Pink and White Geachte collegae, Als voorzitters van de Nederlandse Vereniging voor Parodontologie (NVvP) en de Nederlands Vlaamse Vereniging voor Restauratieve Tandheelkunde (NVVRT) is het ons een eer u met gepaste trots de éénmalige (echt-)verbintenis te mogen aankondigen van de NVvP en de NVVRT voor het gezamenlijke najaarscongres op vrijdag 24 en zaterdag 25 november 2006. Het congres met het thema “Harmony in Pink and White” zal plaatsvinden in De Doelen te Rotterdam. Harmonie tussen de parodontale en restauratieve aspecten van de tandheelkunde zal leiden tot een verantwoord, duurzaam en esthetisch restauratief concept. Er zijn deskundigen van grote naam uitgenodigd. Dr. Galip Gürel uit Istanboel, Prof. Dr. Markus Hürzeler en Dr. Wolfgang Bolz uit Duitsland en Dr. Bjarni Pjetursson uit IJsland zullen u een interessant programma bieden. De inleidende lezing zal worden gehouden door Dick Barendregt, die samen met Ransom Altman ook de moderatie van het wetenschappelijk programma zal verzorgen. Vrijdagmiddag zullen Oral B, platina sponsor van de NVvP en Philips Oral Healthcare, NVvPsponsorplus 2006 en bedrijfsponsor NVVRT, parallel aan het hoofdprogramma twee interessante lezingen aanbieden. De zaterdag zal in het teken staan van een groot aantal specifieke workshops voor kleine groepen. Die personen die zich reeds via de vooraankondiging hebben aangemeld, krijgen voorrang bij de inschrijving voor de verschillende workshops. Met dit programma wordt het een congres, dat uitermate interessant is voor iedere discipline binnen de tandheelkunde, te weten: de tandarts, de mondhygiënist, de preventie-assistent en de tandtechnicus. Van oudsher heeft de parodontologie zich gericht op behandeling en behoud van parodontaal aangetaste gebitselementen. Met de ontwikkeling van de implantologie in de jaren negentig en de daarmee toenemende vraag voor vaste vervangingen van de parodontaal verloren gegane elementen, heeft implantologie en daarmee de restauratieve tandheelkunde een belangrijke plaats gekregen in de moderne parodontologie. Het opstellen van behandelplannen bij patiënten met een sterk parodontaal aangetaste dentitie waar extracties noodzakelijk zijn, is ondenkbaar geworden zonder een duidelijk restauratieve component. Aan de hand van enkele voorbeelden zal worden aangegeven hoe de afgelopen 8 jaar in een nauw samenwerkingsverband in Rotterdam van tandartsen, tandtechnici, endodontologen, orthodontisten, parodontologen en kaakchirurgen, optimale oplossingen zijn verkregen bij de individuele patiënt. Door het combineren van de complete diagnostiek op elk tandheelkundig terrein bij iedere patiënt kan de correcte volgorde van handelen worden bepaald om zo op de meest efficiënte manier op lange termijn voorspelbare behandelresultaten te verkrijgen.“Trouble shooting” na behandelingen op basis van onvolledige diagnostiek b.v. binnen de implantologie, is een toenemende dagbesteding van verschillende tandartsen in Nederland. Binnen die diagnostiek speelt de academisch opgeleide tandarts een sleutelrol. Door met name vroege diagnostiek in de eigen patiëntenpopulatie kunnen op termijn complexe en moeilijk voorspelbare behandelingen worden voorkomen. Op basis van de eigen ervaring en interesse kan worden besloten complexe behandelingen geheel of gedeeltelijk te verwijzen naar collega’s werkend in een teamverband met expertise op andere tandheelkundige terreinen. Hierdoor kan worden gewaarborgd dat alle aspecten van de tandheelkunde worden afgewogen en niet alleen die waar de interesse van de betreffende tandarts ligt. De presentatie heeft een praktische insteek, namelijk het geven van een mogelijk handvat voor het inventariseren van problemen en het in goede volgorde plaatsen van de behandelopties.
With latest improvements in the field of dentistry, aesthetic approaches are booming in the past two decades. It has been a standard to obtain more and more natural looking restorations in our daily clinical practices. However in doing so, nobody should even underestimate the important role of function. With the introduction of implants and bonded restorations, occlusion is gaining more and more importance from a single on-lay restoration to a full mouth reconstruction. Occlusion and function should always be based on solid foundations. Meanwhile, one should always be able to differentiate scary bruxist patients from those who may only have disfunction that can be easily treated. In this lecture, occlusion as a very integral part of function will be analised and it’s application to our daily work will be discussed.
Curriculum Vitae DICK S. BARENDREGT studeerde in 1988 af als tandarts aan de Rijksuniversiteit van Groningen. Hij is daarna ondermeer werkzaam geweest als tandarts bij de Koninklijke Marine en heeft gedurende 3 jaar in diverse algemene praktijken waargenomen. In 1991 begon hij met zijn Post Academische opleiding in de parodontologie aan het ACTA. Deze opleiding rondde hij in 1994 af. Vervolgens startte hij als tandarts-parodontoloog in de Kliniek voor Parodontologie Amsterdam en de Praktijk voor Parodontologie Utrecht. In 1996 werd door hem de Kliniek voor Parodontologie Rotterdam opgericht. Daar houdt hij zich bezig met de parodontologie en implantologie. De laatste 5 jaar is in dat kader de nadruk komen te liggen op patiënten behandeling in een interdisciplinair teamverband met collega tandartsen zoals orthodontisten, endodontologen, prothetisten, kaakchirurgen en tandtechnici. Collega Barendregt is verder verbonden aan de afdeling parodontologie van het Academisch Centrum Tandheelkunde Amsterdam (ACTA). Daarnaast is hij betrokken bij verschillende postacademische cursussen.

GALIP GÜREL Creating the precision in terms of the preparation, fit and the aesthetic final outcome needs a serious treatment planning which is different for each case... Designing a new smile consists of many steps which are so very important and if followed right, produce predictable success. The key to aesthetic excellence requires patient communication as well as the communication with the laboratory from the beginning of the case till the end. In order to achieve a very precise and predictable tooth preparation, wax-up, silicon indexes and related techniques such as APR (Aesthetic Pre Recontouring), APT (Aesthetic Pre-evaluative Temporaries) and preparation through the APTs are extremely crucial. This eliminates all the possible mistakes and destruction of the depth cutters that could have been done if the tooth was to be prepared without realizing the aging affects on the enamel (volumetric enamel loss) or their unappropriate teeth positions on the dental arch. Curriculum Vitae DR. GALIP GÜREL graduated from the University of Istanbul, Dental School in 1981. He continued his education at the University of Kentucky, Department of Prosthodontics. Dr. Gürel is the founder and current president of EDAD (Turkish Academy of Aesthetic Dentistry). He is a member of the American Society for Dental Aesthetics (ASDA) and the honorary diplomate of the American Board of Aesthetic Dentistry (ABAD) as well as a member of the American Academy of Cosmetic Dentistry (AACD) and an active member of European Academy of Esthetic Dentistry (EAED). He is also the editor-in-chief of the Quintessence Magazin in Turkey and on the editorial board of the AACD journal, PPAD (Practical Procedure & Aesthetic Dentistry) and Spectrum. He has been lecturing on dental aesthetics all over the world and giving post graduate lectures on aesthetics at the NYU and Yeditepe University in Turkey. He is the author of “The Science and Art of Porcelain Laminate Veneers” published by Quintessence publications in 2003. He has been practicing in his own clinic in Istanbul, specializing in Aesthetic Dentistry, since 1984. A reknown sportsman, Dr. Gürel has been the Captain of the Turkish National Team of Water Polo for 110 times as well as the World Champion of the Camel Throphy and a participant of the Paris-Dakar Rally.

There is a growing sense among clinicians that the long-term prognosis of implant therapy demonstrate a higher level then the prognosis of complex periodontal therapy. As a consequence more teeth are extracted and replaced with implants in periodontal patients. This approach is based on two important assumptions: (1) implants perform better than compromised teeth in periodontal patients and (2) implant longevity is independent of the individual susceptibility profile of the specific patient. This presentation will specific address the clinical relevance of the available scientific evidence. A clinical concept will be presented how we in our clinic approach this area of conflict on a daily base.
This presentation will focus on the latest developments in esthetics and microsurgical concepts. It is a clinically-oriented presentation with emphasis on the surgical techniques and instrumentation, as well as practioner effectiveness.

Evidence based evaluation of risks in reconstructive dentistry

In the daily praxis, dentists face the challenge of making difficult clinical decisions. When planning a fixed reconstruction, the most common possibilities are a tooth supported or an implant supported fixed partial denture (FPDs) or single crowns (SC). In the treatment planning, several risk factors have to be taken into account. For tooth and implant supported reconstruction, complications can occur over the years in function. It can be minor complications, which can be corrected or repaired without investing lots of time and effort. But there are also complications, classified as major complications, resulting in a lot of time and effort to be invested or even worse, the reconstruction has to be remade. It has been shown that despite of a high survival of FPDs, biological and technical complications are frequent. Comparing tooth and implant supported reconstruction, the tooth supported have higher risk of biological complications. On the other hand, the implant supported have higher risk of technical complications. As there are no randomized controlled clinical trials in the dental literature comparing tooth supported FPDs to implant supported FPDs, a series of systematic reviews was performed to assess and compare the 5 and 10 year survival of tooth (conventional and cantilever FPDs), combined tooth-implant and solely implant supported FPDs and to evaluate the incidence of biological and technical complications. In this lecture, the result from this systematic reviews will be presented and the different risk factors, that have to be taken into account in treatment planning of fixed reconstruction, will be discussed. Finally, the question will be addressed whether it is possible to practice evidence based treatment planning in prosthetic dentistry. Is there enough evidence available to guide us in choosing between conventional FPDs, cantilever FPDs, combined tooth-implant supported FPDs, solely implant supported FPDs and implant supported single crowns when we do our treatment planning or do we still have to go by the “gut” feeling and our clinical experience?
While the aim of periodontal therapy is to regain periodontal health, the objective of supportive periodontal therapy is the continued preservation of gingival and periodontal stability obtained as a result of the periodontal treatment. Proper self-performed mechanical plaque removal and compliance with needs related-recall visits are a critical component of successful prevention and therapy. However, poor motivation regarding oral health or non-compliance with maintenance protocols negatively influence periodontal stability. The influence of tobacco on oral health has been the subject of numerous scientific studies worldwide over the past few years. Thus it has been well established, that the use of tobacco products has a detrimental effect on both the periodontal tissues and the oral mucosa. Since the dental team is in regular contact with their patients for a number of years, it is quite reasonable for tobacco use prevention and cessation to be incorporated in the clinicians’ daily practice. Similar to poor oral hygiene and tobacco use, the consequences of malnutrition on oral and periodontal health being the increase of decay or periodontal attachment loss have been reported in the literature. All the consequences mentioned above may follow a chronic false behavior which is a wrong habit applied over a long period of time. Successful therapy of these diseases and prevention of oral health need sustained behavior change support in addition to professional dental therapy. For tobacco use cessation, comprehensive behavioural change support combined with either nicotine replacement therapy or bupropion are shown to be the most effective interventions in dental settings. Various strategies for prevention of tobacco use and methods of motivating quit attempts in patients are currently being studied. For all kinds of support provided, it seems to be beneficial when basic communicational skills for patient motivation (i.e. with motivational interviewing), are appropriately being used. Motivational Interviewing (MI) has been defined as, “A client-centered, directive method for enhancing intrinsic motivation to change by exploring and resolving ambivalence” (Miller & Rollnick, 2002). The dental team should be educated and trained with the knowledge and skills necessary to be able to promote and maintain predictable behavior change in relation to instruction of oral hygiene practices, tobacco use cessation and dietary counseling.

Curriculum Vitae DR. DAVID WINKLER received his DDS from The Royal Dental College of Copenhagen, Denmark, in 1983 after having worked as a dental technician for 3 years. He is the founding president of the Scandinavian Academy of Esthetic Dentistry, past-president of the European Academy of Esthetic Dentistry, and a founding member of the British Academy of Aesthetic Dentistry. He is a member of the International College of Prosthodontists and a Fellow of the International College of Dentists. Betaling
De congreskosten zijn inclusief pauzeconsumpties, lunch en borrel. Bij de deelnemers met een bank- of girorekening in Nederland worden door het invullen en ondertekenen van deze éénmalige machtiging de congreskosten omstreeks 20 oktober 2006 van uw rekening afgeschreven door de B.V. Diensten NVvP te Avenhorn (bankrekeningnummer 58 81 40 457). De buitenlandse deelnemers ontvangen een factuur. Zij dienen de verschuldigde congreskosten over te maken aan B.V. Diensten NVvP te Avenhorn, BIC code: ABN ANL2A, IBAN: NL 49 ABN A058808140457.
Annulering
Annulering van inschrijving dient schriftelijk te geschieden bij VVAA congres- en evenementenservice. Bij annulering vóór 20 oktober 2006 wordt € 30,- administratiekosten in rekening gebracht. Bij annulering vóór 1 novermber 2006 vindt restitutie plaats: 70% van congreskosten. Bij annulering vóór 15 november 2006 vindt restitutie plaats: 50% van de congreskosten. Bij annulering na 15 november 2006 vindt géén restitutie plaats.
Bevestiging
Bij inzending ná 20 oktober 2006 geldt een toeslag van € 40,-. Uw inschrijving wordt binnen 2-4 weken bevestigd. 12

Routebeschrijving De Doelen Bereikbaarheid en parkeren Trein, tram, bus en metro stoppen bij de Doelen voor de deur. Bovendien biedt de omgeving van de Doelen ruime parkeermogelijkheiden. Openbaar vervoer Concert- en congresgebouw de Doelen bevindt zich op loopafstand (5 minuten) van het Centraal Station in de binnenstad van Rotterdam. Metro - halte Centraal Station (begin- en eindpunt), alle lijnen. Tram - halte Weena en Kruisplein, lijn 1, 4, 5, 7, 8. Bus - halte Weena, lijn 33, 36, 37, 38, 44, 49; halte Centraal Station, lijn 55 Het Station en het Stationsplein worden op dit moment ingrijpend vernieuwd. Ook bezoekers van de Doelen hebben hiermee te maken. Kies ‘Metamorfose Stationsplein’ voor meer informatie. Auto uit Amsterdam / Den Haag A13 richting Rotterdam, bij Kleinpolderplein richting Centrum volgen, bij tweede stoplicht borden Euromast/Maastunnel volgen, na stoplicht rechtertunnel nemen, bij stoplicht links. U komt nu uit op het Weena. Zie verder de plattegrond (Route A). Auto uit Utrecht A20 richting Den Haag / Hoek van Holland, afslag Rotterdam Centrum / Schiebroek / Hillegersberg, bij einde afslag borden Centrum volgen (Schieweg / Schiekade). U komt nu uit op het Hofplein. Zie verder de plattegrond (Route B). Auto uit Breda / Dordrecht Kies op de A16 de rechterbaan (volg Kralingen / Rotterdam Centrum). Rijd over de Van Brienenoordbrug, eerste afslag (Rotterdam Centrum). Onderaan de afslag linksaf (rotonde richting Centrum), de Maasboulevard op. Rechtdoor (langs de Boompjes). Bij Hotel Inntel (aan uw rechterhand) gaat u rechtsaf de Schiedamsedijk op. Volg deze tot aan de kruising Coolsingel-Westblaak. Ga op deze kruising naar links de Westblaak op of rijd rechtdoor richting Hofplein. Zie verder de plattegrond (route C). Fiets stallen Uw rijwiel kunt u stallen in de onbewaakte stalling van Parking Schouwburgplein onder het Schouwburgplein, te bereiken via de (auto)ingang aan de Schouwburgzijde. Bij het Centraal Station is een bewaakte fietsenstalling. Parkeren In de buurt van de Doelen zijn zes parkeergarages op loopafstand. (zie pag. 16)
