Vnímání některých odchylek v estetice frontálního úseku chrupu laiky Esthetics of frontal teeth according the perception of lay people

Hodnoceny byly tyto odchylky: symetrické a asymetrické zúžení horních laterálních řezáků, otevřený gingiválníprostor mezi horními středními řezáky a symetricky i asymetricky snížená úroveň marginálnígingivy horních středních a laterálních řezáků. Úsměv 28leté ženy byl upravován v počítačovém programu Adobe Photoshop 7.0; 30 panoramatických fotografií ve formátu 9x13 mm bylo náhodně rozmístěno do alba stejného formátu. Každý hodnotitel v dotazníku vyznačil na úsečce míru atraktivity každého úsměvu a uvedl svůj věk a pohlaví. Dotazník vyplnilo 120 žen a 65 mužů ve věku nad 18 let. Za méně atraktivní pokládala nadpoloviční většina dotazovaných laiků tyto stupně odchylek: symetrické a asymetrické zúžení horních laterálních řezáků o 4 mm, 5 mm dlouhý otevřený gingivální prostor mezi horními středními řezáky, symetricky sníženou úroveň marginální gingivy horních středních řezáků o 3 mm, jednostranně sníženou úroveň marginální gingivy horního středního řezáku, symetricky i jednostranně sníženou úroveň marginální gingivy horních laterálních řezáků o 2 mm (Ortodoncie 2006,15, č. 4, s. 39-49). Abstract The aim of the study was to identify the degree of some discrepancies in the esthetics of the frontal segment of dentition that are considered less attractive by majority of respondents. The following anomalies were monitored: symmetrical as well asymmetrical narrowing of maxillary lateral incisors, open gingival space between maxillary central incisors, symmetrically as well asymmetrically lowered level of margin gingiva of maxillary central and lateral incisors. The smile of a 28-year old female was modified in Adobe Photoshop 7.0; 30 panoramic pics of the format 9x13 mm were randomly placed in the album of the same format. In the questionnaire each of the respondents marked on the line segment the degree of attractiveness of each particular smile, and put down their age and sex. The questionnaire was filled up by 120 females and 65 males of the age over 18. More than 50% of the respondents considered less attractive the following degrees of anomalies: symmetrical as well as asymmetrical narrowing of maxillary lateral incisors by 4 mm, 5 mm long open gingival space between maxillary central incisors, symmetrically lowered level of marginal gingiva of maxillary central incisors by 3 mm, unilateral lowered level of marginal gingiva of the maxillary central incisor, symmetrically and asymmetrically lowered level of marginal gingiva of maxillary lateral incisors by 2 mm (Ortodoncie 2006,15, No. 4, p. 39-49). obvykle způsobeny ztrátou kosti, divergencí kořenů, tvarem zubů nebo kombinací těchto příčin [4, 7]. Důležitým znakem estetického úsměvu je křivka (oblouk) úsměvu. Je definována jako vztah křivky řezacích hran horních řezáků, špičáků, premolárů a molárů ke křivce dolního rtu při sociálním úsměvu. U ideálního oblouku úsměvu jsou tyto dvě křivky při úsměvu paralelní (konsonantní). Nekonsonantní neboli plochý úsměv je charakterizován přímou křivkou řezacích hran vzhledem ke křivce dolního rtu. Přímá či dokonce obrácená linie úsměvu přispívá k atraktivitě obličeje podstatně méně. Obrácená linie navíc bývá často spojena s výraznou abrazí horních řezáků. Křivka incizálních hran se zdá být více vyjádřena u žen než u mužů a v průběhu stárnutí se oplošťuje [5, 6, 8]. Příjemný úsměv také závisí na kvalitě a „kráse" zubů a na jejich harmonické integraci. Úsměv je primárně charakterizován architekturou horních frontálních zubů. Dolní frontální zuby mohou však hrát také důležitou roli, zvláště u mužů astarších jedinců. Přítomnost nepravidelných incizálních hran, abradovaných zubů a nevyváženost v morfologii zubů, mohou limitovat naši možnost vytvořit estetický výsledek. Incizální hrany postižené abrazí postrádají běžnou konturu. Na jedné straně ztrácí zub abrazí své typické rozměry, na druhé straně se ztrácí pozitivní průběh linie úsměvu [8, 9]. Dentálnístředníčára je důležitým centrálním bodem estetického úsměvu. Dosažení souladu středních čar zubních oblouků s obličejovou střední čárou je jedním z cílů ortodontické léčby. Souhlasné středy jsou důležitou složkou funkčníokluze a umožňují klinikovi dobré sledování bukáni interdigitace, zakousnutí. Mírná diskrepance hornístředníčáry je přijatelná, pokud je kontaktní aproximální plocha (konektor) středních řezáků vertikální [8,10]. Angulace korunek je důležitým faktorem estetického výsledku léčby. Mírná meziální inklinace všech zubů působí velmi esteticky. Pokud je korunka horního řezáku distálně inklinovaná, ruší to paralelitu incizálních hran horních řezáků s konturou dolního rtu. Při výrazné meziální angulaci vznikne mezi řezáky tmavý interdentální prostor, který nemůže gingivální papila vyplnit. To může působit i u jinak estetického výsledku rušivě [4,11]. Střední řezáky mívají často tvar trojúhelníkovitý, který není příznivý. Často je třeba jej korigovat, abychom eliminovali jednobodový kontakt těchto zubů. Nejčastěji udávaný poměr délky a šířky středního řezáku je asi 80 % šířka v porovnánís jeho délkou, s udávaným rozmezím 66-80 % [12,13]. central incisors may be present. Open gingival space is usually the result of bone loss, roots divergence, shape of teeth or combination of the above mentioned [4, 7]. The curve (arc) of a smile is an important feature of the esthetic smile. The curve is defined as the relation of the curve of incisal edges of maxillary incisors, canines, premolars and molars to the curve of lower lip in a social smile. In the ideal smile arc the two curves run parallel (they are consonant). Non-consonant or the so called flat smile is characterized by the direct curve of incisal edges to the curve of lower lip. Direct or even reversed line of a smile contributes much less to the attractiveness of a face. Reversed line is often accompanied with significant abrasion of maxillary incisors. The curve of incisal edges seems to be more expressed in women than in men, and during aging it flattens. [5, 6, 8]. An esthetically pleasing smile depends also on the quality and ,,beauty" of the teeth as well as on their harmonic integration. A smile is primarily characterized by the architecture of maxillary frontal teeth. However, lower frontal teeth may also play an important part, especially in men and elderly people. Irregular incisal edges, their wear and the imbalance of teeth morphology may restrict our potential to achieve an esthetic result. Incisal edges affected with the wear lack the common contour. On the one hand the tooth loses its typical dimensions due to attrition, on the other hand the positive run of a smile is lost [8, 9]. Dental midline is an important central point of a pleasing smile. One of the objectives of orthodontic treatment is to achieve conformity of dental arches midlines and facial midline. Conforming midpoints are significant parts of functional occlusion and make it possible for the clinician to control the buccal interdigitation, occlusion. Moderate discrepancy of maxillary midline is acceptable in case the contact approximal surface of central incisors is perpendicular [8,10]. An important factor contributing to the esthetic result of the treatment is a crown angulation. Moderate mesial inclination of all the teeth looks very esthetic. If the maxillary incisor crown inclines distally, it disturbs the parallel run of incisal edges of maxillary incisors and lower lip contour. In distinct mesial angulation there occurs dark interdental space between the incisors that cannot be filled in with gingival papilla. This may significantly disturb the otherwise esthetic treatment result [4,11]. Central incisors resemble frequently a triangle, the shape is not favorable. Therefore, it is often necessary to correct the shape in order to eliminate one-point contact of the teeth. The most frequently given proportion of the length and width of central incisor is about 80% (width: length), with the interval of 66-80% [12, 13]. A short clinical crown may be the result of incisal wear or excessive gingiva. If maxillary incisors in the 41

nebo vůbec, délka klinických korunek může být incizálně zvětšena estetickou stomatologií (kompozitní dostavba, fazety apod.) [4, 6, 8]. Často můžeme pozorovat odchylky ve tvaru horních laterálních řezáků. Malformované laterální řezáky mají obecně dva různé tvary. Jedním je čípkovitý tvar. Jiné majítvar normálního řezáku, ale jsou zřetelně užší, tenčí a kratší. Je-li laterální řezák oboustranně lehce užší, můžeme se rozhodnout, že nebudeme vytvářet místo pro dostavbu. Je-li diskrepancejen lehká, může být vliv na okluzi a estetiku nepozorovatelný [7].

Discrepancies in the shape of maxillary lateral incisors are often seen. Malformed lateral incisors have usually two different shapes - peg shaped or a normal incisor shape, but narrower, thinner and shorter. In case a lateral incisor is bilaterally slightly narrowed, we may decide to make no place for build-up. If the discrepancy is just moderate, its impact on occlusion and esthetics is unnoticeable. [7]

Otevřené qinqivální prostory mezi horními středními řezáky Otevřené gingivální prostory mezi horními středními řezáky byly měněny k simulaci odchylek tvaru zubní korunky. Byla zachována přirozená interdentální papila. Aproximální bod kontaktu byl po 1 mm posunován incizálně, v rozsahu od 1 mm do 6 mm. Měnící se výška otevřeného gingiválního prostoru byla měřena od vrcholu interdentální papily k aproximálnímu bodu kontaktu (Obr. 5). Úroveň marginální qinqivy horních středních a laterálních řezáků Délka klinické korunky byla zmenšována cílenou změnou výšky marginální gingivy. V případě horních středních řezáků byla měněna oboustranně i jednostranně (vpravo) po 1 mm, v rozsahu od 1 mm do 4 mm. V případě horních laterálních řezáků byla měněna oboustranně i jednostranně (vpravo) po 1 mm, v rozsahu od 1 mm do 3 mm (Obr. 6,7). Referenčním bodem pro měření byl nejvyšší bod na labiální části marginální gingivy horních středních a laterálních řezáků. Úroveň incizální hrany byla zachována. Album Celkem bylo zhotoveno 30 panoramatických fotografií ve formátu 9x13 mm, které byly s výjimkou první a poslednífotografie náhodně rozmístěny do alba stejného formátu. Jako první a samostatně hodnocená byla fotografie původního nezměněného úsměvu. Další pozměněné fotografie byly pozorovány po dvou,

Open gingival space between maxillary central incisors Open gingival space between maxillary central incisors was modified to simulate deviations in the shape of the crown. A natural interdental papilla was maintained. The approximal point of contact was moved incisally by 1 mm, within the range of 1 mm - 6 mm. The changing height of the open gingival space was measured from the top of the interdental papilla to the approximal point of contact (Fig. 5). Level of marginal gingiva of maxillary central and lateral incisors The length of a clinical crown was altered by pointed change of the height of marginal gingiva. In case of maxillary central incisor it was altered both bilaterally and unilaterally (on the right) by 1 mm, within the range of 1 mm - 4 mm. In case of maxillary lateral incisors it was altered both bilaterally and unilaterally (on the right) by 1 mm, within the range of 1 mm - 3 mm (Fig. 6, 7). The point of reference for measurement was the highest point in the labial portion of marginal gingiva of maxillary central and lateral incisors. The level of incisal edge remained unchanged. Album 30 panoramic photographs of 9 x 13 mm format were made; they were placed in the album of the same format randomly (with the exception of the first and the last photograph). The first photograph which was evaluated independently was the photograph of the original unaltered smile. Other photographs were viewed

tak jak byl album otevřen. Závěrečná fotografie, opět samostatná, byla opět původní nezměněná fotografie. Dotazník Každý hodnotitel obdržel k albu dotazník ve formátu A4, kde uvedl v záhlaví svůj věk a pohlaví. Dále vyznačil křížkem v tabulce, jakse mu očíslovaná fotografie líbí či nelíbí. Tabulka obsahovala třicet 50-ti milimetrových měřítek, na obou koncích označených protipóly atraktivity (líbí na levém okraji, nelíbí na pravém okraji) a střední čárkou, označených číslem hodnocené fotografie. Jako méně atraktivní byly vyhodnoceny fotografie, u kterých hodnotitel umístil křížek za střední čárku odpovídající 2,5 mm. Hodnotitelé Vtéto studii byla použita jedna skupina hodnotitelůlaiků. Každý hodnotitel dostal o studii tak málo informací, jak jen to bylo možné. Dotazníky byly rozdávány doprovodu pacientů v čekárně ortodontického oddělení Stomatologické kliniky VFN v Praze. Dále s laskavou pomocí příbuzných a přátel, kteří rozdali dotazníky ve svém okolí a na pracovištích. Celkem bylo vyplněno 185 dotazníků, 120 ženami a 65 muži ve věku od 18 let. Statistické vyhodnocení Studie byla statisticky vyhodnocena Chí-kvadrát testem. Mezi hodnocením muži a ženami a mezi různými věkovými skupinami nebyl Studentovým t-testem nalezen statisticky signifikantní rozdíl.

The table included thirty 50 mm scales, with two antipoles of attractiveness ("like" on the left, "dislike" on the right) and a midline; photographs were numbered. The photographs in which the respondent put the cross beyond the midline corresponding to 2.5 mm were considered as less attractive. Evaluators/respondents One group of lay respondents was involved. Each respondent got as little information about the study as possible. The questionnaires were handed out to those accompanying the patients in the waiting room of the Orthodontic Department of the Clinic of Dental Medicine, University Hospital in Prague. Our relatives and friends were kind enough to spread the questionnaires in their neighbourhood and at workplaces. Overall, 185 questionnaires were filled in, 120 by female respondents, 65 by males, the lay people over 18 years of age. Statistical evaluation The study was statistically processed with the Chisquare test. Student t-test did not prove any statistically significant difference between males and females, and within various age groups.

Diskuse V posuzování estetického výsledku léčby bývají často ortodontisty negativně vnímány otevřené gingivální prostory (tzv. černé trojúhelníky). Tato studie však zjistila, že teprve 5 mm dlouhý otevřený gingivální prostor považuje přibližně polovina dotazovaných za méně atraktivní. Ve studii Kokiche jr. považovali hodnotitelé za méně atraktivní již 3 mm dlouhý tento prostor [14]. Zde je třeba podotknout, že obě studie simulovaly posun kontaktu horních středních řezáků bez parodontálního problému. Kokich aplikoval pravidlo zlatého poměru v ortodoncii při popisu správné dostavby čípkovitých laterálních řezáků u ortodontických pacientů. Hodnota zlaté proporce pro tento zub je 0,618 neboli přibližně 2/3 šířky sousedního středního řezáku [7]. V této studii bylo ve shodě se studií Kokiche jr. zjištěno, že teprve odchylku 4 mm v šířce laterálních řezáků považuje za méně atraktivní nadpoloviční většina laiků [14]. Symetrickou změnu u obou laterálních řezáků hodnotili dotazovaní hůře než změnu jednostrannou. Tyto informace mohou být užitečné při rozhodování, jak velké místo otevřít pro dostavbu laterálního řezáku, i když názor pacienta a situace v dutině ústní bude rozhodujícím faktorem. Zajímavé byly výsledky hodnocení úrovně marginální gingivy a tím délky klinických korunek. Je třeba si uvědomit, že náš model při úsměvu beze změny odhaloval celou délku klinických korunek středních řezáků a tyto výsledky lze tedy aplikovat na pacienty se střední a vysokou linií úsměvu. Posun marginální gingivy laterálních řezáků o 2 mm jednostranný i symetrický hodnotila většina laiků jako neestetický. Symetrickou změnu hodnotila hůře, zřejmě pro větší celkové množství viditelné gingivy. Symetrický posun marginální gingivy horních středních řezáků o 3 mm hodnotilo negativně téměř 90% dotazovaných, podobně negativně hodnotili laici jednostranný posun úrovně marginální gingivy horního středního řezáku, a to již o 2 mm. Vyšší citlivost dotazovaných k jednostranné změně je zřejmě důsledkem toho, že byla tato změna lokalizována blíže střední čáry, proto je více nápadná než jednostranná změna u laterálních řezáků. Ve studii Kokichejr. nedokázali dotazovaní laici rozlišit změnu úrovně marginální gingivy u laterálních řezáků, změny byly prováděny jen do 2 mm. U středních řezáků považovali změnu o 2 mm za méně estetickou. Jednostranné změny prováděny nebyly [14]. Dále nebyly nalezeny rozdíly v hodnocení estetických odchylek ve frontálním úseku chrupu mezi muži a ženami, kde bychom předpokládali, že budou ženy ke změnám citlivější. Discussion Orthodontists frequently consider open gingival space (the so called "black triangles") as unfavourable in the evaluation of the esthetic treatment result. However, this study found out that only 5 mm long open gingival space was considered less attractive, by approximately 50% of respondents. In the study of Kokich Jr. respondents considered less attractive the space of 3 mm [14]. We have to remarkthat both studies carried out the simulation of the movement of maxillary central incisors contact without periodontal problem. Kokich applied the golden rule of the proportion in orthodontics in description of correct build-up of peg shaped lateral incisors in orthodontic patients. The value of the golden proportion is 0.618 or approx. 2/3 of the width of adjacent central incisor [7]. In this study, in agreement with the study by Kokich Jr., it was found out that only the deviation of 4 mm in the width of lateral incisor is considered less attractive by more than 50% of respondents [14]. Symmetrical deviation in both lateral incisors was considered worse than the asymmetrical one. The information may be useful when it is to be decided what space to open forthe lateral incisor build-up. However, the patient's view and the situation in the oral cavity will be the most decisive factors. The results of evaluation dealing with the level of marginal gingiva, and thus the length of clinical crowns, were rather interesting. Our model showed in smile all length of clinical crowns of central incisors and therefore the results may be applied for the patients with a high line of smile. Most respondents considered both asymmetrical and symmetrical shift of marginal gingiva of lateral incisors by 2 mm as unesthetic. Symmetrical alteration was evaluated worse, most probably because of greater amount of gingiva visible. Symmetrical shift of marginal gingiva of maxillary central incisors by 3 mm was regarded as unesthetic by almost 90% of respondents. Similarly the unilateral shift of the level of marginal gingiva of maxillary central incisor was considered bad, even at 2 mm. Asymmetrical alteration was located near to midline, therefore, it is most striking than unilateral alteration of lateral incisors. In the study by Kokich Jr. respondents were not able to distinguish the change in the level of marginal gingiva in case of lateral incisors; the modifications were made only up to 2 mm. In case of central incisors they considered the modification by 2 mm esthetically unfavourable. There were made no asymmetrical alterations [14]. There were found no differences in the evaluation of esthetics of front segment of dentition between men and women, though we would assume women to be more sensitive to the modifications made.
