Presentation Lars Tummers Introducing the model of ‘policy alienation’ Problems with policies from government Professionals working in the public interest often have problems with new policies. This ranges from Israeli teachers striking against school reforms, Dutch professors protesting against cost-cutting in higher education, and American physicians feeling overwhelmed by a constant flow of policy changes, resulting in conflicts and burn-outs. An illuminating quote comes from a Dutch insurance physician who had to implement stricter rules for work disability insurance, causing many former citizens classified as work disabled to lose their benefits1: “The UWV [Dutch organization for employees’ insurance] is nowadays called the Lourdes of the North: you visit the agency as work-disabled, you leave able to work… It is becoming extremely controversial. I cannot reconcile it with my conscience anymore.” When professionals cannot identify with a policy, this can has severe consequences. It can negatively influence policy effectiveness, as professionals do not execute the policy or even try to sabotage it. Furthermore, professionals themselves can become dissatisfied with their work. Some professionals even experience burn-out or quit their jobs entirely. Discovering the ‘policy alienation model’ to understand pressured professionals Hence, when professionals have identification problems with new policies, this is important. Such problems have been acknowledged by many scholars, managers and professionals. However, there was no coherent model for analyzing it. In my PhD-thesis, I therefore developed a new model to understand why professionals cannot identify with governmental policies. This model is called the ‘policy alienation model’. Policy alienation occurs when professionals feel psychologically disconnected from the policy they have to implement. The policy alienation model consists of five dimensions. In the table below, each dimension is shown, and an example of a high score on each dimension. I must stress that there are of course also policies where there is a low level of policy alienation (see for instance the case of midwives, chapter 9 in my PhD-thesis).

1. Strategic powerlessness
A social worker feeling that a new policy developed by the Ministry of Social Affairs and Employment is drafted without even consulting the professional associations for social workers.
2. Tactical powerlessness
A physician stating that the managers in her hospital did not consult her or her colleagues when designing the implementation process for the new policy.
3. Operational powerlessness
A psychologist answering ‘fully agree’ to a survey question on whether he feels that his autonomy during the implementation of a new financial policy is lower than it should be.
4. Societal meaninglessness
A professor stating in an interview that “I agree with the policy goal of enhancing the value of science for the public (valorisation), but I do not see how this specific policy could ever help in achieving that goal.”
5. Client meaninglessness
A teacher who argues that a particular policy seriously lowers the chances of his students to learn a new skill properly.
The first three dimensions refer to powerlessness. Powerlessness occurs when professionals feel unable to influence a policy on strategic (national), tactical (organizational) or operational (own work) levels. For instance, tactical powerlessness occurs when you, as a professional, feel that you and your fellow professional colleagues have not been consulted when a new policy was implemented in your organization: Managers didn’t involve professionals. Meaninglessness occurs when professionals feel that a policy is not really adding any value to society at large or for their own clients. For instance, a teacher can feel that a certain new policy developed by the Ministry of Education is not beneficial for the students he teaches, or that this policy is even harmful for some students. Suggestions based on studies using policy alienation model I used the policy alienation model to analyze the problems of professionals with governmental policies in various settings, ranging from education to healthcare and social security. I used both qualitative and quantitative analyses. Based on these analyses, I want to provide the following three – sometimes provocative - suggestions on how to reduce the resistance of professionals towards governmental policies. Suggestion 1 – Do not let professionals participate The first suggestion is counterintuitive: do not let professionals participate. Contrary to this statement, many scholars and managers – often coming from change management and HRM – emphasize participation. Although the impact of participation (here analyzed using the powerlessness notion) is not zero, it is far less important than often argued. Professionals, we found, stated that they did not want to get involved, especially when policies were being developed which were only indirectly related to their own work. For instance, psychiatrists did not want to get involved in the specifics of a new financial reimbursement policy. They wanted to focus on their own work, which was treating patients.

Suggestion 2 – Make more meaningful policies Rather than focusing especially on participation, we should improve the meaningfulness of a policy. I found that when professionals felt that a policy was valuable for society or for their own clients, they were far less resistant. This impact was in all cases stronger than the influence of strategic and tactical powerlessness. Hence, we should increase the value of policies. This can be achieved by developing pilots before the ‘rolling out’ of a policy nationwide. These pilots could be used to improve the design of a policy, thereby increasing its meaningfulness for society and clients. Another option would be to communicate the policy more intensively, for example by highlighting why it is needed and the results already achieved. Suggestion 3 – Give professionals autonomy The last suggestion is that we should provide professionals with autonomy during policy execution. Autonomy is a defining characteristic of professional work. I found that high operational powerlessness (hence, low autonomy) is strongly related to resistance to implement policies. Related to this, Michael Lipsky (author of ‘Street-level Bureaucracy’) also argues that attempts to control professionals via hierarchical ways only increases their tendency to resist. Providing autonomy to professionals can also have several important effects for professionals themselves, such as more satisfaction, less stress and more work engagement. This seems all the more important given that many feel that their autonomy in their work is nowadays rapidly diminishing. Conclusion: Accept the power of professionals to implement policies In the coming years, politicians will strive to make public organizations more cost-effective, transparent, and client oriented. To achieve this, numerous policies will be developed. Professionals will often be the ones who have to implement these policies, and public managers must administer and oversee this. Hence, politicians, managers, and professionals will be inevitably connected. It is their shared task to serve the public. I hope that my PhD-thesis –the developed policy alienation model, the measurement instrument and the conclusions – helps to increase our understanding of how professionals experience the policies they have to implement and shows how policy implementation can be improved. If this happens, the level of policy alienation of professionals will diminish, and maybe we will be able to say that the policy alienation model was put to good use, namely, to really lower policy alienation. De introductie van het model van ‘beleidsvervreemding’ Bijdrage voor Jaarverslag Stichting Praemium Erasmianum Dr. Lars Tummers www.larstummers.com Professionals hebben vaak problemen met overheidsbeleid Professionals die werken voor de publieke zaak hebben vaak problemen met overheidsbeleid. Dit varieert van docenten uit Israël die 64 dagen lang protesteerden tegen nieuw overheidsbeleid, Nederlandse hoogleraren die in opstand komen tegen bezuinigingen in het hoger onderwijs en Amerikaans artsen die overdonderd worden door de constante stroom van nieuw beleid, wat zorgt voor conflicten en burn-outs. Een illustratief voorbeeld komt van een Nederlandse verzekeringsarts die reageert op een nieuw beleid om WAO-ers te herkeuren op basis van strengere criteria.2 Dit zorgde ervoor dat veel voormalig arbeidsongeschikten hun uitkering verloren. “Het UWV wordt ook wel het ‘Lourdes van het Noorden’ genoemd: je gaat er arbeidsongeschikt heen en komt er arbeidsgeschikt weer vandaan… Het wordt extreem controversieel. Ik kan het niet meer in overeenstemming brengen met mijn geweten.” Als professionals zich niet kunnen identificeren met overheidsbeleid kan dit ernstige gevolgen hebben. Professionals kunnen het beleid bijvoorbeeld niet uitvoeren of zelfs saboteren. Hiernaast kunnen professionals ontevreden worden of zelfs stoppen met hun baan. Ontwikkeling van model beleidsvervreemding De problemen met overheidsbeleid zijn onderkend door verschillende onderzoekers, managers en professionals. Er bestaat alleen nog geen coherent theoretisch raamwerk om dit onderwerp te analyseren. Dit proefschrift ontwikkelt daarom een theoretisch raamwerk over beleidsvervreemding (‘policy alienation’). Ik definieer beleidsvervreemding als een algemene cognitieve staat van psychologische ontkoppeling met het beleid, in dit geval van de publieke professional die het beleid uitvoert. Het model van beleidsvervreemding bestaat uit vijf dimensies. In de tabel geef ik elke dimensie weer, inclusief een voorbeeld van een hoge score. Ik moet hierbij opmerken dat er natuurlijk ook situaties zijn waar weinig beleidsvervreemding voorkomt (zie bijvoorbeeld hoofdstuk 9 in het proefschrift, over verloskundigen).

1. Strategische machteloosheid
Een maatschappelijk werk die het gevoel heeft dat een nieuw beleid van het Ministerie van Sociale Zaken en Werkgelegenheid ontwikkeld is zonder de professionele verenigingen van maatschappelijk werkers te betrekken.
2. Tactische machteloosheid
Een arts die stelt dat de managers in het ziekenhuis waar zij werkt haar en haar collega’s niet hebben betrokken bij de invoeren van het beleid.
3. Operationele macheloosheid
Een psycholoog die ‘volledig mee eens’ scoort op een vraag of hij het gevoel heeft dat hij te weinig autonomie heeft om het beleid aan te passen aan de kenmerken van specifieke cliënten.
4. Zinloosheid voor de samenleving
Een hoogleraar die in een interview aangeeft ‘Ik ben het eens met het beleidsdoel om valorisatie van de wetenschap te versterken, maar ik denk niet dat dit specifieke beleid ooit gaat helpen om dit doel te bereiken’
5. Zinloosheid voor de eigen cliënten
Een docent die ervaart dat een nieuw overheidsbeleid zeer negatief uitpakt voor zijn studenten.
De eerste drie dimensies verwijzen naar een gevoel van ‘machteloosheid’. Professionals ervaren machteloosheid als zij het beleid niet kunnen beïnvloeden op strategisch (nationaal), tactisch (organisationeel) of operationeel (eigen werk) niveau. Tactische machteloosheid treedt bijvoorbeeld op als u, als professional, het gevoel heeft dat u en uw collega-vakgenoten niet worden geraadpleegd bij een nieuw beleid in uw organisatie. De directie of managers betrekken professionals niet. Zinloosheid treedt op als professionals het gevoel hebben dat een beleid weinig waarde toevoegt aan de samenleving of voor hun eigen cliënten. Zo kan een leraar het gevoel hebben dat een nieuw overheidsbeleid ongunstig is voor zijn studenten, of zelfs schadelijk. Suggesties op basis van studies over beleidsvervreemding Ik heb het model van beleidsvervreemding gebruikt om de problemen van professionals met overheidsbeleid te analyseren in de gezondheidszorg, sociale zekerheid en het onderwijs. Ik gebruikte zowel kwalitatieve als kwantitatieve analyses. Op basis van deze analyses wil ik drie - expres provocerende - suggesties doen. Suggestie 1 - Laat professionals niet participeren De eerste suggestie is contra-intuïtief: laat professionals niet participeren. Veel onderzoekers – vaak met achtergronden in verandermanagement en HRM - benadrukken juist participatie. Hoewel de invloed van participatie (hier geanalyseerd via de dimensies van machteloosheid) niet nul is, is deze veel minder belangrijk dan was verwacht. Professionals vertelden ons onder andere dat ze niet altijd betrokken hoefden te worden, vooral wanneer beleid niet zoveel te maken had met hun kernwerkzaamheden. Psychiaters gaven bijvoorbeeld aan dat ze niet zo geïnteresseerd waren in de specifieke kenmerken van een nieuw financieel systeem in de zorg. Ze wilden zich vooral richten op hun ‘echte’ werk, namelijk de behandeling van patiënten, en hier niet teveel van afgeleid worden.

Suggestie 2 – Ontwikkel zinvoller beleid In plaats van te richten op participatie, moeten we juist meer aandacht besteden aan de zinvolheid van nieuw overheidsbeleid. Als professionals het beleid zinvol vinden voor de samenleving of hun eigen cliënten vertonen ze veel minder weerstand. Dit effect was in alle analyses sterker dan het effect van strategische en tactische machteloosheid. Een logische conclusie is dan ook om te proberen om te zinvolheid van het beleid te verhogen. Dit kan bijvoorbeeld door het ontwikkelen van pilots voordat nieuw beleid landelijk wordt ingevoerd. Door te leren van deze pilots kan de waarde van het beleid voor de samenleving en cliënten worden verhoogd. Een andere mogelijkheid is om de voordelen van het beleid intensiever te communiceren, want soms hebben professionals wel het gevoel dat het geen waarde heeft, maar is deze waarde wel aanwezig maar niet goed gecommuniceerd. Suggestie 3 - Geef professionals autonomie De laatste suggestie is om professionals autonomie te geven bij beleidsuitvoering. Professionals verwachten door hun hoge opleiding en status autonomie. Een hoge mate van operationele machteloosheid (oftewel lage autonomie) zorgt voor meer weerstand tegen beleid. Michael Lipsky (auteur van 'Street-level Bureacracy') geeft ook aan dat pogingen om professionals in te perken alleen maar zorgt voor meer problemen. Als professionals juist wel autonomie ervaren kan dit ook positieve effecten hebben voor henzelf, zoals meer tevredenheid, minder stress en meer bevlogenheid. Deze suggestie lijkt ook belangrijk omdat veel hedendaagse professionals het gevoel hebben dat hun autonomie in hun werk aan het verdwijnen is. Conclusie: Accepteer de kracht van professionals In de komende jaren zullen politici proberen om publieke organisaties efficiënter, klantgerichter en transparanter te maken. Om dit te bereiken zullen ze waarschijnlijk verschillende nieuwe beleidsmaatregelen ontwikkelen. Professionals moeten dit beleid vaak uitvoeren, en publieke managers wordt gevraagd dit proces te overzien. Politici, managers en professionals zijn dus onvermijdelijk aan elkaar verbonden. Het is hun gezamenlijke taak om het publiek te dienen. Ik hoop dat mijn proefschrift – inclusief het model van beleidsvervreemding model, het meetinstrument hiervoor en de conclusies - helpt dit proces te verbeteren. Zo kan het model van beleidsvervreemding uiteindelijk worden gebruikt voor een goed doel, namelijk de mate van beleidsvervreemding verminderen.
