrijven) in een subgroep van patiënten met ‘freezing of gait’. Freezing of gait (FOG), of het bevriezen van de gang, is één van de meest invaliderende bewegingsstoornissen bij de ziekte van Parkinson. FOG is het kortstondig en onvrijwillig stilvallen van het stappen alsof patiënten met hun voeten aan de grond blijven plakken. De behandeling van FOG is moeilijk en complex omdat FOG gepaard gaat met algemene problemen in bewegingscontrole en cognitie. Daarom voerden we een aantal gedragsmatige en medische beeldvormingsstudies uit om beter te begrijpen hoe de uitvoering en revalidatie van functionele bewegingstaken in patiënten met FOG afhangt van hun specifiek motorischcognitief profiel. We testten het effect van nieuwe behandelingsmethoden die gebruik maken van sensorische cues (vb. auditieve tonen) en aandachtsstrategieën. De resultaten van het onderzoek droegen bij tot de ontwikkeling van nieuwe revalidatiemethoden ter verbetering van gang- en evenwichtscontrole, bewegingen in de bovenste ledematen en cognitieve vaardigheden in patiënten met freezing of gait. EN The Neuromotor Rehabilitation Research Group aims to increase the rehabilitation potential of people with neurological disorders such as Parkinson’s disease. The awarded project was specifically designed to improve difficulties in walking and functional upper limb movements (e.g. writing) in clinical subgroups of Parkinson patients with and without freezing of gait (FOG). FOG is an incapacitating gait disorder inducing a sudden inability to start or continue walking, as if patients’ feet are glued to the floor. FOG is difficult to manage therapeutically, in part because it is associated with complex disturbances in motor and cognitive control. Therefore, we conducted a series of behavioral and neuroimaging studies to investigate how the performance and rehabilitation potential of functional motor tasks depends on patients’ precise motor-cognitive profile and tested the efficacy of novel rehabilitation methods based on sensory and attentional strategies to address these specific problems. FR Etude pilote de stimulation magnétique transcranienne répétitive (rTMS) des régions frontales impliquées dans le contrôle de la douleur comme traitement de l’algie vasculaire de la face (ou cluster headache) épisodique ou chronique. L’algie vasculaire de la face (ou cluster headache) est une céphalée plus rare que la migraine, affectant surtout les hommes. Cette maladie peut avoir des conséquences dévastatrices sur le patient en raison de l’intensité de la douleur (on la surnomme ‘céphalée-suicide’), des effets secondaires liés aux traitements médicamenteux, et de la résistance de certaines formes à tous les traitements existants. Des études récentes suggèrent que l’algie vasculaire de la face pourrait être associée à une diminution de l’activité de régions du cerveau impliquées dans le contrôle de la douleur, plus particulièrement les zones profondes du cortex frontal. Le but de notre étude pilote est d’activer le fonctionnement de ces zones profondes au moyen d’une technique de neurostimulation non invasive appelée stimulation magnétique transcrânienne, chez les patients atteints d’algie vasculaire de la face, et d’évaluer l’effet thérapeutique de cette stimulation dans différentes formes de la maladie. EN Cluster headache is a primary headache that chiefly affects young men, and is less common than migraine. This disease can have devastating consequences due to the pain intensity (it is also called ‘suicide headache’), to the side effects of the drug preventive therapies, and to the resistance of some subtypes of the headache to all existing medications. Recent studies suggest that cluster headache could be associated with a decrease of the activity of some pain-controlling brain regions, especially deep frontal areas. The aim of our pilot study is to activate these areas using a non-invasive neurostimulation technique called transcranial magnetic stimulation in cluster headache sufferers, and to evaluate the therapeutic effect of this stimulation in various subtypes of the disease.

[email protected] 25.000 € NL In een reeks van 10 originele studies hebben wij een nieuwe meetmethode ontwikkeld en gevalideerd om de vermoeibaarheid van de spieren te evalueren als een directe en objectieve parameter voor het vermoeidheidssymptoom bij kwetsbare oudere mensen. De test meet de tijd waarbinnen de handknijpkracht afneemt tot 50% van de maximale waarde tijdens een aangehouden maximale inspanning. Deze parameter hebben wij gedefinieerd als ‘Fatigue Resistance’ (=spiervermoeibaarheid). Eerder bestond er nog geen meetinstrument om de vermoeibaarheid van de spieren op een directe wijze evalueren bij kwetsbare en zieke oudere patiënten. De nieuwe meetmethode die wij ontwikkeld hebben is robuust en betrouwbaar, en kan ingezet worden om zelfs erg zieke en bedlegerige oudere patiënten te evalueren.

Vervolgens hebben wij de spiervermoeibaarheidstest verder verfijnd en hebben wij een nieuwe parameter ontwikkeld die wij gedefinieerd hebben als ‘Grip Work’ (=spierarbeid). Grip Work integreert de maximale handknijpkracht met de spiervermoeibaarheid en weerspiegelt de fysiologische arbeid die verricht wordt door de voorarmspieren tijdens de spiervermoeibaarheidstest. Zowel Grip Work als Fatigue Resistance hangen nauw samen met de klinische toestand van kwetsbare oudere patiënten en deze parameters laten toe om de ‘fysieke vermoeidheid’ te onderscheiden van de ‘psychologische vermoeidheid’ (welke gemeten wordt met vragenlijsten). Op basis van deze informatie kan de arts een gerichtere behandeling voorschrijven bij oudere patiënten die klagen over vermoeidheid. Tot slot hebben wij de dramatische impact van complexe ontstekingsprocessen op de spiervermoeibaarheid bij oudere personen in kaart gebracht, welke de rationale heeft gevormd voor het opzetten van interventiestudies met nieuwe ontstekingsremmende behandelingen om kwetsbaarheid te bestrijden en de kwaliteit van leven op hogere leeftijd te verbeteren. EN In a series of 10 original studies we have introduced, refined and validated a new assessment method for muscle fatigue resistance as a direct and objective outcome parameter of the exhaustion component of Frailty in elderly persons. The test measures the time during which grip strength decreases to 50% of its maximal value during a sustained maximal contraction. We have defined this new parameter as 'Fatigue Resistance'. Before, there was no direct instrument available to evaluate muscle fatigue in frail and ill elderly patients. The new instrument that we have developed is robust, reliable and feasible even when assessing very ill and bedridden elderly patients. Next, the fatigue resistance test has been further refined and we have created a new parameter which we defined as 'Grip Work'. Grip work can be obtained by integrating maximal grip strength with fatigue resistance, and reflects the physiological work delivered by the forearm muscles during the fatigue resistance test. Both grip work and fatigue resistance are closely related to the clinical condition of frail elderly patients and allow to distinguish 'physical fatigue' from 'psychological fatigue' (as assessed by self-perceived fatigue questionnaires). Based on this information, clinicians can prescribe more appropriate treatments when dealing with elderly patients complaining from fatigue. Finally, we portrayed the dramatic impact of complex inflammatory processes on muscle fatigue resistance in elderly persons, providing the rationale for new inflammation-reducing intervention studies to counter Frailty and improve quality of life at higher age.

NL Wereldwijd leven 35 miljoen mensen met dementie. Dit aantal zal naar schatting toenemen tot meer dan 115 miljoen tegen 2050. Deze tsunami van dementie vormt daarom één van de grote uitdagingen van de 21ste eeuw. Ouder worden speelt een centrale rol bij dementie, maar er zijn ook andere risicofactoren zoals levensstijl en erfelijkheid. In dit onderzoek bestuderen we de erfelijkheid van jongdementie. Men spreekt van jongdementie wanneer patiënten de eerste symptomen van dementie ontwikkelen voor de leeftijd van 65 jaar. Omdat deze patiënten op zo een jonge leeftijd getroffen worden door wat eigenlijk een ouderdomsziekte is, kan verwacht worden dat ze drager zijn van een ernstig genetisch defect. In deze patiënten hebben we daarom een grotere kans om nieuwe fouten in sleutelgenen en sleuteleiwitten van het dementieproces te ontdekken. Bovendien verwachten we dat de ziekteprocessen bij jongdementie ook een centrale rol zullen spelen bij de veel meer voorkomende ouderdomsdementie. Op die manier gebruiken we het erfelijkheidsonderzoek van jongdementie als toegangspoort om de ziektebiologie van dementie beter te begrijpen en zo nieuwe aangrijpingspunten te vinden voor therapie. EN Worldwide over 35 million people are affected by dementia. This number is projected to increase to over 115 million by 2050. As such, the dementia epidemic is representing one of the major challenges of the 21st century. Dementia is primarily a disease of aging, yet we know that other risk factors are involved, such as life style and genetics. With this research project we aim to study the heritability and genetics of early-onset dementia. Early-onset dementia patients develop first symptoms of dementia before the age of 65 years. Because these patients develop an aging disease at such a young age, they are expected to carry a serious genetic defect. In these patients we therefore have a bigger chance to detect novel defects in key genes and key proteins in the dementia process. Moreover it can be expected that the disease processes in early-onset dementia are also central in the much more common forms of late-onset dementia. As such, we use genetic research of early-onset dementia as an entry point in to increase our understanding of the underlying disease biology of dementia and find new targets for therapy.

Ces MCPs peuvent en effet se différencier en cellules musculaires contractiles cardiaques (cardiomyocytes), en cellules endothéliales et en cellules

musculaires lisses. Jusqu’il y a peu, les mécanismes moléculaires contrôlant le développement de ces MCPs restaient mal compris. Par des techniques de culture cellulaire et de biologie moléculaire, nous avons pu récemment mettre en évidence le rôle clé d’un facteur de transcription appelé ‘Mesp1’ dans la spécification de ces progéniteurs cardiovasculaires, initiant le développement cardiovasculaire, et induisant une différenciation cardiaque, endothéliale et musculaire lisse des cellules. D’importantes questions restent cependant non résolues à l’heure actuelle : comment le réseau de gènes s’organise-t-il en aval de Mesp1? Peut-on identifier des gènes ou groupes de gènes capables de promouvoir spécifiquement la différenciation des cellules vers les lignées cardiaque, endothéliale et musculaire lisse? Dans ce projet, partant d’une analyse bioinformatique, nous allons identifier quels gène ou groupe de gènes agissent en aval de Mesp1 pour contrôler la spécification, la différenciation et ou la migration des MCPs. Ces résultats permettront d’approfondir la connaissance des étapes précoces du développement cardiaque, mais aussi d’identifier de nouvelles stratégies pour produire des cellules cardiovasculaires à grande échelle, potentiellement utilisables à des fins thérapeutiques, pour des études pharmacologiques, ou pour modéliser des maladies cardiovasculaires. EN During embryonic development, the different cell types that constitute the heart arise from the differentiation of specialized cells called ‘multipotent cardiovascular progenitors’ (MCPs). These progenitors can generate all type of cells contributing to cardiac development, such as the contractile cardiomyocytes, endothelial and smooth muscle cells. So far the molecular mechanisms controlling MCP specification and differentiation in the different cardiovascular cell lineages remained poorly understood. Using embryonic stem cells (ESCs), we recently showed that the early mesodermal transcription factor Mesp1 is a key regulator of MCP specification, acting at the initial step of cardiovascular development and promoting the differentiation of ESCs toward cardiac, endothelial and smooth muscle lineages. A key question is how the cardiovascular gene network is organized beyond Mesp1. In particular, can we identify genes or groups of genes that are able to specifically promote the differentiation of the cells toward cardiac, endothelial, or smooth muscle fates? In this project, starting from a bioinformatical analysis, we will identify genes or groups of genes that act downstream of Mesp1 to drive MCP specification, migration, and their differentiation into specific cell lineages. These results should provide better insights into the early step of cardiovascular development, as well as new ways to produce cardiovascular cells, that could be used for drug screening, cellular therapy or to model human cardiovascular diseases.

FR Dans le laboratoire d’Anabelle Decottignies, je m’intéresse aux différents mécanismes que les cellules tumorales développent pour acquérir et maintenir leur activité de prolifération, leur permettant ainsi d’envahir l’organisme et de former des métastases. De façon plus précise, je m’intéresse aux mécanismes que les cellules tumorales mettent en place pour maintenir leurs extrémités chromosomiques –qu’on appelle les télomères- au cours des divisions cellulaires. La capacité de maintenir ses télomères est une condition sine qua non pour la prolifération cellulaire indéfinie et cibler ces mécanismes apparaît donc comme un moyen prometteur de lutte contre le cancer. Le laboratoire d’accueil s’intéresse plus particulièrement à un mode de maintien des télomères qui est indépendant de l’activité de la télomérase mais fait appel à des événements de recombinaison homologue. Ce mécanisme alternatif de maintien des télomères, appelé ALT, est fréquemment détecté dans les sarcomes et les glioblastomes, deux types de cancer très fréquents chez les enfants. Parvenir à une meilleure compréhension du mécanisme ALT présente dès lors un intérêt thérapeutique potentiel dans le cadre du développement de traitements anti-ALT ou d’outils de détection des cancers ALT. Le but de mon séjour post-doctoral est donc d’identifier des facteurs essentiels à l’activation de ce mode alternatif de maintien des télomères dans les cellules cancéreuses. EN In the laboratory of Anabelle Decottignies, I am currently studying the different mechanisms that tumor cells develop to obtain and maintain their proliferative activity, allowing them to invade the body and to form metastases. More specifically, I am interested in understanding the mechanisms that tumor cells set up to maintain their chromosome extremities, the socalled telomeres, across cell divisions. Telomere maintenance is a sine qua non condition for indefinite cellular proliferation and targeting these mechanisms has thus become a promising target for cancer therapy. The host laboratory is mostly interested in a telomere maintenance mechanism that is independent from telomerase activity but relies on homologous recombination-dependent events. This alternative mechanism of telomere maintenance, dubbed ALT, is frequently detected in sarcomas and glioblastomas, two cancer types highly prevalent in children. Gaining a more detailed understanding of ALT mechanism to enable the development of ALT-targeted treatments or the early detection of ALT-positive cancers may therefore be of particular therapeutic value. Thus the goal of my fellowship project is to identify factors/events that are essential for the activation and the maintenance of this mechanism in cancer cells.

[email protected] 360.000 € FR Cette recherche pluri- et transdisciplinaire menée conjointement et en parallèle par l’ULB et la VUB comporte deux volets visant à : (Partie I) établir l’inventaire des difficultés scolaires et psychosociales chez les rescapés d’une tumeur cérébrale en âge enfant et évaluer les facteurs qui influencent la mise en place d’une prise en charge basée sur la pratique des difficultés d’apprentissage, et (Partie II) identifier les moments clés tout au long du trajet des soins après l’intervention neurochirurgicale et analyser de façon qualitative comment les patients et leurs parents vivent les décisions liées aux soins de santé, plus particulièrement en ciblant les interactions entre les patients et leurs parents d’une part et les soignants de l’autre. Les résultats de la présente étude pilote menée à l’UZ Brussel de la VUB et à l’Hôpital Erasme de l’ULB pourront être vérifiés par la suite dans un contexte multicentrique plus large qui pourrait constituer le point de départ d’une étude sur le devenir des enfants rescapés d’une tumeur cérébrale en âge enfant. La finalité de la présente étude est de pouvoir formuler des recommandations pour faciliter le dépistage précoce des difficultés scolaires et psychosociales et, de ce fait, optimiser l’accompagnement de ces enfants tout au long de leur trajet des soins. NL Dit multi- en transdisciplinair onderzoeksproject bestaat uit twee luiken die met elkaar verband houden en in parallel zullen worden uitgevoerd door onderzoekers van ULB en VUB, met als doel : (deel I) het opstellen van a) een inventaris van leermoeilijkheden en psychosociale problemen bij mensen die een hersentumor overleefden als kind ( CBTs ), en van b) de factoren die de implementatie van maatregelen voor de aanpak van leermoeilijkheden die aangewezen zijn op basis van de huidige praktijk beïnvloeden, (deel II) de cruciale gebeurtenissen in het zorgtraject dat volgt op een heelkundige resectie van een hersentumor te identificeren, en op een kwalitatieve manier te analyseren hoe patiënten en ouders zorggerelateerde beslissingen ervaren, met speciale aandacht voor de interacties tussen patiënten, ouders en zorgverleners. De bevindingen van dit pilootproject uitgevoerd bij CBTs behandeld in het UZ Brussel - VUB en het Erasmus Ziekenhuis - ULB kunnen nadien geverifieerd worden in een bredere, multi-universitaire context, en kunnen het startpunt vormen van een outcome-studie. Het onderzoeksproject beoogt het opstellen van aanbevelingen voor vroegtijdige screening van leermoeilijkheden en aanverwante psychosociale problemen met oog op het optimaliseren van het opvoedkundig en psychosociaal zorgtraject bij CBTs.

EN This multi- and transdisciplinary research project jointly led by ULB and VUB consists of two interrelated parts which will be carried out in parallel, with the following aims: (Part I) to make an inventory of learning difficulties and related environmental problems in childhood brain tumor survivors (CBTS), and to assess the factors which influence the implementation of a potential management package based on the existing practice for learning difficulties, and (Part II) to identify key events in the care trajectory following surgery for childhood brain tumor, and to qualitatively analyze how patients and parents experience care-related decisions, with special attention directed to the interactions between patients and parents, and care professionals. The findings of this pilot project in CBTS treated at VUB-UZ Brussel and ULB-Hôpital Erasme might later on be verified in a larger, multi-university context and, so, constitute a starting point for an outcome study. [email protected] 20.000 € NL Het afweersysteem of immuunsysteem kan ons niet enkel beschermen tegen bacteriën en virussen, maar speelt ook een rol in de verdediging tegen kanker. Deze kennis is de drijvende kracht achter de alsmaar groeiende interesse van wetenschappers om het immuunsysteem in te zetten voor de behandeling van kanker in de zogenaamde immuuntherapie. Een aantrekkelijke piste die momenteel intensief verkend wordt, is het gebruik van dendritische cellen. Deze cellen kan men beschouwen als de bevelhebbers van het immuunsysteem: zij geven instructies aan andere cellen van het afweersysteem en kunnen deze aanzetten tot een gerichte aanval van de kankercellen. Onze onderzoeksgroep heeft een uitgebreide ervaring in het gebruik van dendritische cellen, waarbij deze als een soort vaccin toegediend worden aan patiënten met een agressieve vorm van bloedkanker of leukemie. De eerste resultaten met dit vaccin zijn erg veelbelovend: in een deel van de patiënten slaagden we erin herval van de leukemie te voorkomen of uit te stellen. In dit project zullen we nu de volgende stap zetten in de ontwikkeling van deze nieuwe kankerbehandeling door meer patiënten te vaccineren en de resultaten te vergelijken met die van een controlegroep.

EN The immune system can not only protect us against bacteria and viruses, it also plays a role in the defense against cancer. This knowledge is the driving force behind the ever growing interest of scientists to use the immune system for the treatment of cancer in so-called immunotherapy. An attractive strategy which is currently extensively being explored, is the use of dendritic cells. These cells can be seen as the commanders of the immune system : they give instructions to other cells of the immune system and can instigate a targeted attack of the cancer cells. Our research group has extensive experience in the use of dendritic cells by administering them as a type of vaccine to patients with an aggressive form of blood cancer or leukemia. The first results of this vaccine are very promising: in some of the patients we managed to prevent or postpone relapse of leukemia. [email protected] 20.000 € NL Juveniele myelomonocytaire leukemie (JMML) is een zeldzame kwaadaardige aandoening van het beenmerg die voorkomt op kinderleeftijd. De enige behandelingsoptie waarmee momenteel een volledige genezing bekomen kan worden, is een stamceltransplantatie. Onderzoek naar de ontstaansmechanismen en de kenmerkende eigenschappen van deze leukemie heeft ondermeer als doel andere geneesmiddelen te identificeren waarmee genezing bekomen kan worden met minder toxiciteit voor de patiënt. In dit onderzoek worden RNA moleculen (meerbepaald ‘non-coding’ RNA, d.i. RNA dat niet wordt omgezet in eiwit) in JMML cellen van patiënten opgespoord. De interacties tussen deze RNA moleculen onderling en met andere onderdelen van de cel zullen in kaart gebracht worden, evenals de link met reeds gekende ontstaansmechanismen van JMML. We verwachten dat sommige ‘non-coding’ RNA moleculen gebruikt zullen kunnen worden in de diagnostiek van JMML, als prognostische merker en als aangrijpingspunt voor nieuwe behandelingen. EN Juvenile myelomonocytic leukemia (JMML) is a rare malignant bone marrow disorder affecting young children. The only available treatment option to cure these patients is a hematopoietic stem cell transplantation. Research into the cause and the development of JMML, and into the characteristics of this leukemia will lead to the identification of targets for new, less toxic, drugs that ultimately might cure the patients. In this research project, RNA molecules (‘non-coding’ RNA, i.e. RNA which will not be translated to proteins) will be identified and measured in JMML cells of patients. The mutual interactions between these RNA molecules and their interactions with other components of the cell will be elucidated and linked to known mechanisms in JMML. We expect that some of the ‘non-coding’ RNA molecules will be useful as diagnostic or prognostic marker, or can serve as potential target for new drugs.

Dit researchproject spitst zich toe op de functie van het retinaal pigmentepitheel (RPE), de diepste laag van het netvlies, door de studie van ziekten veroorzaakt door de slecht functioneren van deze laag. Het RPE is essentieel voor het normaal functioneren van het netvlies, maar ook voor de embryologische ontwikkeling ervan. Meer specifiek hebben we ziekten bestudeerd die veroorzaakt worden door mutaties in het BEST1 gen. Enkel de RPE cellen gebruiken dit gen, om het eiwit ‘bestrophine’ aan te maken. In ons researchproject hebben we aangetoond dat verschillende types mutaties in het BEST1 gen verschillende blindmakende netvliesziekten veroorzaken die we samen de ‘bestrophinopathieën’ genoemd hebben. Meer specifiek hebben we de oorzaak van autosomaal dominante vitreoretinochoroidopathie (ADVIRC) gevonden, en hebben we een nieuwe aandoening beschreven die we autosomaal recessieve bestrophinopathie (ARB) hebben genoemd. Voornamelijk de identificatie van deze laatste aandoening was van groot belang, aangezien zeer recent werd aangetoond dat corrigerende gentherapie in een hondenmodel voor deze aandoening leidt tot genezing. De prijs zal gebruikt worden om de kennis van deze groep van aandoeningen verder uit te breiden. FR La rétine est le film sensible à la lumière, localisé à l'intérieur au fond de l'oeil. Ce projet de recherche se focalise sur la fonction de l'épithélium pigmentaire rétinien (EPR), la couche la plus profonde de la rétine, par l'étude des maladies dues au malfonctionnement de cette couche. L'EPR est essentiel pour le bon fonctionnement de la rétine pendant la vie, ainsi que pour son développement embryologique. Plus spécifiquement, nous avons étudié des maladies dues à des mutations dans le gène BEST1. Seulement les cellules de l'EPR utilisent ce gène, afin de produire une protéine appelée ‘bestrophine’. Dans notre projet de recherche, nous avons démontré que différents types de mutations dans le gène BEST1 sont responsables pour des maladies diverses qui rendent les patients aveugles, et que nous avons collectivement nommées les ‘bestrophinopathies’. Plus spécifiquement, nous avons élucidé la cause de la vitréorétinochoroidopathie autosomique dominante (VIRCAD). Nous avons également décrit une nouvelle condition appelée la bestrophinopathie autosomique récessive (BAR). Surtout l'identification de cette dernière est d’une grande importance: très récemment une thérapie génique correctrice s’est révélee efficace dans un modèle canin de cette condition. Le prix contribuera à la continuation de nos études dans ce groupe de conditions.

EN The retina is the light sensitive film located at the inner side of the back of the eye. This research project is aimed at improving our understanding of the function of the retinal pigment epithelium (RPE), the deepest layer of the retina, by studying the diseases caused by malfunctioning of that layer. The RPE is essential for proper functioning of the retina during life, but also for its embryological development. More specifically, we have studied diseases due to mutations in the BEST1 gene. Only the RPE cells use this gene, to produce a protein called ‘bestrophin’. In our research project, we have shown that different types of mutations in the BEST1 gene lead to different blinding diseases we have collectively called ‘bestrophinopathies’. More specifically, we have elucidated the cause of autosomal dominant vitreoretinochoroidopathy (ADVIRC), and we have described a new condition called autosomal recessive bestrophinopathy (ARB). The identification of this latter condition was especially important, as in a dog model for this disease it has very recently been proven that it is amenable to corrective gene therapy. The prize will be used to further our understanding of this group of diseases.

Technologische ontwikkelingen hebben het in toenemende mate mogelijk gemaakt om op grote schaal DNA sequenties te bepalen, maar tot dusverre waren deze methodes niet geschikt voor het kosteneffectief analyseren van individuele genen in grote groepen personen. De identificatie van zoveel mogelijk personen met RDH12 mutaties is belangrijk om bewustwording te bewerkstelligen voor dit type netvliesziekte, vanwege de noodzakelijke investeringen voor het ontwikkelen van gentherapie, en om het aantal personen dat in aanmerking zou kunnen komen voor een dergelijke behandeling te vergroten. In dit project stellen wij voor om 4.000 personen met aangeboren blindheid of retinitis pigmentosa afkomstig uit Canada, de VS, en Europa, te testen op het voorkomen van mutaties in RDH12 en 104 andere genen betrokken bij erfelijke netvliesziekten. Hierbij maken wij gebruik van een kosteneffectieve methode. RDH12 varianten werden eerder bij 3% van personen met aangeboren blindheid en bij 1.5% van personen met retinitis pigmentosa gevonden. Wij verwachten dat deze studie 60 nieuwe families met RDH12 mutaties zal opleveren. Deze families kunnen daarna worden opgenomen in de RDH12 stichtingen van België en de VS.

EN In the last two decades, approximately 80% of the genes underlying inherited retinal degeneration have been identified. Technological developments have increasingly enabled large-scale mutation analyses, but thus far, they have not been designed for the cost-effective analysis of individual genes in large groups of individuals. The identification of as many individuals with RDH12 mutations as possible is important to create awareness of the importance of this retinal disease subtype in view of necessary investments to design genetherapy, and to increase the pool of individuals that are eligible for novel treatments. In this project, we propose to systematically test 4.000 cases with congenital blindness or retinitis pigmentosa from Canada, the US, and Europe for mutations in RDH12 and 104 other inherited retinal disease-associated genes, using a cost-effective high-throughput method. RDH12 variants have previously been found in 3% of persons with congenital blindness and 1.5% of persons with retinitis pigmentosa. We expect that this study will reveal 60 additional families with RDH12 mutations, which can then connect to the RDH12 foundations in Belgium and the US.

Door blootstelling aan een hoge dosis allergeen kan de ontstekingsreactie dusdanige proporties aannemen dat deze aanleiding geeft tot de welgekende astma-aanvallen of exacerbaties. Naast allergeenblootstelling op zich kunnen virale luchtweginfecties met bv. het respiratoir syncytiaal virus (RSV) eveneens optreden als katalysator van de longontsteking en bijgevolg resulteren in een astma-aanval. Alveolaire macrofagen zijn gespecialiseerd in het verwijderen en vernietigen van virussen die de longen binnendringen. Ons recent onderzoek heeft aangetoond dat de antivirale werking van deze cellen sterk is afgenomen na een astmatische fase. In dit project beogen we de antivirale functies van de ‘astmatische’ alveolaire macrofagen te herstellen om zo RSV-geïnduceerde astma-exacerbaties beter te controleren en in te perken. EN Allergic asthma is a lung disease which is characterized by a chronic airway inflammation. When the lungs are highly exposed to the allergen, the bronchial inflammation can elicit an exacerbation reaction or ‘asthmatic attack’. In addition to allergen exposure, viral airway infections with for instance the respiratory syncytial virus (RSV) can also act as a catalyst of the lung inflammation, eventually leading to these exacerbation reactions. Alveolar macrophages are highly specialized in the efficient removal of airborne viruses. Our recent

research showed that the antiviral actions of this cell population are strongly impaired as a result of the preceding asthmatic attack. In this project, we want to modulate the ‘asthmatic’ alveolar macrophage function in order to restore its antiviral functions and hence to better control and limit RSV-induced asthma exacerbations.

[email protected] 20.000 € NL Beweging geeft aanleiding tot een sterke verbetering van suikercontrole in type 2 diabetes (T2D). Echter heeft men tot op heden enkel getracht aan symptoomcontrole van T2D te doen door middel van bewegingstherapie, namelijk het verlagen van de bloedsuikerspiegel. In het EXTIDI (EXercise TImining in DIabetes) project gaat men trachten één van de belangrijkste oorzaken van T2D, insulineresistentie, positief te beïnvloeden door middel van bewegingstherapie. Recente studies tonen aan dat vetzuurstapeling in de skeletspier aanleiding geeft tot insulineresistentie. Deze vetzuurstapeling komt vaak voor in personen met T2D. Door nuchter te sporten zal men vooral die gestapelde vetzuren verbranden en de vetverbrandingscapaciteit sterk verbeteren. Dit resulteert, alleszins in jongvolwassenen, tot een veel sterkere toename van insulinegevoeligheid on vergelijking met sporten in nietnuchtere toestand (waar vooral suikers en vetten uit de bloedbaan verbrand worden). In het EXTIDI project wordt onderzocht of sporten in nuchtere toestand aanleiding geeft tot sterkere verbetering van suikercontrole in T2D patiënten in vergelijking met sporten in nietnuchtere toestand. EN Exercise significantly improves glycemic control in patients with type 2 diabetes (T2D). However, until present mainly symptomatic control of T2D through exercise training has been aimed at, namely the reduction in blood glucose levels. In the EXTIDI (EXercise TIming in DIabetes) project it is the aim to positively affect one of the most important causes for T2D: insulin resistance. Recent studies indicate that fatty acid accumulation in the skeletal muscle cell leads to insulin resistance. Such fatty acid accumulation is often present in patients with T2D. By exercising in fasting condition these fatty acids will be oxidized and the fatty acid oxidation capacity increases significantly. This results, at least in young adults, to a much stronger improvement in insulin sensitivity as opposed to exercise training in fed condition (in which mainly blood glucose and fatty acids are oxidized). In the EXTIDI project it will be explored whether exercise training in fasting condition would lead to a stronger improvement in glycemic control as opposed to exercise training in fed condition in T2D patients.

[email protected] 80.000 € FR Depuis 1994, l’administration d’une prophylaxie antirétrovirale aux femmes enceintes infectées par le VIH a considérablement réduit le risque de transmission verticale, qui est actuellement inférieur à 1%. Des études récentes issues de pays en développement mais aussi des observations faites dans notre hôpital ont mis en évidence que ces enfants exposés au VIH mais non infectés étaient plus à risque de présenter des infections sévères pendant la première année de vie ainsi que des retards de développement et d’autres problèmes neurologiques. Les mécanismes impliqués dans ces complications ne sont pas élucidés mais pourraient inclure des altérations immunologiques et la toxicité des médicaments antirétroviraux durant la vie fœtale. Nous avons mis sur pied une cohorte de naissance incluant 120 enfants exposés au VIH et non infectés et 120 enfants contrôles qui seront suivis pendant les 3 premières années de vie. Le suivi comprend l’évaluation clinique des enfants et l’étude des événements infectieux, des réponses immunes innées et adaptatives et du développement neurologique.

EN Since 1994, the administration of antiretroviral prophylaxis to pregnant HIV-infected women has dramatically reduced mother to child transmission of HIV, which is now less than 1%. Recent studies conducted in HIV-exposed uninfected children in developing countries as well as in our hospital have demonstrated that HIV-exposed uninfected children are at higher risk of severe infections in early life. Moreover, these children seem to present an increased risk of neurological development delay or abnormalities. The mechanisms underlying these alterations could involve the immune system and the toxicity of antiretroviral exposure during fetal life. We have set up a birth cohort of 120 HIV-exposed infants and 120 controls who are followed-up during the first 3 years of life. De huidige surveillantie van de HIV epidemie in België laat niet toe om een recente HIV infectie van reeds langer bestaande HIV infecties te onderscheiden. Nochtans is dit essentiële informatie om een efficiënte bestrijding van de HIV epidemie toe te laten. Dit project poogt met behulp van verschillende testen een algoritme te valideren dat toelaat het tijdstip van de infectie met een nauwkeurigheid van enkele maanden te kunnen vaststellen. Deze informatie kan op zijn beurt gelinkt worden aan de genetische informatie van het HIV virus om transmissie netwerken in het licht te stellen. De combinatie van de kennis van recente infectie en gelinkte besmettingen zal uiteindelijk toelaten om subgroepen te ontdekken met een hoog risico op overzetting van het HIV virus in België. EN In order to control the number of new infections in Belgium, there is a need for better prevention, treatment and especially surveillance in Belgium. The current surveillance of the epidemic in Belgium does not allow us to distinguish recent HIV infection and chronic HIV infections. Nevertheless, this is essential information to allow effective control of the HIV epidemic in Belgium. This project seeks to validate laboratory assays and find an algorithm that allows to determine the time of the infection with an accuracy of a few months. This information will in turn be linked to the genetic information of the HIV virus allowing to elucidate transmission networks in Belgium and populations at high risk of transmitting the virus.
