Please submit the documents requested. Where original documents are specifically requested, originals must be submitted with your application. You should keep a certified true copy of any originals you send us for your records. Some countries require original documentation which will not be returned to you. You may submit the original or a photocopy that is certified as true for any of the documents where originals are not required. Original documents will be returned to you promptly. If you submit the photocopies of documents, they must be certified by: an accountant, chiropractor, Commissioner of Oaths, dentist, doctor, employee of a federal or provincial government department or one of its agencies, funeral director, Justice of the Peace, lawyer, magistrate, manager of a financial institution, member of parliament or of a provincial legislature or their staff, minister of religion, municipal clerk, notary, official of a country with which Canada has a social security agreement, official of an embassy, consulate or high commission, pharmacist, police officer, postmaster, professional engineer, social worker or teacher. GUIDE The following benefits may be applied for: Old Age Pension, Full Invalidity Pension, Partial Invalidity Pension, Survivor´s Pension and Orphan´s Pension. The Application is to be sent to the Czech Republic Social Security Administration, Křížová 25, 225 08 Praha 5, through the International Operations Division of the Canadian Department of Human Resources Development, which will include a statement concerning creditable periods in Canada. Please type or print when completing the application. Certified copies of documents are to be enclosed with the application. To speed up the processing, we recommend enclosing a certified translation into Czech. Creditable periods in Canada will be certified by the International Operations Division of the Department of Human Resources Development Canada. If you have not been assigned a Birth Registration Number, leave the section blank. Always give your surname at birth and all previous surnames, even if you are male. In the List of Employment, Insurance Periods and Substitute Insurance periods, include work performed in the Czech Republic and in Canada as well as in other countries (including the name of the country). The list of Employment Periods may be continued on an additional sheet. An orphan of 18 years of age or older applies for an Orphan´s Pension on a separate application form under his/her Birth Registration Number. A parent applies for an Orphan´s Pension on behalf of orphans under 18 years of age. Where an orphan applies for an Orphan´s Pension after both parents have died, two applications must be completed (a legal guardian applies on behalf of an orphan under 18 years of age). The section pertaining to the continuation of the marriage and entering into a subsequent marriage are to be left blank in an application for an Orphan´s Pension. When applying for Survivor´s Pension, give only information concerning the deceased in the section List of Employment, Insurance Periods and Substitute Insurance Periods. Enclose certified copies of the following documents with your pension application: - your birth certificate - education certificates, even if the course of study was not completed - certificates of military and civilian service in the Czechoslovak Army - birth certificates of your natural and adopted children as well as children you had in foster care - court decree for adopted children - your personal Pension Security (insurance) document, if one has been issued to you by the Czech Social Insurance Administration - certificates confirming periods of employment in other countries (certification of creditable periods in Canada will be provided by the International Operations Division of the Department of Human Resources Development Canada) Enclose the following with an application for Survivor´s or Orphan´s Benefits: - marriage certificate or an extract from marriage register (only for Survivor´s Pensions) - death certificate of the deceased spouse (parent) - birth certificates of children; where the children completed compulsory school attendance, enclose also a certificate of attendance of a course of study or vocational training and any other documents proving that the child is dependent (medical certificates) Documents enclosed with a previous application do not need to be provided again. P POUČENÍ Žádost lze uplatnit o tyto druhy důchodů: starobní, plný invalidní, částečný invalidní, vdovský, vdovecký a sirotčí. Žádost se zasílá České správě sociálního zabezpečení, Křížová 25, 225 08 Praha 5, prostřednictvím Divize mezinárodních operací Ministerstva pro rozvoj lidských zdrojů v Kanadě, která připojí přehled o dobách pojištění v Kanadě. Žádost doporučujeme vyplnit strojem nebo hůlkovým písmem. K žádosti se dokládají ověřené kopie dokladů. Pro urychlení řízení doporučujeme ověřený překlad do českého jazyka. Započitatelné doby v Kanadě budou úředně potvrzeny Divizí pro mezinárodní operace oddělení rozvoje lidských zdrojů Kanada. Doklady ověřují kanadské úřední osoby. Údaj o rodném čísle se nevyplňuje, pokud Vám toto číslo nebylo přiděleno. Rodné jméno i všechna dřívější používaná příjmení se vyplňují vždy, a to i u mužů. Do přehledu v činnostech a náhradních dobách vyplňte nejen údaje o činnostech vykonávaných na území České republiky a v Kanadě, ale i údaje o činostech na území jiných států (včetně názvu státu). Ve výčtu dob zaměstnání lze pokračovat na samostatně vloženém listu. Sirotek starší 18 let uplatňuje nárok na sirotčí důchod samostatnou žádostí pod svým rodným číslem. U sirotka mladšího 18 let je žadatelem žijící rodič. Uplatňuje-li sirotek nárok na sirotčí důchod po obou rodičích, je nutno sepsat dvě žádosti (u sirotka mladšího 18 let uplatňuje nárok zákonný zástupce - opatrovník). V žádosti o sirotčí důchod se nevyplňují prohlášení o trvání manželství a uzavření nového manželství. V žádosti o pozůstalostní důchod v oddíle "Přehled o činnosti a náhradních dobách .." uveďte pouze údaje o zemřelé(m). K žádosti o důchod se předkládají ověřené kopie těchto dokladů: - Váš rodný list - doklad o studiu i nedokončeném - o vojenské, civilní službě v čs. Armádě - rodné listy dětí vlastních, osvojených nebo které jste převzal(a) do péče nahrazující péči rodičů - u osvojených dětí rozhodnutí soudu - osobní list důchodového zabezpečení (pojištění), pokud Vám byl Českou správou sociálního zabezpečení vydán - osvědčení započitatelných dob v Kanadě bude opatřeno Divizí pro mezinárodní operace oddělení rozvoje lidských zdrojů v Kanadě K uplatnění nároku na vdovský, vdovecký, sirotčí důchod, kromě dokladů o zemřelé(m) (viz níže) předložte: - oddací list, příp. výpis z matriky oddaných (pouze u žádosti o vdovský - vdovecký důchod) - úmrtní list manžela(ky) (rodiče dítěte) - rodné listy dětí, v případě, že skončily povinnou školní docházku, potvrzení o studiu, even. učení, příp. další doklady, prokazující nezaopatřenost dítěte (zdravotní stav). Pokud již byly některé doklady přiloženy k dřívější žádosti, není třeba je dokládat. Osvědčení dle zák.č.255/1946 Sb. (osvědčení týkajícíse účasti v odbojovém hnutí) je třeba doložit originálem nebo notářsky ověřenou fotokopií osvědčení.
