III. This confirmation is issuedfor the term up to December31, 2004. The evidencenumber for purposesgiven in §13, Part 1, Letter a), and in §13, Part 8, ofthe Act No: 18/1997is this number: 61389005. Reasoning: This Decree,I issue accordingto your Application submittedin December16, 1998and basedon the results ofthe control visit ofyour laboratoryin January29, 1998performed byan inspectorofthe StateOffice for Nuclear Safety,the submitteddocumentationmeet demandsof §13, and the SupplementI of the sited Act. During the control of your laboratory no deficiencieswere found that would precludeissuing of this Decreeof the handling of open,closedradionuclideemittersand generatorsof radiations. Odùvodnìní: Rozhodnutí vydávám na Vaši žádost ze dne 16.12.1998a na základì výsledku kontrolní návštìvy pracovištì dne 29.1.1998 provedenéinspektorem SÚJB.Pøedložená dokumentace splòuje požadavky § 13 a pøílohyI citovanéhozákona.Šetøenímna pracovišti samémnebyly zjištìny závažnénedostatky, které by bránily vydání rozhodnutí pro používání otevøených, uzavøenýchradionuklidových záøièùa generátorùzáøení.
