Nanobody-based targeting of tissue macrophages for inflammation monitoring during experimental arthritis and hepatitis

Summary Imaging techniques play a crucial role in the diagnosis and follow-up of many diseases. However, for several, especially immune related, pathologies, there is still a paucity of information on the cells involved in the disease process or molecular events happening in and around inflammatory foci. Thus, the capacity to specifically image certain populations of immune cells can greatly benefit prognosis determination and/or decisions on treatment regimens. Our work focuses on tracking the highly plastic macrophage cells in vivo during inflammatory responses. These cells play a crucial role in many immune responses and in maintaining tissue homeostasis. To this end, two macrophage specific membrane receptors were selected: V-set and immunoglobulin domaincontaining 4 (Vsig4) and C-type lectin domain family 4, member F (Clec4f). In order to track these markers high-affinity Nanobody (Nbs) agents were developed that bind to Vsig4 or Clec4f. These camelid derived single-domain antigen-binding antibody fragments were shown before by the Cellular and Molecular Immunology lab to be excellent tools for molecular tumor and inflammation imaging. Real time PCR and immunoflouresence histochemistry confirmed Vsig4 expression in the synovia of mice, in addition to its abundant presence on Kupffer cells (KCs). SPECT/µCT imaging with

Tc-NbV4m119 demonstrated specific targeting of Vsig4+ liver and synovium
macrophages in wild type but not in Vsig4-/- mice. In mice with collagen induced arthritis (CIA) 99m

Tc-NbV4m119 can specifically detect inflamed joints in multiple
arthritis models, including the methylated bovine serum albumin induced arthritis model and a modified collagen induced arthritis model. In a preliminary therapeutic monitoring experiment,
SPECT/CT imaging quantification of joint inflammation in CIA mice using
99m
Tc-NbV4m119
yielded a clear decrease of Vsig4 signal in dexamethasone (DXM) treated mice as compared to saline treated mice. Yet, the 99mTc-NbV4m119 accumulation in knees remained high after DXM treatment, correlating with a temporary inhibition of symptoms in the extremities and the observation that the clinical symptoms in the paws returned immediately when the DXM treatment stopped. Overall, these results offer perspectives for using in vivo imaging via Nbs targeting Vsig4 for non-invasive monitoring of arthritic lesions in the context of early diagnosis and therapy efficacy evaluation, thereby offering complementary information that goes beyond the current clinical scoring system and which may thus at least partially reduce the need for extensive terminal examination by histopathology. During whole body imaging, DXM treatment extensively down regulated the Vsig4 signals in the liver, irrespective of the presence of CIA in the animals. We therefore investigated if the expression of KC specific markers can be used to non-invasively monitor liver inflammation or damage. To this end we included Nbs targeting Clec4f in this study, a marker reported to be abundantly and specifically expressed on KCs, in addition to the anti-Vsig4 Nbs. Nbs and

The Nbs targeting Vsig4 and Clec4f thus offer perspectives for non-invasive monitoring of KCs in the liver, thereby distinguishing KCs from other myeloid cells such as monocytes and neutrophils. In summary, we generated and started to validate anti-Vsig4 and anti-Clec4f Nbs as valuable, quantitative, specific tools for the non-invasive in vivo monitoring of tissue macrophages, thereby providing molecular and cellular information for tracking the inflammation progression in preclinical models of arthritis and hepatitis.
Samenvatting Medische beeldvormende technieken spelen een cruciale rol in de diagnose en opvolging van vele aandoeningen. Voor verscheidene pathologieën, immuungerelateerde in het bijzonder, is slechts beperkte informatie beschikbaar over de specifieke cellen die betrokken zijn bij het ziekteproces of bij de moleculaire gebeurtenissen in en rond de inflammatoire foci. De mogelijkheid om specifiek bepaalde immuuncelpopulaties in beeld te brengen, kan bijgevolg significant bijdragen aan de bepaling van prognoses en/of de behandeling van deze aandoeningen. Dit/ons werk focust op het in vivo opvolgen/in kaart brengen van de zeer plastische macrofagen gedurende de inflammatoire reacties. aangetoond in dexamethasone (DXM) behandelde muizen ten opzichte van de met zoutoplossing behandelde muizen. Zelfs na DXM behandeling bleef de

Tc-NbV4m119 accumulatie in de
knieën hoog, wat correleert met een tijdelijke symptoominhibitie in de extremiteiten en met de observatie dat de klinische symptomen in de poot onmiddellijk terugkeerden na de stopzetting van de DXM behandeling. Globaal bieden deze resultaten dan ook mooie perspectieven voor het gebruik van in vivo imaging middels Vsig4 specifieke Nbs voor niet-invasieve monitoring van artritische laesies in het kader van vroege diagnose en evaluatie van de behandelingsefficiëntie. Bovendien kan zo complementaire informatie worden geboden die het huidig klinisch scoring systeem overstijgt en als dusdanig de nood aan uitgebreid onderzoek via histopathologie op zijn minst partieel reduceert. Tijdens de whole body imaging werd opgemerkt dat de Vsig4 signalen in de lever significant afnamen door de DXM behandeling, ongeacht de aanwezigheid van CIA in de dieren. Daarom werd onderzocht of de expressie van KC specifieke merkers gebruikt kan worden voor noninvasieve monitoring van leveronsteking of –schade. Daarom werden in deze studie naast de anti-Vsig4 Nbs ook Nbs geincludeerd gericht tegen Clec4f, een merker beschreven overmatig en specifiek tot expressie te komen op KCs. Door middel van SPECT/CT kon worden aangetoond dat zowel 99mTc-Vsig4 en 99mTc-Clec4f Nbs accumuleerden in de lever van naïeve muizen. Flow cytometrische en immunohistochemische analyses demonstreerden dat anti-Clec4f en anti-Vsig4 Nbs bonden aan KCs, maar niet aan andere celtypes in de lever. In een muismodel van acute leverschade, geïnduceerd door injectie van Concanavaline A, daalden de in vivo imaging signalen voor
99m
Tc-Vsig4 en
99m
Tc-Clec4f Nbs na 6 tot 24u en herstelden deze zich 48u na de
injectie, wat correleert met een afname van KC aantallen en een transiënte modulatie van Clec4f en Vsig4 expressie op KCs.
