SAMENVATTING
Hoofdstuk 1 N a inleiding en historisch overzicht wordt een overzicht gegeven van de ver
schillende nomenciaturen, die in de literatuur worden gebruikt.
De leeftijdsverdeling vertoont hetzelfde beeld als voor het primaire ulcus, met
een kleine verschuiving van enkele j aren.
Wat het geslacht betreft, zijn het overwegend de mannen, die aan een ulcus
pepticum jejuni lijden.
Enkele oorzaken hiervan werden nagegaan.
Het ulcus pepticum jejuni is hoofdzakelijk in de afvoerende lis gelocaliseerd.
In hoofdstuk II wordt de frequentie van het ulcus pepticum jejuni besproken.
Het ulcus is na operaties wegens maagcarcinoom zeer zeldzaam, na operaties
we gens ulcus ventriculi gering. Het grootst aantal ulcera peptica jejuni ont
staat na operaties wegens ulcus duodeni.
Ook de operatiemethode heeft grote invloed.
De hoogste recidief percentages komen voor na unilaterale pylorus uitschake
ling volgens v. Eiselsberg, gastro-enterostomie en uitschakelings resectie.
Minder frequent doch nog in beduidend percentage na vagotomie en drainage
methoden (gastro-enterostomie en pyloroplastiek).
Nog minder frequent na subtotale maagresectie.
De laagste percentages worden gevonden na vagotomie en antrectomie ('com
bined procedure').
In hoofdstuk III wordt de pathogenese besproken. Gesteld wordt, dat peptische
werking conditio sine qua non voor de ontwikkeling van een peptisch ulcus is.
N a bespreking, in het kort, van de anatomie- en histologie, wordt nader inge
gaan op de physiologie van de maagsapsecretie. De stimulatie van de maag
sapsecretie, wordt door verschillende phasen beinvloed n1.:

Uit proeven, vooral uit het laboratorium van Dragstedt, werd de invloed van
gas trine aangetoond, alsmede de prikkels tot afscheiding van di t hormoon.
De laatste jaren staat de afscheiding van gastrine 0I1der invloed van vagus
prikkeling op de voorgrond.
Bij lijders aan ulcus duodeni wordt een groter aantal parietale cellen ge
vonden dan bij normale patienten . De hoogte van de zuurwaarden , verkregen
door maximale stimulatie, is evenredig met het aantal parietale cellen.

Zuursecretie wordt geremd door lage PH . in het antrum en duodenum . De vraag of dit geschied door een afzonderlijk remmend hormoon is nog niet opgelost. De bescherming van het slijmvlies berust op 3 factoren: l. De slijmlaag. 2. Weerstand van het epithee!. 3. Buffering en neutralisatie door voedsel en darmsap. De anatomische grenzen van het antrum worden besproken , evenals het ana
tomisch onderzoek en de bepaling van de grenzen van het antrum, door middel
van kleurmethoden en PH . meter durante operatione.
Nagegaan werd, in hoeverre de bekende operatiemethoden (gas tro-entero
stomie, subtotale maagresectie, uitschakelingsresectie, segmentale resectie , va
gotomie met drainagemethoden en de combined procedure) de zuursecretie be
invloeden.
De invloed van de reconstructiemethoden (antecolische anastomose, lange of
korte aanvoerende lis, Raux Y anastomose) wordt besproken , alsmede de in
vloed van extra gastrische factoren .
In hoofdstuk IV wordt de symptomatologie besproken. Het vrije interval is
beduidend langer na gastro-enterostomie dan na maagresectie.
Het ulcus pepticum jejuni treedt later na de primaire operatie op, dan in de
oudere literatuur wordt aangegeven.
De pijn staat als symptoom op de voorgrond, soms met ongewone uitstraling;
gewichtsverlies, zuurbranden, misselijkheid en braken en anaemie zijn verdere
factoren, die de aandacht verdienen.
Van het laboratoriumonderzoek is het bepalen van maagzuurwaarden volgens
de verschillende methoden van groot belang. Daar naast het bloedonderzoek
en onderzoek van de faeces op occult bloed.
Het onderzoek met rontgenstralen is moeilijker dan bij het ulcus duodeni of
ventriculi. Met in acht nemen van bepaalde maatregelen is bijna dezelfde
zekerheid te behalen als bij het onderzoek van een ulcus duodeni.
In hoofdstuk V wordt de differentiele diagnose In aanmerking komen:

In hoofdstuk VI worden de complicaties besproken, bloeding, perforatie en maag- jej uno -colonfistel. Bloed ing komt zeer frequent v~ ~ r, meestal in de vorm van melena. De prognose quo ad vitam is beter dan bij bloeding uit ulcus ventriculi of ulcus duodeni : Conservatieve th e rapie is meestal voldoende. Perforatie is een gevaarlijker aand c ening en kc mt in plm. 10-15% der ge vallen van ulcus pepticum jejuni v~~r. Doer vergroeiingen van voorafgaande operaties kan de symptomatologie vertroebeld zijn. Perforatie met abcesvorming in de voorste buikwand ke mt bij ulcus pepticum jejuni na antecolische anastomose v~~r. De aangewezen therapie bij een perfora tie is primaire resectie , overhechting aileen wordt vaak gevolgd door hoge mortaliteit en door reperforatie. Maag-jejuno-colonfistel komt in plm . 10% der gevallen van ulcus pepticum jejuni v~~r. Andere oorzaken van maag- jejuno-colonfistel worden besproken. Hoofdsymptomen zijn: 1. Frequente, waterdunne diarrhee. 2. Extreem gewichtsverlies. 3. Faecaal braken en faecale ruetus. Bij het rontgen onderzoek wordt de diagnose na een barium clysma bijna steeds
gesteld. Bij toediening van barium pap per os is de fiste! slechts ze!den aan
toonbaar.
Na de differentieIe diagnostiek wordt de therapie besproken. Aileen de opera
tieve therapie heeft kans van slagen. Korte voorbehandeling, waarbij normali
satie van eleetrolyten , waterhuishouding, Hb. en bloed-eiwitten, wordt nage
streefd , is noodzakelijk.
De operatieve therapie dient zich te richten op:
I. Opheffing van de fistel. 2. Behandeling van het ulcus pepticum jejuni, dat er de oorzaak er van was. Bij algemeen goede toestand van de patient kan in een tempo een 'en bloc' . resectie geschieden. I. Reseetie van dee! van het colon transversum, sluiten van de opening In
het col on. 2. Resectie van het fistel dragend deel van maag en jejunum. 3. Eventueel vagotomie. 4. Reconstruetie, waarbij de antecolische anastomose de voorkeur verdient. Is de algemene toestand slecht, dan dient in tempi te worden geopereerd . Een preliminaire anus praeter naturalis of ileosigmoideostomie wordt in eerste tempo aangelegd. In 2e tempo vindt de eigenlijke resectie plaats. Er wordt een overzicht gegeven van de verschillende operatie technieken, de operatieve mortaliteit en de resultaten.

vormende deel en of nog antrumweefsel aanwezig is.
Is de maagresectie lege artis uitgevoerd, dan verdient vagotomie de voorkeur.

gens voldoende maagresectie kan hiermede worden volstaan.

werden verzameld. (5,4%).

Bij de 1.532 patienten, die operatief werden behandeld , was het recidief per

in de literatuur.
In hoofdstuk IX worden de verkorte ziektegeschiedenissen van 60 patienten
meegedeeld, 58 van hen leden aan ulcus pepticum jejuni. Van de overige 2,
leed een aan een gastro-ileostomie, terwijl bij de ander een jejuno-colostomie
was aangelegd.
In hoofdstuk X wordt de analyse gegev en van de ziektegeschiedenissen van

interval bij 20 patienten langer dan 5 jaar, bij slechts 3 korter.

13 patienten in de eerste 5 jaar na de operatie op, bij de overige 13 patienten
was het vrije interval lange r dan 5 jaar.

Bij de patienten met ulcus ventriculi waren deze getallen resp. 2,40/0 en 1,2010, totaal 3,60/0. Voor het totaal aantal patienten 2,4010 bewezen ulcera peptica jejuni, 3,10/0 verdachte, samen 5 ,50/0. Dieet hi elden 21,6010 der patienten geregeld of af en toe,78,4% hield geen enkele vorm van dieet.

SUMMARY
Chapter I is an introduction and historical review, followed by discussion of
the various nomenclatures used in the literature.
The age distribution shows the same features as that of primary ulcer, with a
small shift of a few years . Jejunal peptic ulcer shows male predominance;
some of the causes of this predominance are discussed.
The principal localization of jejunal peptic ulcer is in the efferent loop.
Chapter II discusses the frequency of jejunal peptic ulcer, which is seldom
seen after operations for gastric carcinoma, infrequently after operations for
gastric ulcer but most frequently after operations for duodenal ulcer. The sur
gical procedure used is a factor of importance in this respect.
The highest relapse rate occurs, after a unilateral pylorus exclusion according
to v. Eiselsberg, gastroenterostomy and resection by exclusion. A less high but
still significant percentage of cases is observed after vagotomy and drainage
procedures (gastroenterostomy and pyloroplasty). The rate of relapse is even
lower after subtotal gastrectomy and the lowest rate is seen after the com
bined prGcedure of vagotomy and antrectomy.
Chapter III discusses the pathogenesis. It is maintained that peptic activity is
a conditio sine qua non for the development of a peptic ulcer. After a brief
discussion of the anatomy and histology , the physiology of gastric juice secre
tion is considered . The stimulation of gastric juice secretion is influenced by
various phases , viz.:

Experiments , particularly those carried out in Dragsledl's laboratory, have
shown the effect of gastrin and demonstrated the stimuli that govern the se
cretion of this hormone . In the past few years emphasis has been on gastrin
secretion under the influence of stimulation of the vagus nerve.
In patients with duodenal ulcer, parietal cells are found to be more numerous
than in normal patients . The acidity values achieved by maximum stimulation
are proportionate to the number of parietal cells. Defensive factors are dis
cussed.
Acid secretion is inhibited by a low pH in the antrum and the duodenum. The
question as to whether this is governed by a separate inhibitory hormone re
mains to be solved.

Protection of the mucosa is based on three factors, viz.: 1. the layer of mucus, 2. the resistance of the epithelium, 3. buffering and acid neutralization by food and intestinal juice. The anatomical boundaries are discussed with reference to anatomical exami
nation, and to demarcation of the antrum by means of staining' techniques and
pH determination during the operation.
The effect of known surgical procedures (gastroenterostomy, subtotal gast
rectomy, resection by exclusion segmental resection, vagotomy with draining
techniques and the combined procedure) on acid secretion is discussed.
A special discussion is devoted to reconstructive procedures (antecolic anasto
moses, a long or a short afferent loop, the Roux- Y anastomosis) and to extra
gastric factors.
Chapter IV discusses the symptomatology. The asymptomatic interval after
gastroenterostomy is considerably longer than that after gastrectomy. Jejunal
peptic ulcer occurs later after the primary operation than the older literature
indicates.
Pain is the most prominent symptom, sometimes with unusual distribution; loss
of weight, heartburn, nausea and vomiting and anaemia are additional factors
which require attention.
In laboratory investigations, determination of the gastric acid values by various
methods is of great importance; in addition, haematological studies and the
examination of stools for occult blood are of importance.
X-ray examination in these cases is more difficult than in duodenal or gastric
ulcer. By using certain precautions when it is possible to achieve almost the
same degree of certainty as when examining duodenal ulcers.
Chapter V deals with the differential diagnosis. Important in this respect are:
severe forms of gastrojejunitis; carcinoma of the gastric stump; obstruction of the afferent or efferent loop; retrograde intussusception of the jejunum; the Zollinger-Ellison syndrome; upper abdominal affections such as hiatus hernia, cicatricial rupture, epI gastric hernia and especially cholecystopathies; 7. changes in the aboral part of the digestive tract such as fixation of the omentum, an ulcer in Mechel's diverticulum, regional ileitis and ulcerative colitis.

Chapter VI discusses complications such as haemorrhage, perforation and gastro-jejuno-colic fistulization. Haemorrhage is very frequent, usually in the form of melaena. The prognosis quo ad vitam is better than that in hae morrhage from a gastric or duodenal ulcer. Conservative therapy usually suf fices.

Perforation is a more dangerous condition, which occurs in some 10-15% of
cases of jejunal peptic ulcer. The symptomatology may be complicated by
adhesions from previous operations. Perforations with abscess formation in the
anterior abdominal wall occurs in jejunal peptic ulcer after antecolic anasto
mosis. The treatment of choice is primary resection; simple suturing has a high
mortality and a high rate of re-perforation.
Gastrojejunocolic fi stulization occurs in some 10% of cases of jejunal peptic
ulcer. Other causes of gastrojejunocolic fistulization are discussed. The prin
cipal symptoms are 1. frequent watery diarrhoea; 2. extreme weight loss; 3.
faecal vomiting and feculent eructations.
X-rays after a barium enema nearly always disclose the diagnosis but after
a barium meal the fistula is seldom demonstrable.
After the differential diagnosis, the treatment of these fistulae is discussed .
Only surgical therapy can be wccessful. A brief preparatory period for nor
malization of electrolytes, water metabolism, estimation of the Hb concen
tration and blood proteins is indispensable.
Surgical therapy should aim at:
1. abolition of the fistula, 2. treatment of the causative jejunal peptic ulcer.
Good general condition permits of a one-stage resection en bloc which involves:
1. Resection of part of the transverse colon or closure of the opening in the colon. 2. Resection of the fistulized part of stomach and jejunum. 3. Vagotomy, if required. 4. Reconstruction , with preference for an antecolic anastomosis.
Poor general condition calls for surgery in stages . The first stage consists of establishment of a preliminary preternatural anus or ileosigmoidostomy. The second stage consists of the resection proper. A review is given of the various techniques for operation, the mortality at or after operation and the results obtained. Chapter VII discusses the treatment of jejunal peptic ulcer. Non-radical therapy: 1. Rest, diet and medication. 2. X-ray treatment. 3. Ncn-radical surgery, viz.: a. Excisi on of the ulcer (100% relapse). b. Abolition of the gastroenterostomy ; restoration of the old anatomical con ditions; relapse of the duodenal ulcer may occur. c. Establishment of a new gastroenterostomy beside the old one; jejunal peptic ulcer will occur in the new gastroenterostomy. d. Jejunostomy, adequate palliative therapy ; the jejunal peptic ulcer relapses after abolition of the jejunostomy.

Definitive therapy:
A distinction must be made according to whether the jejunal peptic ulcer has
occurred.
A. after gastroenterostomy, B. after gastrectomy. In the former case, gastrec tom y is the treatment of choice for jejunal peptic
ulcer. In the latter case, it must be decided whether the gastrectomy had ade
quately removed the acid-forming part or has left some antral tissue in situ.
After a technically flawless gastrectomy, vagotomy is to be preferred. If antral
tissue has been left in situ, then this must be removed; this is sufficient if the
gas trectomy has been otherwise adequate.
Resection must be resorted to in all other cases. Emphasis must be placed on
investigation for antral tissue left in situ and on inspection of the pancreas for
insul inomata.
The procedures are discussed .
Chapter VIII reviews the mortality in 2622 cases of jejunal petic ulcer col
lected from the literature si nce 1947 (5.4010). A review is also presented of the
results obtained since 1947 in patients with jejunal peptic ulcer as reported
in the literature, viz.:
1. 774 patients with jejunal peptic ulcer after gas troenterostomy; 2. 996 patients with jejunal peptic ulcer after gas trectomy. Of this group , 10 7 were given non-surgical treatment, and 50 of these de veloped a relapse. The rate of relapse among the 1532 patiens given sur g ical treatment, was 20.5 % • Relapse percentages reported in the literature range from 10 to 15010. Chapter IX gives summaries of 60 case histories, including 58 cases of jejunal peptic ulcer, 2 with ulcer, 2 with a gastroileostomy and a jejunocolos tom y, respecti vel y. Chapter X analyses the case histories of the 58 patients with a jejunal peptic ulcer. Their ages ranged from 24 tot 68 years; 54 were males and 4 were females. The nature of the primary ulcer was known in 52 cases (duodenal ulcer in 45 pyloric or prepyloric ulcer in 5 and gastric ulcer in on ly 2 cases). The primary operation had been a gastrectomy in 26, a bypass resection in 6, a gastroen terostomy in 23 cases; its nature was unknown in 1 case, a Billroth I in 1 case and a Billroth II in one case; 5 patients with residual jejunal peptic ulcer were frist admissions. The position of the jejunal peptic ulcer was known in 54 cases; a dual ulcer existed in 2 cases and a relapse in 6 cases ; the ulcer was positioned in the efferent loop in 32 and in the anastomosis in 18 cases .

An X-ray examination was made in 47 cases; jejunal peptic ulcer was de
monstrated with certainty in 33 cases; 7 cases showed highly suspicious X-ray features ; in 7 cases, no ulcer was demonstrable.
Complications Haemorrhage was seen in 19 cases, perforation into the free abdominal cavity occurred in 10 and a gastrocolic fistula formed in 3 cases. In all cases except one, treatment was surgical. A review of the operations performed is presented: 1. 2. 3. 4. 5.

Chapter IX reviews all patients treated in the Groningen Surgical Clinic from 1st January 1950 to 31st December 1954. Of the total of 278 patients, 9 died in the clinic; 38 died after discharge; 7 left the country. There remained 224 for foHow-up. Ten of these were lost sight of in the course of the 10-15 year follow-up period, leaving 214 patients . Of the total number of 278 patients, 236 were men and 42 were women; the ages ranged from 19 to 80 years. Follow-up Of the 224 patients asked follow-up questions , 215 replied and only 210 of the replies were of any use. Of the 7 patients with relapse of the ulcer after a BILLROTH I, 3 reported a satisfactory result; in the other 4 the result was unsatisfactory.
