ANGST EN DEPRESSIE
ANGSTSTOORNIS ¡Prevalentie in Nederland §4-8% bij kinderen tussen 0 en 12 jaar §Ruim 10% bij kinderen boven de 13 jaar §Meisjes > jongens §Voornamelijk sociale en specifieke fobie
ANGSTSTOORNIS BIJ ASS ¡45-84% kinderen met ASS voldoen ook aan criteria angststoornis (Leyler et al., 2006; Murris et al., 1998) §Angst komt algemeen voor bij kinderen met ASS, maar fobieën weer veel minder, terwijl dit bij angststoornissen wel vaker voor komt
DEPRESSIE ¡Bijna 3 procent van de jongeren van 13 tot 18 jaar heeft een depressie. Meisjes hebben vaker een depressie dan jongens. En ook hierbij neemt het aantal jongeren met een depressie toe met de leeftijd.
DEPRESSIE BIJ ASS ¡Kinderen en jongeren met ASD hebben meer en ernstigere kenmerken van depressie symptomen dan gemiddeld (o.a. Gadow et al., 2005; Kim et al., 2000). ¡Depressie risico neemt toe naarmate leeftijd toeneemt (Brereton et al., 2006; Howlin et al., 2000)
DEPRESSIE BIJ ASS
¡Verschil tussen écht depressie en sombere buien! §5-13% van kinderen/jongeren met ASS behaalt depressie criteria (Leyfer et al., 2006; Simonoff et al., 2008; Green et al., 2000). §Helft van ASS-ers is echter wel chronisch ongelukkig! § Ontwikkelingsniveau speelt belangrijke rol: HF > LF
DEPRESSIE BIJ ASS
¡Kenmerken van depressie zoals slecht slapen, zijn bij kinderen/jongeren met ASS niet direct gekoppeld aan depressie, maar meer algemeen voorkomend ¡Depressed mood komt vaak voor bij ASS, maar andere kenmerken zoals gevoel van waardeloos zijn, schuldgevoel etc. komen minder frequent voor.

¡DSM IV liet weinig r uimte tot differentiaal diagnose: §Angst en depressie zou naast leer-, taal- en verstandelijke problemen, motorische problemen, ADHD en externaliserende problemen ((O)CD), evenals bepaalde medische problemen een onderdeel zijn van ASS.

FYSIOLOGIE
ANGST ¡Angst is een emotie veroorzaakt door een waargenomen of beleefde bedreiging en die meestal leidt tot een vermijding of ontwijking daarvan.
ANGST ¡Basaal overlevingsmechanisme, als reactie op een specifieke prikkel, zoals pijn of dreiging van gevaar een vecht of vlucht reactie ¡ Bij extreme vormen van angst kan verstarring optreden: men is dan als het ware ‘verlamd door angst’.
ANGST & HERSENEN ¡Meerdere gebieden in de hersenen zijn betrokken bij angst, oa.: de amygdala, de mediale frontale cortex en de hippocampus.
AMYGDALA ¡Speelt met name een centrale rol ¡Werkt als een schildwacht §Alle binnenkomende prikkels hier beoordeeld op ‘gevaar’. Zo ja, dan zet de amygdala, het lichaam in de vecht/vluchttoestand
CORTEX EN HIPPOCAMPUS ¡ Mediaal frontale cortex: Referentie kader en beoordeling ¡Hippocampus: geheugen
ANGST ¡ Nieuwe prikkels of gebeurtenissen kunnen angstsysteem in g ang zetten. ¡ Onbeg rip kan angstsysteem aanzetten gebeurtenis

STRESS SYSTEEM ¡Angst zorgt voor activatie van het stress systeem ¡Bij ‘stress’ ervaren we in de meest voorkomende gevallen een gevoel van ‘opwinding’; hart g aat sneller kloppen, bloeddruk g aat omhoog, zweten, trillende handen; als gevolg van omhoog schieten van de stof adrenaline.

ONTWIKKELING HERSENEN EN RISICO OP PATHOLOGIE ¡Stress kan bij kinderen het achterblijven van de groei van de mediaal prefrontale schors veroorzaken. Hierdoor is de kans op depressie en angststoornissen in het latere leven mogelijk groter

¡Door stress ontstaat een tijdelijke verhoging van cortisol. ¡Door langdurige stress langdurige activatie HPA as ¡Ontregeling van de HPA as speelt een belang rijke rol bij ontstaan van aandoeningen zoals depressie (Vreeburg, 2010).

Veel onderzoek uit Japan – Suïcide incidentie 24,4/100,000 – 12,8% van 94 ziekenhuisopnames na TS waren mensen met ASS in (Mikami, 2009) – 7,3% van 587 dossiers met een TS hadden een diagnose ASS (Kato et al, 2013) • Minder TS in de voorgeschiedenis • Meer mannen en alleenstaanden • Minder intoxicaties
• ASS is en risico voor suïcidaliteit – 28% keer verhoogd risico in 791 kinderen met ASS (Mayes et al., 2013) – Retrospectieve analyse 336 TS dossiers: OR=4 (Takara & Kondo, 2014)
• Risicofactoren uit review van 11 studies (Richa et al., 2014) – Misbruik of pesten – Hogere intelligentie – Comorbiditeit depressie of psychose
Recente cohortstudie (2014) • Klinisch cohort 374 volwassenen met stoornis van Asperger (256 mannen, 118 vrouwen) – 35% suïcidale ideaties of pogingen gerapporteerd, wat veel hoger is dan het populatiegemiddelde (OR=9·6 [95% CI 7·6–11·9], p<0·0001) – Bijkomende depressieve klachten werden gezien als risicoverhogend (Cassidy et al., 2014).
Wat weten we uit de praktijk? • Rigide balans als oplossing “A + B = suïcide” • Vaak heel onvoorspelbaar • Vaak na langdurige worsteling met veel lijdensdruk • Uitzichtloosheid

Angst en depressie bij iedereen. • Angst is natuurlijke bescherming tegen gevaar. • Iedereen kan angsten, onzekerheden, negatiever zelfbeeld of depressies ontwikkelen. • Bepalend zijn: – Het individu, genetisch en opvoeding/milieu – De mate van – De tijdsduur
Verhoogd risico voor mensen met ASS • Afwijkende waarden van neurotransmitters zoals serotonine en dopamine. • Gevolgen: – Beperktere communicatie tussen hersencellen, hersendelen. – Beperkter ervaren van geluksgevoelens
Stress verhogende factoren 1 A Centrale Coherentie. - Opslaan van info in fragmenten. Geen gehelen, geen voldoende samenhang. - Daarom is geheugenbeeld onsamenhangend, ongeordend. Moeite met vormen van een realistisch zelfbeeld. Input van buitenaf zéér bepalend. Imitatiegedrag i.p.v. intuïtie.
Stress verhogende factoren 2 B Beperkte Theory of Mind ( TOM ) Moeite om gevoelens, emoties en verwachtingen te “lezen” - Sociale isolatie. Deze toets (5) is nog net niet genoeg, ik help je een stukje. De volgende keer haal je vast een dikke voldoende. Ik merk dat je onrustig wordt, je hebt al heel lang geluisterd. Ga even ….. en dan proberen we die laatste 10 minuten samen vol te houden.

We beginnen met het stimuleren van de sociale initiatief name van de cliënt
Zonder goede afstemming en bereidheid tot samenwerking kan de interventie niet starten. Dus we investeren eerst in het contact, soms betekent dit dat we veel geduld moeten hebben, met name bij kinderen met een laag ontwikkelingsniveau. Bij volwassenen is de vorm van de interventie anders en krijgt het karakter van een sociaal cognitieve training, gericht op het verbeteren van informatie verwerking. De ingrediënten en de opbouw van de interventie blijft gelijk en is gericht op het stimuleren van het verwerken van informatie in samenhang.
