Huisarts C.A.J. Huisarts C.A.J.

Zo publiceert de FDA op haar website onder meer het volgende : 2004 Safety Alert: Zyprexa (olanzapine) http://www.fda.gov/medwatch/SAFETY/2004/zyprexa.htm March 1, 2004 Re: Safety data on Zyprexa ® (olanzapine) - Hyperglycemia and Diabetes Dear Doctor, Eli Lilly and Company would like to inform you of important labeling changes regarding Zyprexa (olanzapine). The Food and Drug Administration (FDA) has asked all manufacturers of atypical antipsychotic medications, including Lilly, to add a Warning statement describing the increased risk of hyperglycemia and diabetes in patients taking these medications, including Zyprexa. In addition to Zyprexa, the atypical antipsychotic class includes Clozaril ® (clozapine, Novartis), Risperdal ® (risperidone, Janssen), Seroquel ® (quetiapine, AstraZeneca), Geodon ® (ziprasidone, Pfizer), and Abilify ® (aripiprazole, Bristol Myers Squibb and Otsuka American Pharmaceutical). Accordingly, the Zyprexa prescribing information has been updated with the following information: WARNINGS Hyperglycemia and Diabetes Mellitus Hyperglycemia, in some cases extreme and associated with
ketoacidosis or hyperosmolar coma or death, has been reported in patients treated with atypical antipsychotics including Zyprexa. Assessment of the relationship between atypical antipsychotic use and glucose abnormalities is complicated by the possibility of an increased background risk of diabetes mellitus in patients with schizophrenia and the increasing incidence of diabetes mellitus in the general population. Given these confounders, the relationship between atypical antipsychotic use and hyperglycemia-related adverse events is not completely understood. However, epidemiological studies suggest an increased risk of treatment-emergent hyperglycemia-related adverse events in patients treated with the atypical antipsychotics. Precise risk estimates for hyperglycemia related adverse events in patients treated with atypical antipsychotics are not available. Patients with an established diagnosis of diabetes mellitus who are started on atypical antipsychotics should be monitored regularly for worsening of glucose control. Patients with risk factors for diabetes mellitus (e.g., obesity, family history of diabetes) who are starting treatment with atypical antipsychotics should undergo fasting blood glucose testing at the beginning of treatment and periodically during treatment. Any patient treated with atypical antipsychotics should be monitored for symptoms of hyperglycemia including polydipsia, polyuria, polyphagia, and weakness. Patients who develop symptoms of hyperglycemia during treatment with atypical antipsychotics should undergo fasting blood glucose testing. In some cases, hyperglycemia has resolved when the atypical antipsychotic was discontinued; however, some patients required continuation of anti-diabetic treatment despite discontinuation of the suspect drug. Should you have any questions or concerns regarding this important safety information, please contact your Eli Lilly and Company sales representative or contact the Lilly medical department at 1-800-Lilly-Rx . Please refer to the full prescribing information for Zyprexa included with this letter. As always, we request that serious adverse events be reported to Lilly at 1-800-Lilly-Rx or to the FDA MedWatch program by phone (1-800-FDA-1088 ), by fax (1-800-FDA-0178 ) or by email (www.fda.gov/medwatch). Sincerely, Dr. Paul Eisenberg Vice President, Global Product Safety Eli Lilly and Company
Op 17 vrijwillig opgenomen in p/a Zon & Schild,
procedure waarmee Hoogenboom in een IBS oktober 2002 is Hoogenboom afd. Amergaard .
Orientatie is goed. geheugen is intact. Client is nu rustig. Medicatie advies (eventueel toegediende medicatie) : Voor vannacht 2 tabletten Oxazepam 10 mg." Was getekend Mevrouw J.M.A. Janssen, manager Psychiatrie, coordinator Psychiatrische hulpdienst, dd. 17 oktober 2002. (patient is gezien op 15 Oktober 2002 om 17:00h) Ter info : http://www.psyweb.com/Drughtm/serax.html : "Oxazepam ( Serax ) is an antianxiety agent ( benzodiazepines. ) Used primarily for treatment mild to moderate anxiety. It may also be used to treat symptoms of alcohol withdrawals or alcohol addiction."
Janssen, manager Psychiatrie, coordinator Psychiatrische hulpdienst : Medicatie advies: Voor vannacht 2 tabletten Oxazepam 10 mg. Volgens http://www.psyweb.com/Drughtm/serax.html : Oxazepam ( Symptoms or Effects ) Common: Clumsiness / Sleepiness. Rare: Abdominal cramps, blurred vision, dry mouth, racing heartbeat / palpitations, shaking / slurred speech, urination problems, seizures, hallucinations, memory loss, trouble breathing, staggering / trembling, headache or confusion. See physician always: Abdominal cramps, blurred vision, dry mouth, racing heartbeat / palpitations, shaking / slurred speech, urination problems, seizures, hallucinations, memory loss, trouble breathing, staggering / trembling, headache or confusion. See physician if severe: Clumsiness / Sleepiness. See physician NOW: Confusion or seizures. Stop taking and see physician NOW: Convulsions, hallucinations, memory loss, trouble breathing or staggering / trembling.
Roomer in October 2002.
Ondergetekende, A.C.J. Hoogachtend, drs A.C.J. Hoogenboom, student Psychiatrie UMC Utrecht.

The following is a translation from Dutch into English to illustrate how and with what particular methods, deliberate loopholes in legislation and technology the local Dutch authorities are able to frame certain people into remote satellite harassments using US Patented devices like the neurophone. The motive for doing so? Well thats a very interesting question. A wealth of material on this can be found on http://www.surveillanceissues.com/ . Here comes a official complaint from a remote satellite harassment victim Ton Hoogenboom to demand a 2nd opinion from a psychiatrics Professor at the Dutch Hospital UMC Utrecht in the Netherlands. Dr. Kahn claims that victim Ton Hoogenboom actually is suffering from schizophrenic disease.

Sadly enough my partner cannot be present. Instead a trusted person, R.M. Stockmann will attend. Furthermore i hereby use this occasion to place some remarks and present a couple of relevant facts. As you should know, according my medical files, in the past a medication with brand name ZYPREXA was forced upon me, without my approval. A year ago the American FDA, Food and Drug Association, published a couple of important and frighting Safety Alerts on Zyprexa. One of the alerts which should be taken rather seriously is the following : 2004 Safety Alert: Zyprexa (olanzapine) http://www.fda.gov/medwatch/SAFETY/2004/zyprexa.htm March 1, 2004 Re: Safety data on Zyprexa ® (olanzapine) - Hyperglycemia and Diabetes Dear Doctor, Eli Lilly and Company would like to inform you of important labeling changes regarding Zyprexa (olanzapine).
The conclusion of this publication is that the use of Zyprexa as medication for the treatment of schizophrenia will inevitably be accompanied with an substantial risk of getting additional diseases like hyperglycemia and diabetes. The group of people who are possible affected with these additional diseases are characterized by the FDA as "the general population". As a next remark i hereby want to point out that the legal procedure by which the free admittance into psychiatric care of Mr. A.C.J. Hoogenboom has been transformed into a legal preservation proposition by law enforcement, is rather shoddy to put it mildly. On October 17, 2002 Hoogenboom was taken into psychiatric care by his free admittance into the p/a Zon & Schild facility, dept. Amergaard.
So is written inside a letter from RIAGG (the coordinating dutch body for psychiatric care) to general practitioner B.E. Spelberg at October 18, 2002, subject Psychiatrie/2822210/ck, the following : "Patient was taken by free admittance on October 17, 2002 into the Symforagroup, location Zon & Schild. Because soon after arrival at Z&S the client kept demanding his resignation from treatment, a judgement was held by undersigned and a IBS (legal preservation proposition) was delivered by the court." As was signed by : Ms J.C.S. van den Berg, social psychiatric nurse and Mr. J. Wernand, psychiatrist. (patient was seen on Friday October 18, 2002 at 12:48h) Inside a previous letter from RIAGG to general practitioner B.E. Spelberg at October 17, 2002, subject Psychiatrie/2822210, dept. crisis-service however something completely different was written : "Findings/psychiatric research: a 59 year old, somewhat thin man, at the moment rather quiet. The panic feeling has totally disappeared. According client this could be the case because he took 5 tablets of Seroxat, a so-called prozac variant, during the last 24 hours and had been taking some cannabis. The total bizar feeling has now disappeared and was not troubled anymore by suicidal feelings or behavior. According his roommate, client was having strange experiences over the last 6 months. Orientation is good, memory is intact. Client seems theatrical though. Conclusion: 59 year old man who, possibly under influence of to much Seroxat + cannabis, experienced strange feelings. Possibly we can conclude a mild psychotic situation picture, which the man violently denies. Client is quiet now. Policy: For the night people are present inside the house. Client feels safe with this. Urgent advise is given to not take Seroxat in combination with cannabis. Client was advised to present his case with Riagg for further diagnostic research. Client agrees to do so. Client can make agreement for tonight that nothing will happen. Medication advise (eventual administered medication) : For tonight 2 tablets of Oxazepam 10 mg." As was signed by Ms J.M.A. Janssen, manager Psychiatry, co-ordinator psychiatric help line at October 17, 2002. (patient was seen on October 15, 2002 at 17:00h) FYI : http://www.psyweb.com/Drughtm/serax.html : "Oxazepam ( Serax ) is an antianxiety agent ( benzodiazepines. ) Used primarily for treatment mild to moderate anxiety. It may also be used to treat symptoms of alcohol withdrawals or alcohol addiction."
The next day, Friday October 18, 2002 at 11:33h a fax is sent from Zon & Schild (Dept. Amergaard, from fax-number 033-4695455) to lawyer Mr. P.J.M. van Galen from Kubatsch Oerlemans - Van Galen, lawyers and solicitors in order to present a IBS (a legal preservation proposition) to the court in Utrecht, by decree of the Mayor of Amersfoort, A. van Vliet-Kuiper. What did happen then in the night of October 17 to October 18, 2002? Well as was written into this fax to the court house by Mr. J.J. Wernand, psychiatrist at the town of Amersfoort, the following had happened :
"4a. On the basis of which symptoms, behavior and facts does serious suspect arise with yourself that the person concerned is suffering from impairment of mental capabilities? Person Concerned (PC.) has been investigated this morning in the house environment by undersigned concerning increased delusions, paranoid imaginations and restlessness. Since a few days PC. is convinced he will be the victim of a (reckless) gang. Together with his partner he is planning to escape to Canada, make himself unrecognizable with the aid of plastic surgery in order to prevent a certain death. He is hearing voices and suspects his upper neighbours of influencing his brains. There was some self-awareness. 4b. Which behavioral conduct and facts (as mentioned inside question 4a) are not observed by yourself, but were informed to you by others? (indicate clearly by whom this was informed.) PC. agreed with a voluntary hospitalization inside Z&S. But at present he disagrees with this and has repeatedly demanded his resignation and has asked for 24 hour police surveillance. 4c. At what preliminary diagnosis have you arrived according your research? Paranoid psychotic condition, possibly combined with high dosis of Seroxat and cannabis abuse. 5a. Based on what behavioral conduct by PC., does serious suspect arise that the disorder of mental capabilities create a clear and present danger for the person involved, for others or for the general safety of persons or properties? PC. feels a permanent threat from paranoid experiences and hallucinations. As a result he becomes more afraid and dejected. Only yesterday he wanted to escape his pursuers by dropping himself in front of a train. Its not impossible PC. acted due to imperative hallucinations. 5b. What does this imminent threatening danger consist of? Suicide. 5c. Did already events occur which are a indication for the expected danger ? Last night PC. was walking with a knife inside the house. Without police protection he doesn't have the courage to go outside. 5d. What behavioral conduct and facts as mentioned inside questions 5a, 5b en 5c are not observed by yourself but were informed to you by others? 5a. Mentioned by PC. himself. 5c. Information from house-mate. 6a. On what grounds do you base your judgement that without intervention of persons outside a psychiatric care facility this danger cannot be prevented and which measures have been considered or tried in this matter? As long the psychiatric condition continues a danger is present for impulsive suicide. Adequate treatment and protection is only possible inside a clinic. 6b. What statements and suggestions should as yet be mentioned as important? Considering, as can be concluded, it concerns a first psychotic decompensation with a 59 year old man, a thorough investigation is needed to exclude organic factors” as was signed at October 17, 2002, by J.J. Wernand, Psychiatrist
(patient seen and judged on October 17, 2002 at 14:30h) (F. Kruisdijk, Psychiatrist Dept. Amergaard is presented as attending psychiatrist inside this document, and B.E. Spelberg, general practitioner as the consulted family doctor, but could not be reached) NOTE : This document was created and signed on Thursday October 17 2002 at 14:30h. The same document however is sent as a fax to the court house on Friday October 18, 2002 at 11:33h. It is tempting to conclude and strongly suggested that in the above enumeration of events the time-stamps of cause and consequence are systematically reversed. Next we mention here that the following medication, in the night of Tuesday October 15, 2002 onto Wednesday October 16, 2002, was prescribed by Ms J.M.A. Janssen, manager Psychiatrics, co-ordinator psychiatric help line : Medication advise: For the night 2 tablets Oxazepam 10 mg. According http://www.psyweb.com/Drughtm/serax.html : Oxazepam ( Symptoms or Effects ) Common: Clumsiness / Sleepiness. Rare: Abdominal cramps, blurred vision, dry mouth, racing heartbeat / palpitations, shaking / slurred speech, urination problems, seizures, hallucinations, memory loss, trouble breathing, staggering / trembling, headache or confusion. See physician always: Abdominal cramps, blurred vision, dry mouth, racing heartbeat / palpitations, shaking / slurred speech, urination problems, seizures, hallucinations, memory loss, trouble breathing, staggering / trembling, headache or confusion. See physician if severe: Clumsiness / Sleepiness. See physician NOW: Confusion or seizures. Stop taking and see physician NOW: Convulsions, hallucinations, memory loss, trouble breathing or staggering / trembling.
If you want to have someone forget the time-stamps of important events, Oxazepam (Serax) appears to be an excellent medication for this purpose. Next the following observations are made by the undersigned : There are alternative witness testimonies about dr. Wernand, psychiatrist, which state that he was being used to have persons placed under governmental observation, using documents which contain similar contents of the fax outlined above. The court then unthinking accepts these fax requests, which only after 3 weeks are then made undone. The court result is being kept secret for the brought in patient so he can stay a little longer. Alternative behavior conducted by personnel of Dept. Amergaard may not be mentioned. A requested conversation with dr. Frank van Kruidijk from the Zon & Schild facility has been sabotaged by an unknown medicine prescriber. Note that dr. Frank van Kruidijk was mentioned as attending psychiatrist inside Zon & Schild for patient A.C.J. Roomer in October 2002. Ms. drs. E.P. Ten Have, neurologist with dokters.nl and Meander, has mentioned alternatively that the attending psychiatrist and medicine prescriber of the refusing student Hoogenboom actually was A. Steen, psychiatrist, with an unknown address and where abouts for his clients.
She invited his partner drs G.J. Hoogenboom and tells him alternatively that he (A. Steen) is having severe troubles with student Hoogenboom who apparently was giving him hard times. This was informed to her by Mr. A Steen (psychiatrist). This information was presented without giving a single notion or word to student Hoogenboom. He is not welcome anymore. An alternative witness testimony from a lawyer from Zeist who states that the Dept. Amergaard takes measures when a lawyer visits the clinic, by putting the patient out by injection for longer periods of time. He consequently won't accept expatients anymore from Dept. Amergaard. An appointment which has not been met, about the sending of the investigative report by dr. Nijboer, psychologist and his assistant to student Hoogenboom. An appointment made by telephone with drs. Breetveldt on 11/05/2005 versus a written appointment made with the secretary on 24/05/2005. By pursuing this kind of alternative behavior by the ladies and gentlemen of the medical discipline, student Hoogenboom is severely harmed in his functioning and is forced to undergo an alternative permanent sound-treatment for the coming time. Undersigned declares not be be schizophrenic and is not willing to accept this under pressure. Undersigned will remain patient against his will en is pointing to a ruling of the High Court of the Netherlands that he does not find himself a patient of dr. Kahn en certainly cannot be forced to confess otherwise or undergo a treatment outside the facility. I note that the precarious and dubious side of these psychiatrists, as school of medication has not been touched upon. Undersigned, A.C.J. Hoogenboom, Student Psychiatry, finally comes to the following conclusions and recommendations : – – – –
His complaint is in particular interpreted by the professional servants as false and is put away as being mentally ill. An open investigation into the cause of hospitalization and the methods how this was being setup, is highly needed. An open investigation into alternative practices inside psychiatry, where undersigned clearly has been victim of, is in this matter unavoidable. Two independent American tests point to the result "NO VERDICT" in the field of schizophrenia.
Undersigned feels deeply damaged in his dignity. Yours Sincerely, drs A.C.J. Hoogenboom, student Psychiatry UMC Utrecht.
